[Results of surgical correction of congenital talipes equinovarus].

Different surgical procedures in the treatment of clubfoot were analyzed, especially in correlation to over-correction and inadequate correction. Indications for surgery, surgical errors and their influence on outcome were assessed. 82 children (28 females and 54 males) with 130 congenital equinovarus underwent surgery between 1988 and 1994. Age at the time of operation ranged from 6 to 13 months (average: 9 months). Posterior release (i.e. partial subtalar release) was conducted in 68 cases of clubfeet and complete subtalar release in 62 cases. During follow-up 44 children with 64 clubfeet we

Histological abnormalities of the calcaneum in congenital talipes equinovarus.

Six calcaneal fragments from patients aged 2, 3, 4, and 5 years with relapsed talipes, and two normal feet from a 40-week-old stillborn fetus were studied. All tissue was sectioned in the sagittal or coronal plane and stained using alcian blue and sirius red to distinguish cartilage and bone. Immunocytochemistry was performed to illustrate collagen types I and II. Within the clubfoot calcaneum, there were fewer chondrocytes and a diminished number of cartilage canals. Although a growth plate was present, the zones of differentiated chondrocytes were not apparent and the chondrocytes were small…

Sinus tarsi syndrome in a patient with talipes equinovarus.

The literature reports that 70% of the cases of sinus tarsi syndrome are post-traumatic, following an inversion sprain, and that 30% result from inflammatory disorders, such as rheumatoid arthritis, ankylosing spondylitis, and gouty arthritis. However, in the case presented, talipes equinovarus deformity and sinus tarsi syndrome coexisted. One of the corrective goals in the management of the talipes equinovarus deformity is the realignment of the articulation between the medial plantarly deviated talar head and the anteromedial segment of the calcaneus. The calcaneus must be rotated from a pla…

Results of surgical treatment of talipes equinovarus congenita.

A series of 153 feet in 103 patients were surgically treated for idiopathic clubfeet (mean follow-up period of 10.3 years). Thirty-four percent had prior surgery before referral to the authors' institution. Functional results were excellent in 28.7%, good in 37.9%, fair in 13.1%, and unsatisfactory in 20.3%. The complication rate was 7.4%. Serious complications included translocation of os calcis, avascular necrosis of the talus, and injuries to neurovascular structures. Pain after strenuous activities was noted in 11.3% of the feet. In 9.8% of feet, limitation of activities was noted by the p

Use of the Cincinnati incision in congenital talipes equinovarus.

A series of 16 patients (24 feet) with idiopathic congenital talipes equinovarus (CTEV) corrected by a Cincinnati incision is presented. Minimum follow-up was 2 years. This series demonstrated that the incision does not adversely affect the outcome, since 75% of these feet had a good or excellent correction. No significant wound problems were encountered. The advantages of the Cincinnati incision, together with the fact that no adverse sequelae resulted from its use, mean that it can be recommended for future use and should be taught to other physicians.

The management of congenital talipes equinovarus in developing countries.

Congenital Talipes Equinovarus often remains untreated in developing countries. At our Institute in Calcutta, where we have modern facilities, a programme was undertaken using simple techniques in selected cases. Since 1964, 5,312 cases of congenital talipes equinovarus were treated by percutaneous elongation of the Achilles tendon, plantar fasciotomy and corrective plasters. In 4,502 cases the results are good enough to recommend this technique, even in developed countries.

The early treatment of talipes equinovarus with adhesive taping.

Adhesive taping provides an effective form of early treatment of talipes equinovarus. This paper describes the technique of taping in detail. Its main advantage over rigid immobilization is that continuous stretching can be carried out during the newborn period. The basic principles of early diagnosis and treatment do not vary from other treatment programs. However, taping and continuous stretching (followed as needed by cast and surgical treatment) may offer some advantages.

Magnetic resonance image findings in congenital talipes equinovarus.

Magnetic resonance imaging (MRI) was used to image the feet of 10 infants with congenital talipes equinovarus. Images in an intermediate density and T2 sequence are most useful. In the transverse plane, the primary deformity appears to be localized to a medical deviation of the talar neck and head, with an accompanying internal rotation of the calcaneus relative to the talar body. The posterior calcaneus is laterally deviated by this rotation. In the sagittal plane, the deformity appears to lie in a parallelism in the subtalar relationship.

Reverse club foot. Rigid and recalcitrant talipes calcaneovalgus.

We describe a congenital deformity of the foot which is characterised by calcaneus at the ankle and valgus at the subtalar joint; spontaneous improvement does not occur and serial casting results in incomplete or impermanent correction of the deformities. Experience with five feet in four children indicates that release of the ligaments and tendons anterior and lateral to the ankle and lateral to the subtalar joint is the minimum surgery necessary; subtalar arthrodesis may be required in addition. The foot deformity described may occur as an isolated condition or in association with multiple c

Lack of seasonal variation in idiopathic talipes equinovarus.

BACKGROUND: One proposed etiology of idiopathic talipes equinovarus is an in utero enterovirus infection. Enterovirus infections demonstrate seasonal variation in temperate climates. METHODS: We collected data on 1202 children with idiopathic talipes equinovarus born in the Northeastern United States, Midwestern United States, and the United Kingdom to investigate a seasonal variation in this congenital deformity. Birth date, gestational age at the time of delivery, gender, race, and laterality were tabulated and subjected to univariate and bivariate analyses. RESULTS: There were 774 boys an…

[Neurotomy of the tibial nerve for treatment of the talipes equinovarus].

OBJECTIVE: To explore indication and approaches of neurotomy of tibial nerve for the treatment of the talipes equinovarus and spastic paralysis. METHOD: A total of 25 patients including 15 patients with cerebral paralysis and other 10 patients were diagnosed with clear etiology, spastic muscles with supplied nerves were analyzed in detail, neurotomy of the tibial nerve was performed under microscope, during operation, motor nerve was lovated with electronic stimuli and monitored with induced electromyography. The surgical results were analyzed. RESULTS: A total of 36 of 37 (97%) neurotomy of…

Relapse in staged surgery for congenital talipes equinovarus.

Between 1988 and 1995, we studied 91 club feet from a series of 120 recalcitrant feet in 86 patients requiring surgical treatment. There were 48 boys and 20 girls. The mean age at operation was 8.9 months. Surgery consisted of an initial plantar medial release followed two weeks later by a posterolateral release. This strategy was adopted specifically to address the problems of wound healing associated with single-stage surgery and to ascertain the rate of relapse after a two-stage procedure. Immobilisation in plaster was used for three months followed by night splintage. The feet were classif
