Timing of repair of blunt thoracic aortic injuries in the thoracic endovascular aortic repair era.

OBJECTIVE: Thoracic endovascular aortic repair (TEVAR) is the preferred operative treatment of blunt thoracic aortic injuries (BTAIs). Its u … benefit persisted across injury severity groups and was independent of the presence of serious extrathoracic injuries. Logistic regression analysis, adjusting for age ≥65 years, Glasgow Coma Scale score ≤8, systolic blood pressure ≤90 mm Hg at admission, and serious extrathoracic injuries, showed a higher adjusted mortality in group 1 (odds ratio, 2.54; 95% confide …

Thoracic endovascular aortic repair for blunt thoracic aortic injuries in complex aortic arch vessels anatomies.

The aim of this study is to report the use of thoracic endovascular aortic repair (TEVAR) in blunt thoracic aortic injuries (BTAIs) presenting with complex anatomies of the aortic arch vessels. Two patients were admitted to our hospital for the management of BTAI. Anomalies were as follow: aberrant right subclavian artery (n = 1) and right-sided aortic arch with 5 vessels anatomy variant (n = 1). TEVAR was accomplished using parallel graft with periscope configuration in the patient with the aberrant right subclavian artery. At 12-month follow-up, computed tomography angiographies confirmed th

[Heart injury following closed thoracic injury].

A 60-year-old man, was admitted in the emergency ward, following a motor vehicle accident. At the time of arrival his clinical state was stable. The initial investigations showed a moderate left haemopneumothorax and fractured ribs. After insertion of a thoracostomy tube into the left pleural cavity he had to undergo surgery for an open fracture of the left arm. Following induction of anaesthesia, a cardiovascular collapse occurred rapidly. An emergency thoracotomy was performed which showed a right ventricular perforation by a rib fragment. The authors discuss the role of possible changes in

Imaging of thoracic aortic injury.

Isthmic aortic rupture or disruption should be systematically sought when there is high kinetic energy trauma to the thorax. This condition is extremely serious and life threatening. It needs to be diagnosed rapidly but diagnostic pitfalls must be avoided. CT angiography is the standard examination. The main CT signs of rupture or disruption of the thoracic aorta are periaortic hematoma, intimal flap, pseudo-aneurysm and contrast agent extravasation. There are three types of lesion: intimal, subadventitial or pseudo-aneurysmal, and complete rupture with lesion of the three tunicae, and it is i

Blunt thoracic aortic injury.

Blunt thoracic aortic injury (BAI) is a rare but often fatal injury that occurs with severe polytrauma. Immediate diagnosis and treatment of BAI are essential for a successful outcome. We reviewed our experience with 20 patients with BAI treated at a Level I trauma center between 1995 and 2006. The mean Injury Severity Score was 38 +/- 14 and 14 patients had an abnormal Glasgow Coma Score; associated injuries included abdomen in 13 patients, extremity in 12, and head in six. Chest x-ray (CXR) findings were suggestive of aortic injury in 15 patients, equivocal in three, and showed no evidence o

Long thoracic nerve injury.

Injury to the long thoracic nerve causing paralysis or weakness of the serratus anterior muscle can be disabling. Patients with serratus palsy may present with pain, weakness, limitation of shoulder elevation, and scapular winging with medial translation of the scapula, rotation of the inferior angle toward the midline, and prominence of the vertebral border. Long thoracic nerve dysfunction may result from trauma or may occur without injury. Fortunately, most patients experience a return of serratus anterior function with conservative treatment, but recovery may take as many as 2 years. Bracin

A war injury to the thoracic trachea.

The authors present a case of war injury to the thoracic trachea with the defect involving more than half of its circumference. After minor difficulties in establishing the right diagnosis, we passed to the surgical procedure and managed the lesion by simple suture. The patient's postoperative course at the Intensive Care Unit was uneventful. Both early and late follow-up roentgenograms, computerized tomography of the trachea and bronchoscopy showed normal findings.

Thoracic great vessel injury.

Thoracic great vessel injury may be secondary to blunt, penetrating, blast, or iatrogenic trauma. A surgeon should be the initial evaluator of and decision maker for these patients, and the aortogram remains the gold standard for specific diagnosis of the arterial injuries except in those patients requiring emergency thoracotomy. Two general types of incisions are employed for these injuries: resuscitative and elective.

