INTRODUCTION: Thrombosis is rare in children and antithrombolytic treatment is controversial. Most commonly used thrombolytic agent is tissue plasminogen activator (t-PA) in pediatrics. In this study, we report our experience in the use of thrombolytic treatment.
METHODS: Eighteen patients who had received systemic t-PA between January 2006 and December 2013 were recorded. The response to t-PA was evaluated as complete, partial, and no. The bleeding complication during t-PA administration was graded as minor or major.
RESULTS: There were 18 patients (2 mo to 12 y) who received systemic t-PA.…

BACKGROUND: Coagulopathy is associated with massive transfusion in trauma, yet most clinical scores to predict this end point do not incorporate coagulation assays. Previous work has identified that shock increases circulating tissue plasminogen activator (tPA). When tPA levels saturate endogenous inhibitors, systemic hyperfibrinolysis can occur. Therefore, the addition of tPA to a patient's blood sample could stratify a patients underlying degree of shock and early coagulation changes to predict progression to massive transfusion. We hypothesized that a modified thrombelastography (TEG) assay…

Acute ischemic stroke is a potentially catastrophic medical emergency. Recently, successful reversal of the neurologic deficits associated with major ischemic strokes has been accomplished in selected patients through the use of intravenous tissue plasminogen activator (tPA), an agent that can accomplish thrombolysis of arterial clots if given within the first few hours after the onset of stroke. Because tPA works by thrombolysis of fresh clots, a potential exists for catastrophic hemorrhage if given to acute postoperative patients. Therefore, the use of tPA has never been studied in postopera…

Therapeutic guidelines of intravenous thrombolysis with tissue plasminogen activator (tPA) for hyperacute ischemic stroke are very strict. Because of potential higher risk of bleeding complications, the presence of unruptured cerebral aneurysm is a contraindication for systemic thrombolysis with tPA. According to the standard CT criteria, a 66-year-old woman who suddenly developed aphasia and hemiparesis received intravenous tPA within 3 h after ischemic stroke. Magnetic resonance angiography during tPA infusion was performed and the presence of a small unruptured cerebral aneurysm was suspect…

UNLABELLED: The aim of our study is estimation of the efficacy of recombinant tissue plasminogen activator for the treatment of fibrinous membranes following cataract extraction.
PATIENTS AND METHODS: The fibrinous membranes were formed, despite conventional antiinflammatory therapy, in 17 eyes after ECCE with IOL and in 3 eyes after ECCE without IOL. We injected the solution of recombinant tissue plasminogen activator-Actilyse (Boehringer Ingelheim) into the anterior chamber of 20 eyes after cataract surgery. Simultaneously, topical corticosteroid and cycloplegic therapy was continued. Routi…

The plasminogen activator (PA) proteolytic cascade comprises two enzymes known as urokinase PA (uPA) and tissue PA (tPA), both of which activate plasminogen to plasmin. In normal human epidermis uPA is the predominant PA. In lesional epidermis from patients with a variety of cutaneous diseases, including psoriasis, pemphigus foliaceous, pemphigus, vulgaris, bullous pemphigoid, and benign chronic pemphigus, however, tPA is selectively elevated and becomes the predominant PA activity. The enhanced tPA is likely to be a reaction to the disease challenge rather than an initiating event in these cl

PURPOSE: To describe the authors' clinical experience with intracameral tissue plasminogen activator (tPA) after glaucoma surgery.
METHODS: Retrospective review of medical records of all patients who received intracameral tPA after glaucoma surgery at the Doheny Eye Institute from November 4, 1992, to June 14, 1994. There were 20 tPA administrations (18 eyes of 17 patients) in doses ranging from 6 to 25 microgram. Indication for tPA administration was decreased bleb function secondary to blood/fibrin clot in aqueous outflow pathway.
RESULTS: Tissue plasminogen activator was given after trabe…

BACKGROUND: We wanted to evaluate whether intracameral injection of tissue plasminogen activator (tPA) is useful in managing traumatic hyphaema.
METHODS: Two eyes with total hyphaema after a severe penetrating injury were treated with a single intracameral injection of 25 micrograms of tPA 5 and 14 days after the injury, respectively.
RESULTS: Most of the blood coagulum dissolved within 24 h, and in one of the two eyes the intraocular pressure decreased from 45 to 8 mmHg. The other eye was hypotonic. No re-bleeding or complications related to the use of tPA were noticed.
CONCLUSION: The res…

Paroxysmal nocturnal haemoglobinuria (PNH) is an acquired clonal disorder thought to arise in a multipotent haemopoietic stem cell. A distinct clinical feature is a tendency to thrombosis, with a particular predilection for the hepatic veins (Budd-Chiari syndrome). We report here on two patients with PNH who developed hepatic vein thrombosis (HVT) and who were treated with tissue plasminogen activator (t-PA). Both patients had a marked clinical and radiological improvement following the t-PA treatment and remain well over 2 years and 6 years after the treatment. This method of thrombolysis for…

