BACKGROUND: The repair of retracted flexor tendons is a challenging problem for hand surgeons. The tendon stump should be handled in an atraumatic manner because any microtrauma to the sheath and tendon can lead to poor functional outcomes.
METHODS: Twenty-three patients with flexor zone 2 injuries and intraoperative finding of retracted tendons were randomly divided into two groups: endoscopic retrieval group and proximal incision group. A flexible endoscope and a flexible grasping forceps were used for endoscopic retrieval of the retracted flexor tendons. The groups were compared in terms o

BACKGROUND AND AIM OF THE WORK: Quadriceps and patellar tendon rupture are relatively uncommon but can result in a disabling condition if untreated. We retrospectively review all our cases treated with suture anchors from 2014 to 2018, to evaluate midterm outcome of this technique.
METHODS: Traumatic and atraumatic quadriceps and patellar tendon preinsertional lesions were acutely treated with Healix Ti and FaStin RC 5mm suture anchors and an aggressive rehabilitation protocol was prescribed to patients.
RESULTS: Good to excellent results according to the Modified Cincinnati Rating System Qu

The function of ligaments and tendons is to support and transmit loads applied to the musculoskeletal system. These tissues are often able to perform their function for many decades; however, connective tissue disease and injury can compromise ligament and tendon integrity. A range of protein and non-protein constituents, combined in a complex structural hierarchy from the collagen molecule to the tissue and covering nanometer to centimeter length scales, govern tissue function, and impart characteristic non-linear material behavior. This review summarizes the structure of ligaments and tendon

PURPOSE: Peroneal tendon tears are relatively common injuries that seem to have a poor healing tendency. The discussion goes that peroneal tendons have avascular zones, contributing to the poor healing of those tears. The purpose of this study was to provide evidence on the vascularization pattern of the peroneal tendons.
METHODS: Ten adult fresh-frozen cadavers were obtained from a university-affiliated body donation programme. The femoral artery was injected with natural coloured latex at the level of the knee. Macroscopic and microscopic dissections were performed to visualize the vascular

Chronic finger extensor apparatus injuries are the result of the initial acute treatment having failed or being flawed. Because of their chronic nature, these injuries present various amounts of tendon retraction, tendon callus lengthening, peritendinous scar adhesions, static and dynamic imbalances with the flexor apparatus and intrinsic muscles, and joint contractures. This article will review the anatomy of the extensor mechanism and then will outline by location, the various clinical pictures that are secondary to chronic tendon injury. The clinical presentation of these injuries can be hi

This report presents the case of a 44-year-old man who presented with elective bilateral carpal tunnel decompression. At the operation, he was found to have bilateral palmaris profundus tendons within the carpal tunnel, impinging on the median nerve. In releasing both carpal tunnels, the patient's symptoms were alleviated and there was regain of full function. There have been very few documented cases of these anomalous tendons implicated in carpal tunnel syndrome and this case highlights how such anatomical variations are important in the surgical approach to carpal tunnel decompression.

BACKGROUND: In overhead sports like volleyball, the onset of a rotator cuff tendinopathy due to functional overload is a common observation. An angiofibroblastic etiopathogenesis has been hypothesized, whereby a greater anaerobic metabolism occurs in critical zones of the tendon with a lower degree of vascularization; this would induce collagen and extracellular matrix degradation, that could then trigger a compensatory neovascularization response. We performed a clinical observational study of 80 elite volleyball players, monitoring the perfusion values of the supraspinatus tendons by oximetr

The biceps muscle of the arm is even well known to laymen because of its partially prominent appearance. It fulfills a very important function in the arm due to its capacity for flexion and supination. Lesions of the short proximal biceps tendon are a rare occurrence. The long biceps tendon that runs through the glenohumeral joint is a frequent source of pain. Tenotomy with or without tenodesis is often a judicious treatment approach. Spontaneous ruptures due to underlying degenerative changes are also frequently observed. The resultant loss of strength in forearm flexion and supination is how

Tears and tendinopathy of the gluteus medius and minimus are potentially underrecognized clinical sources of hip pain. Magnetic resonance imaging is useful in diagnosing gluteal tears. This entity is frequently a result of predisposing conditions but may arise spontaneously. This clinical problem should be considered in the differential diagnosis of patients presenting with acute hip pain. In this article, we report a case of spontaneous rupture of the gluteus medius and minimus tendons in a previously healthy patient with no prior hip symptoms.