[Traumatic Thoracic Aortic Injury].

Traumatic thoracic aortic injury is a life-threatening event and needs emergency treatment. Blunt aortic injury( BAI) is one of the major traumatic aortic injuries and is the second most common cause of death due to non-penetrating trauma. Most of the BAI patients die before reaching the hospital. Patients who survive until hospital arrival might be salvageable, however, multiple injuries can progress to lethal aortic rupture without definitive treatment. We present 4 cases of BAI, who successfully treated by stent graft repair in 2 cases and open surgical repair in 2 cases. We highlighted the

Thoracic and retroperitoneal lymphatic injuries following spine surgery: illustrative cases.

BACKGROUND: Postoperative lymphatic leakage is a rare complication of spine surgery caused by injury to the thoracic duct, cisterna chyli, or retroperitoneal lymphatic channels. Because of its rarity, diagnosis is often delayed and optimal management remains poorly defined. The authors present 3 illustrative cases and review current diagnostic and therapeutic strategies. OBSERVATIONS: A 66-year-old man developed a right chylothorax after posterior T2-3 corpectomy with costotransversectomy and cervicothoracic fusion for metastatic spinal cord compression. Following failed thoracic duct emboliz

[Traumatic injuries of the thoracic aorta-Role of interventional radiology].

Acute aortic injuries are among the most life-threatening emergencies in vascular and emergency medicine. They are mostly caused by high-velocity trauma with deceleration and shear forces in the region of the aortic isthmus. Due to the anatomical features these particularly result in tears or complete transection of the vascular wall. Without rapid diagnosis and treatment the majority of cases are fatal. Contrast-enhanced computed tomography (CT) angiography is considered the diagnostic gold standard and enables a precise classification of the injury according to the severity and immediate tre

Caustic ingestion: CT findings of esophageal injuries and thoracic complications.

Ingestion of caustic substances, whether accidental or for the purpose of suicide, can cause severe lesions of the lips, oral cavity, pharynx, upper gastrointestinal system, and upper airways. In particular, caustic agents could be responsible for severe esophageal injuries resulting in short- and long-term complications. Because of these important clinical implications, timely diagnosis and appropriate management are crucial. In the evaluation of esophageal injuries, thoraco-abdominal computed tomography (CT) is preferable to endoscopy as it avoids the risk of esophageal perforation and allow

Functional outcomes of thoracic injuries in pediatric and adult occupants.

OBJECTIVE: To develop a disability metric for motor vehicle crash (MVC) thoracic injuries and compare functional outcomes between pediatric and adult populations. METHODS: Disability risk (DR) was quantified using Functional Independence Measure (FIM) scores within the National Trauma Data Bank (NTDB) for the top 95% most frequently occurring AIS 2, 3, 4, and 5 thoracic injuries in NASS-CDS 2000-2011. The NTDB contains a truncated form of the FIM score, including three items (self-feed, locomotion, and verbal expression), each graded from full functional dependence to full functional independ…

Penetrating thoracic injuries - treatment of two patients after suicide attempts.

Thoracic injuries are usually caused by penetrating or blunt trauma. The primary method of treatment is surgery. This study describes two cases of male patients with stab wounds of the chest resulting from suicide attempts. The first case involved a 29-year-old patient transported and admitted to the hospital with a knife still in his chest; its blade extended from the jugular notch to the 5(th) thoracic vertebra but did not damage any important structures. The applied treatment, limited to evacuating the knife, resulted in a satisfactory outcome, and the patient was discharged from the intens…

[Tracheobronchial injuries due to blunt thoracic trauma: 10 years experience].

The tracheobronchial injuries are seldom, but life threatening. Their successful diagnosis and treatment require a high level of suspicion. In this report, we reviewed our experience with seven patients with tracheobronchial injuries due to blunt thoracic trauma, treated over the past 10 years. Definitive diagnosis was recognized with bronchoscopy after clinical suspicion. Most injuries were located in the right bronchial tree (five patients), whereas only one patient had an injury located in the left bronchial tree and one in trachea. The majority of the injuries were repaired primarily (five