We describe an autopsy case of severe intracranial hemorrhage which occurred during the infusion of tissue plasminogen activator (t-PA) for acute myocardial infarction. A 75-year-old man was admitted with substernal chest pain of 3-h duration and electrocardiographic changes consistent with an acute inferior myocardial infarction. Physical examination was unremarkable, except for an initial blood pressure reading of 160/96 mmHg. The patient received 3,000 IU intravenous heparin followed by a 2.4 x 10(6) IU bolus dose of tissue plasminogen activator (t-PA) (Alteplase). This was followed by a dr…

Thrombotic vascular occlusion may complicate the clinical course of many neonatal and pediatric pathologic processes. Systemic thrombolytic therapy with heparin, urokinase, or streptokinase may not be appropriate in the critically ill neonate because these agents generate a diffuse coagulopathic state. Direct surgical intervention for repair may be precluded by the small size of the vessels involved. Recombinant tissue plasminogen activator (rTPA) induces only a minimal proteolytic state while inducing thrombolysis within the local environment of the clot. We report our experience with regiona…

We report on the successful repeated treatment of a patient with rapidly progressive severe brain-stem stroke by application of I.V. tissue plasminogen activator (tPA). This treatment twice led to a prompt remission of severe brain-stem symptoms, although a permanent therapeutic success could not be achieved. Unfortunately, the patient died a few days later from pneumonia and sepsis. Necropsy revealed bilateral partial brain-stem infarctions and a subtotal stenosis of the vertebrobasilar junction with superimposed fresh thrombus. The clinically dramatic response to tPA indicates that this type…

Two patients with angiographically proved basilar artery occlusion were treated with systemic recombinant tissue plasminogen activator (rtPA) according to protocol. The first patient was in a locked-in state and gradually deteriorated. On repeat angiography the basilar artery remained occluded. He died and necropsy revealed a pontine haemorrhagic infarction. The second patient, who was comatose and with decerebrate posturing, made a remarkable recovery. Angiography showed reperfusion. Therapy was initiated in the first patient after six hours and in the second after two hours. Treatment with r…

Tissue plasminogen activator (tPA), a zymogen associated with fibrinolysis, was used in an attempt to prevent scarring and the consequent loss of glaucoma filtration blebs. Bilateral trephine filter procedures were performed in 10 new Zealand white rabbits. In a randomized, masked fashion, one eye received intracameral tPA (25 micrograms), and the fellow placebo, immediately following surgery. One week after surgery, outflow facility in the tPA-treated eyes was significantly greater than that in the control (P less than 0.01). Two weeks after surgery, the outflow facility returned to preoperat…

To investigate the hypothesis that diminished endothelial fibrinolysis is present in sickle cell (SS) disease plasma, tissue type plasminogen activator (t-PA) antigen titres were measured before and after a standard stimulus of endothelial t-PA release (venous occlusion of the arm), and plasma t-PA activities after venous occlusion in 33 subjects with SS disease and in 32 healthy subjects. Mean plasma t-PA antigen titres before and after venous occlusion, and mean plasma t-PA activities after venous occlusion did not differ significantly between SS patients and normal subjects. No significant

BACKGROUND: The plasminogen activation system represents a potent mechanism of extracellular proteolysis and is an essential component of normal wound healing. It has also been implicated in the pathogenesis of chronic, nonhealing ulcers. Traditionally, urokinase-type plasminogen activator (uPA) has been associated with pericellular proteolytic activity involved in tissue remodelling processes, and tissue-type plasminogen activator (tPA) mainly with intravascular fibrinolysis.
OBJECTIVES: The present study was conducted to characterize the spatial distribution of the various plasminogen activ

Pathological blood clots cause life-threatening events, including ischemic stroke and myocardial infarction. Tissue plasminogen activator (TPA), a clinically approved fibrinolytic agent, remains a frontline therapy for clot dissolution. However, its short plasma half-life and requirement for high systemic doses pose serious risks, particularly hemorrhagic complications. To address these limitations, a shear-responsive delivery platform was engineered for controlled TPA release under pathological high-shear conditions. This system, termed Single-MOlecule-based materials with structures and func…

Invasive Candida albicans infections (candidiasis) cause progressive organ damage through fungal tissue invasion and toxin-mediated injury, including in the kidney. Hyphal invasion induces apoptosis of renal tubular epithelial cells (RTEC), a key driver of kidney pathology, yet intrinsic renal protective mechanisms remain poorly defined. We identify fibrinolytic tissue-type plasminogen activator (tPA) as a critical mediator of renal tissue protection in candidiasis. tPA is induced by IL-17 and TNFα in renal endothelial cells and RTEC. tPA signals through low-density lipoprotein receptor-relate
