A surgeon managing complex airway disease benefits from knowledge of laryngeal physiology and pathology. The Larynx provides phonation and protection for the lower airway via closure and cough reflex. Disruption of these functions or development of paralysis or stenosis can lead to respiratory failure. Management includes a variety of endoscopic and open management techniques. Understanding of laryngeal physiology can also limit laryngeal functional impairment through careful preservation of motor nerves and early recognition of injuries.

With the ever-expanding implement of screening programs, as well as a raised awareness of patients about their own health, the number of cases of early-stage lung cancer is progressively increasing, leading surgeons to adapt their practice and to develop new surgical techniques that are less and less invasive [...].

Various disease processes may affect the ascending thoracic aorta, aortic arch, and/or descending thoracic aorta, including aneurysms, dissections, intramural hematomas, penetrating atherosclerotic ulcers, and aortic transection/rupture. Many of those conditions require surgical intervention for repair. Multiple open and endovascular techniques are used for treatment of thoracic aortic pathology. It is imperative that the cardiothoracic radiologist have a thorough knowledge of the surgical techniques available, the expected postoperative imaging findings, and the complications that may occur t

Minimally invasive thoracic surgery has come a long way. It has rapidly progressed to complex procedures such as lobectomy, pneumonectomy, esophagectomy, and resection of mediastinal tumors. Video-assisted thoracic surgery (VATS) offered perceptible benefits over thoracotomy in terms of less postoperative pain and narcotic utilization, shorter ICU and hospital stay, decreased incidence of postoperative complications combined with quicker return to work, and better cosmesis. However, despite its obvious advantages, the General Thoracic Surgical Community has been relatively slow in adapting VAT…

Iatrogenic complications of thoracic and cardiovascular surgery are relatively uncommon, but contribute to potentially significant patient morbidity and mortality. The incidence of iatrogenic disease reflects the complexity of surgical procedures, including lung resection, esophagectomy, coronary artery bypass grafting, thoracic aorta repair, and cardiac valve replacement. Some iatrogenic complications are minor and common to all procedures, whereas others can be potentially devastating and are associated with precise technical components of specific surgeries. Multimodality imaging plays an i

As the largest scientific organisation world-wide exclusively dedicated to general thoracic surgery (GTS), the European Society of Thoracic Surgeons (ESTS) recognized that one of its priorities is education. The educational platform designed ESTS addresses not only trainees, but also confirmed thoracic surgeons. The two main aims are (I) to prepare trainees to graduation and to the certification by the European Board of Thoracic Surgery and (II) to offer opportunities for continuous medical education in the perspective of life-long learning and continuous professional development to certified …

Surgery in pulmonary tuberculosis in one form or other gives good results. Indications for surgery include drug resistant pulmonary tuberculosis, massive recurrent haemoptysis, post-tuberculosis bronchiectasis or destroyed lung, empyema with or without bronchopleural fistula and for diagnostic purposes. In all cases a clear indication for surgery is mandatory. Processes of surgery include lung resection, thoracoplasty, decortication, thoracotomy and biopsy, thoracoscopy and ib resection for pleurocutaneous flap procedures. Adequate postoperative management is very important. Complications like

Records of 146 dogs and 41 cats that underwent thoracic surgery at The University of Sydney Veterinary Teaching Hospital were reviewed for age, sex, breed, disease, operation date, periods of pre- and post-operative hospitalisation, use of surgical drains and outcome. Animals were assigned to 16 disease categories, the most common being patent ductus arteriosus, traumatic diaphragmatic hernia and oesophageal foreign body in dogs and traumatic diaphragmatic hernia in cats. Differences were observed between disease categories in all the criteria examined. There were almost equal numbers of male …

Between 1975 and 1980, 9 patients with traumatic rupture of the thoracic aorta were operated at our institution. All patients showed additional multiple limb and internal organ injuries. Leading symptoms of aortic rupture were mediastinal widening (8), left-sided hemothorax (6), and acute aortic coarctation (2). Aortic rupture was proven by angiography in 8 patients and during exploratory thoracotomy in another. The time interval between trauma and operation was one to 48 hours, in 6 patients less than 4 hours. Atrio-femoral bypass was used in 3 patients, TDMAC-heparin shunt in 4 patients and

Today, congenital thoracic malformations (CTMs), which comprise a spectrum of anomalies rather than separate entities, are frequently detected on routine antenatal ultrasound. However, with similar appearances of different congenital lung and nonpulmonary lesions, a definitive diagnosis cannot usually be established antenatally with absolute certainty. The natural history of CTMs is extremely variable. Large lesions may cause serious complications in the fetus, necessitating treatment in utero. Such prenatal therapy, however, is only required in a small minority of fetuses. Many lesions decrea

INTRODUCTION: The impact of sex on quality of life (QoL) and long-term mortality after thoracic aortic surgery is incompletely understood. We investigated whether sex-related differences in these outcomes exist after surgery.
METHODS: Patients undergoing thoracic aortic surgery between 2004 and 2023 were identified using the Manitoba Thoracic Aortic Database, which was linked to population-level registries in the Manitoba Centre for Health Policy. Inverse probability treatment weighting was performed to account for the differences in baseline characteristics between sexes. The outcomes were s

Extended reality (XR) includes the sub-terms of virtual reality (VR), augmented reality (AR) and mixed reality (MR) and describes interactive and immersive technologies that replace the real world with digital elements or seamlessly extend it with such approaches. XR thus offers a very wide range of possible applications in medicine. In surgery, and thoracic surgery in particular, XR technologies can be harnessed for treatment planning, navigation, training, and patient information. Such applications are increasingly being tested and need to be evaluated. We provide an overview of the status q…

Non-intubated video-assisted thoracic surgery (NI-VATS) combines the advantages of a non-intubated surgery with the benefits of a minimally invasive approach. First, NI-VATS is performed in the case of fragile patients when general anesthesia and/or orotracheal intubation can be foreseen as inconvenient. However, NI-VATS indications have been increasingly extended to different patient conditions, considering the increasingly assessed safety and feasibility of the procedure. Currently, the NI-VATS approach is used worldwide for different thoracic surgery procedures, including the management of …
