[Indication of thymectomy and immunotherapies on myasthenia gravis].

First, we studied clinical effects of thymectomy on non-thymomatous ocular myasthenia gravis (MG) patients by comparing operative and non-operative groups. The QMG scores reduced with similar significances in both groups. Thymectomy is considered to be unnecessary in non-thymomatous ocular MG. Second, we discussed over the issue how elderly-onset MG patients could be treated. We believe that thymectomy is sufficiently tolerable and therefore could be selected with low dose oral prednisolone in moderate to severe MG cases. However we should be aware of various complications which are common in …

[A technique of videothoracoscopic thymectomy in diseases of the thymus].

Intensive development of thoracoscopic thymectomy (Tthy) has been initiated during the last decade. Unfortunately very few instances of using the procedures have been described in Russian medical literature. So, the paper describes in detail the technique and results obtained in our clinic. At the period from 1998 through 2004 37 Tthy operations were performed in the St. Petersburg Center of intensive pulmonology and thoracic surgery. There were 21 female and 15 male patients with mean age 41.5+/-18.9 years. Myasthenia gravis (MG) was diagnosed in 19 of those patients. Different thoracoscopic …

Predictors of outcome in thymectomy for myasthenia gravis.

BACKGROUND: Factors determining predictability of response to thymectomy for myasthenia gravis (MG) vary in the literature. METHODS: A 25-year retrospective review (1974 to 1999) of all thymectomies performed at a single institution was undertaken. RESULTS: In 113 consecutive thymectomies for MG, women comprised 79% (89 of 113 patients), and mean age was 40+/-15 years. Complications occurred in 14% of patients (16 of 113). In-hospital mortality was 0, but 90-day hospital mortality was 0.88% (1 of 113 patients). Follow-up was obtained in 81% (92 of 113 patients) at a mean of 51+/-59 months po…

Improvement of dysgeusia after thymectomy with thymoma.

We present a case of 39-year-old woman with thymoma complaining of dysgeusia. This patient had suffered from dysgeusia for the previous 6 months. Thymectomy with the thymoma was performed, and her dysgeusia was improved within 6 months after the operation. The anti-acetylcholine-receptor antibody was reduced from 0.9 nmol/l to 0.4 nmol/l (normal: below 0.6 nmol/l) by the surgical intervention. This case suggested that symptoms of dysgeusia can be associated with myasthenic status.

[Early thymectomy in the treatment of myasthenia gravis].

Over a 10-year period, 1983 through 1992, in the clinic of endocrinologic surgery--Alexander Hospital, Sofia, 89 patients with myasthenia gravis (MG), 57 women and 32 men, are subjected to operative treatment--thymectomy. The mean age in women is 32 years (range 19 to 68), and in men--45.6 years (range 25 to 70). Two age-related distributions in the series are impressive--in female patients the peak is between 20 and 30 years, whereas in men--between 45 and 55 years. The operative intervention consists in total thymectomy. Preference is given to the trans-sternal median access to the thymus gl…

Effects of thymectomy on bone growth in the rat.

The effects of thymectomy (TMX) at 1 or 5 days of age on the subsequent growth of bone have been determined in male and female Holtzman rats. The rats were maintained under aseptic conditions to minimize any effects of infection or wasting disease on bone growth. Femur length and distal epiphyseal cross sectional area were significantly reduced in TMX female rats at 3 months of age. At 1 month of age, the width of the undifferentiated zone was increased and at all subsequent times the columnar zone and growth plate were narrower in TMX rats. The incorporation of 35S into the femur, particularl…

Robotic subxiphoid radical thymectomy for a thymoma.

Robotic-assisted thoracic surgery has emerged as a prominent technique for performing radical thymectomies in patients affected by early-sta … chiocephalic vein, all while minimizing intercostal trauma. We present a robotic subxiphoid radical thymectomy using the da Vinci Xi platform, illustrated by a case involving a patient with a 5-cm thymoma close to the left phrenic nerve.

Robotic thymectomy: a review of techniques and results.

Thymectomy is a well-established therapeutic option in the multidisciplinary treatment of nonthymomatous myasthenia gravis (MG) and in thymoma treatment. Although many surgical procedures for thymectomy have been identified, the transsternal method is still regarded as the gold standard. Minimally invasive procedures, on the other hand, have achieved popularity in the last decades and are now extensively used in this field of surgery. Among them, robotic thymectomy has been the most cutting-edge surgical procedure. Several authors and meta-analyses have shown that a minimally invasive approach…

Thymectomy in Myasthenia Gravis: A Narrative Review.

Myasthenia gravis (MG) is a rare condition caused by autoantibodies against acetylcholine receptors on postsynaptic membrane that leads to weakness of skeletal muscles. About 7 of 10 patients with MG have thymic hyperplasia and about 1 of 10 patients have thymoma. Thymectomy has increasingly been used as a treatment modality for MG. Several observational studies have shown that thymectomy results in improvement in MG and a randomized trial has established that thymectomy leads to a better outcome in non-thymomatous generalized MG. However, thymectomy is yet controversial in some disease subtyp…

Thymectomy for myasthenia gravis: A pathological analysis.

BACKGROUND: Myasthenia gravis (MG) is a prototypic T-cell-dependent antibody-mediated autoimmune disease that leads to ocular or generalized muscular weakness. The disease is most commonly caused by antibodies to the acetylcholine receptors, often with underlying thymic pathology. AIMS: This study is aimed at analyzing the pathological spectrum of the excised thymuses in patients with myasthenia. MATERIALS AND METHODS: This was a retrospective 10-year study of 68 thymectomy specimens performed as a part of the treatment of patients with MG. STATISTICAL ANALYSIS: Nil. RESULTS: There were 47…

[Thoracoscopic thymectomy in children by myasthenia gravis].

The authors' experience of surgical treatment of myasthenia gravis in children was analyzed. 32 patients were operated on, mean age was 13±3 … red in 2 and deterioration of myasthenia in 2 (6.25%) patients. The study proved that thoracoscopic thymectomy obtains the radicalism of open technique, proposing the easier postoperative period and being the method of choice for the surgical treatment of myasthenia gravis in children.

The role of transcervical thymectomy in patients with hyperparathyroidism.

BACKGROUND: The most common location for supernumerary or ectopic parathyroid glands is the thymus. METHODS: A review of patients who underwent parathyroidectomy for hyperparathyroidism from 1990 to 2010 was completed to determine indications for thymectomy, the yield of parathyroid tissue, and outcome of therapy. RESULTS: Seventy of 379 patients with hyperparathyroidism underwent parathyroidectomy and transcervical thymectomy. Intrathymic parathyroid tissue was present in 23 (33%) patients, including supernumerary glands in 8 patients (11%). Indications for thymectomy were renal hyperparath…

Biological implications of thymectomy for myasthenia gravis.

Myasthenia gravis (MG) is an autoimmune disease mediated by autoantibodies to the striated muscle tissue. It is often treated by thymectomy. We review recent studies to investigate the biological implications of thymectomy. In anti-acetylcholine receptor antibody (anti-AchR Ab)-positive patients without a thymoma, abnormal germinal center formation in the thymus seems to play an essential role in the pathogenesis of MG. Specific differentiation of B cells producing anti-AchR Ab takes place uniquely in the thymus, and thymectomy is thought to assist in terminating the provision of high-affinity…
