Enhanced conservation of vital pulp and apical tissues by the application of crown rotation surgery for inversely impacted central incisors: A follow-up analysis of two patients over 4 years.

AIM: To describe a novel surgical method (crown rotation surgery) to manage inversely impacted central incisors with immature roots. METHODOLOGY: Two young patients each presented with an inversely impacted maxillary central incisor. To protect the apical tissues, the two impacted incisors were rotated downwards to a relatively normal position without extraction from their bony sockets. RESULTS: After crown rotation surgery, spontaneous eruption, continuous root development, and periodontal healing of the rotated incisors were observed. The pulp retained vitality and blood flow was normal. M

Inflammatory Diseases of the Teeth and Jaws.

The teeth are unique in that they provide a direct pathway for spread of infection into surrounding osseous and soft tissue structures. Periodontal disease is the most common cause of tooth loss worldwide, referring to infection of the supporting structures of the tooth, principally the gingiva, periodontal ligament, cementum, and alveolar bone. Periapical disease refers to an infectious or inflammatory process centered at the root apex of the tooth, usually occurring when deep caries infect the pulp chamber and root canals. We review the pathogenesis, clinical features, and radiographic findi

Conservative management of an accidentally resected root: a 3-year follow-up.

Despite all the effort by dentists to provide high levels of dental care in clinical practice, there is always the possibility of accidents occurring. This report describes a referred case that presented with irreversible pulpitis in tooth #19. The situation developed after accidental resectioning of the distal root of tooth #19 during the surgical extraction of tooth #18 by another dentist. The resultant open apex in the distal root of tooth #19 was managed conservatively. A positive apical stop was prepared at the root end of the distal root and sterile calcium hydroxide powder was used to c

The Fate of Apically Extruded Sealers: Radiographical Disappearance or Biological Integration? A Narrative Review.

OBJECTIVE: Over the past 2 decades, several novel materials have been introduced into endodontic practice, specifically designed to be bioactive and biointeractive with apical bone tissues. However, despite their widespread adoption, there remains limited consensus on the biological consequences of accidental apical extrusion, particularly when premixed bioceramic sealers are used. This review aims to analyse both the immediate and long-term effects of apical extrusion of new bioactive root canal sealers compared to traditional bioinert obturation materials. METHODS: Open search strategy was

Restoration of Long Standing Traumatized Teeth: A Case Report.

Children are uniquely susceptible to craniofacial trauma. Injuries to the teeth occur often as a result of falls and sport activities. The pulp often gets infected after dental trauma resulting in to numerous complications. The authors present a case report of successful restoration of traumatized teeth with open apex which were weakened due to long standing infection and internal resorption. Initially antibiotic combination of 3- mix was used to disinfect the root canals. One tooth is treated with conventional endodontic treatment and the other tooth with open apex and perforation is managed

Autotransplantation of an Impacted Immature Third Molar Following Orthodontic Activation without Endodontic Intervention.

Autogenous tooth transplantation (ATT) using open-apex donor teeth may preserve pulp vitality, but atraumatic retrieval of impacted third molars is challenging. This case report describes an interdisciplinary workflow combining orthodontic donor activation with digitally guided, vitality-preserving ATT. A 27-year-old woman with an edentulous maxillary left first molar site (tooth #14) underwent ATT using an immature mandibular right third molar (tooth #32) as the donor. After 6 weeks of orthodontic traction, cone-beam computed tomography and intraoral scans were registered for virtual planning

Spatial distribution of lead in the roots of human primary teeth.

Lead remains one of the most hazardous metals in our environment. The concentrations of lead in coronal dentine and enamel have previously been reported but limited information is available regarding lead levels in radicular dentine and cementum. This study reports the distribution of lead in 26 roots of 16 human maxillary primary teeth from seven individuals. In addition, calcium and phosphorous concentrations were also measured to detect any variations in the degree of mineralization in different regions of the roots. The mean lead concentration in these roots was 1.67 +/- 1.43 microg/g, whi

Single-session use of mineral trioxide aggregate as an apical barrier in a case of external root resorption.

External root resorption may occur as a consequence of trauma, orthodontic treatment, bacterial infection or incomplete sealing of the root canal system (bacterial re-infection), and lead to crater formation on the resorbed apex. This would deform the root apex surface, and cause loss of apical constriction. Depending on the extent of the resorptive process, different treatment regimens have been proposed. A 34-year-old male patient presented with an intra-radicular retainer and an inadequate filling on tooth #21, as well as a radiographic image suggesting periapical bone rarefaction. After ro