The complexity and intricacy of hand function are reflected by the anatomy. Extensor muscles have a relatively consistent architecture but also have notable anatomic variations of their tendons, particularly on the ulnar side of the hand. The extensor tendons, juncturae tendinum, intertendinous fascia, and soft tissue function as a plexiform construct to provide stability during power grip and allow for laxity in performing independent fine finger tasks. The greater independence of index finger motion relates to its two tendons, one (EDC-index) with a thin transverse juncturae tendinum, the ot

An 18-year-old woman with congenital flexion contractures of the ulnar three digits bilaterally is presented. She had a three-generation family history of this disorder with autosomal dominant inheritance. Magnetic resonance imaging scan of the wrist and forearm was used to demonstrate a hypoplasia of the digital extensor muscles and tendons.

To distinguish between morbidity caused by harvesting semitendinosus and gracilis tendons and morbidity associated with anterior cruciate ligament reconstruction surgery, we performed a prospective randomized study using 65 patients who underwent anterior cruciate ligament reconstruction using these tendons. The patients underwent either contralateral (N = 34) or ipsilateral (N = 31) graft harvest. For the nonoperated knees in the ipsilateral harvest group, isometric and isokinetic strength of the quadriceps and hamstring muscles increased to approximately 120% of the preoperative value at 12

One hundred eighty-one dissected hands were examined to study the pattern of extensor tendons on the dorsum of the hand. Extensor digitorum often had multiple tendons for the middle and ring fingers. Its contribution to the little finger was usually by a bifurcating tendon common with that of the ring finger. The index finger always received a single tendon. Intertendinous connections between the various tendons of the extensor digitorum were variable but were most frequent between ring and middle fingers. Extensor indicis had one tendon in most of the specimens and it was always on the ulnar

Massive scarring of the Z-lengthened flexor digitorum and flexor hallucis longus is a constant finding in clubfoot surgery. A method of fractional lengthening of the tendons is described. This method has been proven effective in preventing this complication.

Recent reports on the outcome of sports injuries have pointed out that ankle sprains may cause long term discomfort, this occasionally being due to peroneal tendon dislocation which may become chronic. The present paper reports a method, which includes deepening of the retromalleolar groove and at the same time maintaining the cartilagenous gliding layer. Long term follow-up (2 to 10 years) is presented on four patients. One of the patients returned successfully to international track and road races. In each case, the clinical result after the operation was successful, the ankles showing norma

A total of 32 digits, including 3 thumbs, were reconstructed in an outpatient clinic. The patients were allowed to go home 3-4 hours after the operation. The patients had injury to the flexor tendon in no man's land without other complications except for an injury to the digital nerve in 6 cases. The tendon was reconstructed secondarily using a free graft. The minimum follow-up time was 6 months. The results were classified in three ways. With all methods the results, which were deemed satisfactory in 79% and poor in 21%, reached the average standard for this type of repair. Increasing age of

A case of isolated traumatic avulsion of the tendon of extensor carpi radialis longus (ECRL) is described. Surgical repair was attempted 12 days later, but an early contracture of the muscle was encountered. The tendon could not be pulled down to its insertion and was sutured side to side to the tendon of extensor carpi radialis brevis (ECRB). A year later the patient had residual weakness of wrist extension and hand grip. The likely cause of the early contracture of the muscle and the need for early and accurate reposition of extensor carpi radialis longus tendon are described.
