Arthroscopic fixation techniques for tibial eminence fractures in pediatric patients: a review.

The introduction of new internal fixation devices and arthroscopic techniques has led to significant changes in the surgical treatment of tibial eminence fractures (TEFs) in children. In recent years, arthroscopic surgery has arisen as the gold standard for the treatment of TEFs. This popularity of arthroscopic techniques has reduced surgical complications and improved patient prognosis. In this paper, we investigate the current situation of the use of arthroscopic fixation techniques for pediatric TEFs. We searched the PubMed database using the terms "arthroscopic treatment and tibial eminenc

Safety-string: A handy maneuver to control pectus bar removal bleeding complications.

The pectus bar removal procedure is often considered a simple, straightforward surgery with a low incidence of complications. However, life-threatening bleeding complications have been reported requiring major measures for hemostatic control. Our objective is to share a simple maneuver that our group has systematically included in the bar removal procedure to facilitate bleeding control in case hemorrhage occurs.

All-inside Arthroscopic Meniscal Repair Technique Using a Midbody Accessory Portal.

Treatment of symptomatic meniscal tears continues to evolve as we improve our understanding of the biomechanical role of the meniscus and its long-term importance to the health of the knee joint. Suture repair of meniscal tears is challenging, yet the incidence of repairs among our colleagues continues to rise as we aim to preserve meniscal tissue. Many elements of performing a repair are tedious and difficult, including proper meniscal preparation, reduction, mattress suture placement, and fixation. The tear pattern and location present another layer of difficulty. The most widely used all-in

An appliance that permits early mobilization after correction of mandibular prognathism.

With certain orthognathic surgery patients, such as epileptics, severe asthmatics, or those with other medical problems that increase risk during intermaxillary fixation, there occasionally may be an indication for a reduced fixation time. By using Vitallium mesh for stability, immobilization for 2 to 2 1/2 weeks has proved to be sufficient in a limited number of cases. Mobilization before two weeks has not been attempted. The procedure, however, is not recommended as a routine method of fixation.

Management of acute scaphoid fractures.

Scaphoid fractures are a common injury in young, active populations. Non-displaced fractures have a high union rate if promptly treated with cast immobilization. Displaced fractures and proximal pole fractures are best treated with operative fixation. There are a variety of techniques and implants available to the surgeon. The surgical approach and fixation device should be based on fracture characteristics and surgeon experience. Operative fixation of non-displaced fractures is an option in patients who would prefer to avoid prolonged cast immobilization.

Use of a New External Fixator for the Correction of Fixed Flexion Deformity of the Fingers.

Three patients with severe fixed flexion deformity of the fingers caused by trauma were reported. As an alternative treatment, a new mini external fixator was used to correct the deformities. The use of this device has made treatment of contractures simpler and more predictable. Gradual distraction and straightening was applied to loosen and extend the joints, followed by a period of mobilisation while still on the device. In all the three patients, this experimental device and approach led to improvement of the fixed flexion deformities. With use of this device, surgical release may not be ne

[Treatment of distal radius extra-articular malunion in young active patients].

Malunion is a common complication of distal radius fractures despite new fixation devices. The authors discuss the surgical indications, preoperative management and the various surgical techniques used to treat distal radius extra-articular malunion, specifically in active young subjects. The treatment goals in this population group are anatomical restoration of the distal radius and distal radioulnar joint, short immobilization period, quick functional recovery and stable results over time, consistent with high functional demands.

[Application of Dick's Internal Fixation Device in the Treatment of Injuries of the Thoracolumbar Spine, First Experience.].

The authors treated 12 patients with injuries of the thoracolumbar spine, using an internal fixation device as designed by Dick. The advantage of Dick's device is: 1. fixation restricted to the smallest possible portion of the spine, usually only two mobile segments, 2. the possibility to influence the so-called anterior column of the spine from a posterior approach, 3. the possibility of peroperative reposition, 4. the high stability of the fitting, which makes possible early postoperative mobilization of the patient only with a removable orthesis. The internal fixation device makes it possib

A Novel Navigation Device for Precise Percutaneous Placement of the Guidewire in Femoral Neck Fracture Cannulated Screw Fixation Surgery.

The accuracy of screw placement is a key factor for the stability of the cannulated screws used in the fixation of femoral neck fractures. In this study we designed a navigation device for ensuring the screw reaches the ideal position for optimal fixation. From March 2019 to September 2020, 66 patients with femoral neck fracture were enrolled and divided into 2 groups, one group was treated using the traditional free-hand cannulated screw fixation and the other using the new navigation device with assisted fixation. The effectiveness of the 2 methods was compared based on surgery duration, int

Checkrein Deformity of the Hallux Caused by Misplaced Suture-Button Syndesmosis Fixation: A Case Report.

Ankle fractures are common injuries. If an ankle fracture is combined with tibiofibular syndesmosis separation, then tibiofibular transfixation should be used to stabilize the syndesmosis; in such cases, flexible (suture-button) fixation is an option. We surgically treated a male patient's unstable ankle fracture with internal fixation devices, including a suture-button fixator, and observed the development of a flexor hallucis longus checkrein deformity in the postoperative period. The aim of this report is to describe this rare postoperative complication associated with the use of suture-but

Double calcaneal osteotomy using single, dual-function screw fixation technique.

Historically, a variety of fixation techniques for calcaneal osteotomies have been used. The prevention of postoperative sequelae, including delayed or nonunion of the osteotomy sites and minimization of soft tissue damage, is always a primary consideration for foot and ankle surgeons. The present report describes a fixation technique for a double calcaneal osteotomy that entails use of a single screw that functions as both a positional tool, as well as a compressive fixation device. This technique provides compressive and positional qualities and decreases the amount of soft tissue disruption

Managing disorders of the cervicothoracic junction.

A wide range of pathologic conditions occur at the cervicothoracic junction. These conditions are usually the result of trauma, neoplastic processes, infection, prior surgery, or degenerative changes. Instability in this region of the spine is difficult to manage, particularly because of the complex biomechanics involved and the challenging surgical approaches required for treatment. Traditional radiologic evaluation of the cervicothoracic junction is often inadequate; as a result, the standard 3-view cervical spine series should be augmented with swimmer's or oblique views. Surgical treatment

Periprosthetic femoral fractures above total knee replacements.

Periprosthetic femoral fractures above total knee replacements can be managed by a variety of methods, including casting, open reduction and internal fixation, external fixation, or revision arthroplasty. Because no single method has emerged as the optimal choice for all such fractures, it is important to understand which options are appropriate for each fracture pattern. Early classification systems focused on displacement as a major indication for either surgical or nonsurgical management. However, recent techniques and current implants have made surgical management preferable for most perip

Transpalatal distraction as a method of maxillary expansion.

Conventional devices used for surgically assisted rapid palatal expansion are tooth-borne. Dental fixation entails a number of possible drawbacks such as loss of anchorage, skeletal relapse during and after the expansion period, cortical fenestration and buccal root resorption. A bone-borne titanium device with interchangeable expansion modules, used with a callous distraction policy, is presented.

Role of corpectomy in cervical spondylosis.

The management of cervical spondylosis has evolved over the past several decades. Surgical decompressive and stabilization techniques have become more widely accepted for use in patients with intractable pain or neurological deficits. Advances in neuroimaging, surgical technique, and surgery-related technology including the operating microscope and anterior fixation devices have all contributed to the expanding role of surgery for the treatment of this condition. In this paper the author will focus on the role of corpectomy as a surgical option for managing cervical spondylosis.
