Determinants of time to first palliative care consultation in hospitalized patients with advanced or terminal illness - A retrospective analysis.

BACKGROUND: Diagnosis and sex-related biases can affect clinical decision-making in serious illness, including the recognition of palliative needs. Such biases may influence when and how patients receive palliative care (PC). This study aimed to investigate differences in timing and frequency of PC consultations by diagnosis, comorbidities, age, and sex among hospitalized patients. METHODS: This study retrospectively analyzed 918 patients (50.5 % female) who received a PC consultation during hospitalization between 2016 and 2022. Time from admission to first PC consultation was analyzed using

Midwifery tutors' capacity and willingness to teach contraception, post-abortion care, and legal pregnancy termination in Ghana.

BACKGROUND: Ghana has a high maternal mortality rate of 540 per 100 000. Although abortion complications usually are treatable, the risks of morbidity and death increase when treatment is delayed. Delay in care may occur when women have difficulty accessing treatment because health care providers are not trained, equipped, or willing to treat the complications of abortion. Gaps in the midwifery tutors' knowledge on comprehensive abortion care (CAC) have resulted in most midwives in Ghana not knowing the legal indications under which safe abortion care can be provided, and lacking the skills an

Use of red blood cell transfusions in terminally ill cancer patients admitted to a palliative care unit.

Anemia is often associated with neoplastic disorders. Blood transfusions are used to alleviate the discomfort of anemic cancer patients. Of 246 terminally ill cancer patients admitted to our palliative care unit from October 1991 to December 1993 (128 women and 118 men), 31 patients (12.6%) (17 men and 14 women; age, 69.5 +/- 12 years) received on average 2.8 units of packed red blood cells (PRBCs) (range, 2-7 units/patient) in 35 separate admissions. PRBCs were transfused in the presence of low hemoglobin (Hb) levels ( < or = 8 g/dL) and/or severe fatigue or dyspnea. Pre-transfusion performan

Exploring Home-Visit Rehabilitation for Terminally Ill Patients With Cancer: Insights From Japanese Care Managers.

BACKGROUND: Care managers (CMs) play a vital role in planning, managing, providing family support, and coordinating services for individuals in need, particularly in the field of palliative care. To implement home-visit rehabilitation for terminally ill patients with cancer effectively, it is essential for the care plan created by the CM to be incorporated into the system. Collaboration with the CM is key to ensuring the successful implementation of rehabilitation. Despite this necessity, there is a lack of studies investigating the background and challenges preventing progress in this area. T

Effective practices and transdisciplinary team-based approaches in home palliative care for terminal cancer patients: a qualitative descriptive study.

BACKGROUND: In Japan, approximately 60% of patients with cancer prefer to spend their final days at home, yet only 11.8% die at home. Contributing factors include uneven distribution of community resources and difficulties in multidisciplinary collaboration. We aimed to qualitatively identify key practices in home-based palliative care that should be shared among professionals within a transdisciplinary team approach, which may offer solutions to these challenges. METHODS: Between November 2021 and March 2022, semi-structured interviews were conducted with 13 professionals—physicians, nurses,

Estimating the short-term prognosis to adjust the transfer of patients with terminal cancer to medium-stay palliative care units.

BACKGROUND AND OBJECTIVES: Clinical management for terminal patients should consider various aspects, particularly the patient's functional assessment, which correlates well with the short-term prognosis. The prognosis could improve if the presence of symptoms strongly associated with a poorer progression were included. The study's main objective was to assess whether the prognosis according to the Palliative Performance Scale (PPS) improved with the presence/absence of pain-dyspnoea-delirium symptoms. The secondary objective was to determine caregiver satisfaction with the transfer to medium-
