Early detection of external cervical resorption in posterior teeth: a radiographic, cross-sectional study of an adolescent population.

OBJECTIVES: To establish the prevalence and severity of external cervical resorption (ECR) in posterior teeth observed in bitewing (BW) radiographs in an epidemiological study of a 17-year-old patient population from community dentistry. Furthermore, to assess the potential predisposing factors for ECR. METHODS: Posterior BWs from 5596 patients (2717 females, 2879 males; mean age 17.8 years) were assessed by three observers in order to detect ECR (using Heithersay's classification system, severity classes 1-4). When ECR was suspected, cone beam CT (CBCT) was offered to verify diagnosis. Preva

Dentin hypersensitivity: pain mechanisms and aetiology of exposed cervical dentin.

OBJECTIVES: The paper's aim is to review dentin hypersensitivity (DHS), discussing pain mechanisms and aetiology. MATERIALS AND METHODS: Literature was reviewed using search engines with MESH terms, DH pain mechanisms and aetiology (including abrasion, erosion and periodontal disease). RESULTS: The many hypotheses proposed for DHS attest to our lack of knowledge in understanding neurophysiologic mechanisms, the most widely accepted being the hydrodynamic theory. Dentin tubules must be patent from the oral environment to the pulp. Dentin exposure, usually at the cervical margin, is due to a v

Cervical enamel projections and associated pouch-like opening in mandibular furcations.

BACKGROUND: Cervical enamel projections (CEPs) have been listed among the tooth-associated risk factors that could lead to isolated furcation defects around molar teeth. This is more likely when Grade III CEPs are present. However, the histologic aspects of the CEP interface with regard to the presence or absence of cementum over the enamel projection within the furcation have not been well described. This study was initially undertaken to evaluate this relationship. METHODS: Thirty-five mandibular molars with Grade III CEPs were selected from a collection of extracted teeth and evaluated for

Cementoenamel junction of deciduous teeth: SEM-morphology.

AIM: The aim of this study was to investigate the cementoenamel junction of a group of 11 primary sound mandibular incisors extracted for orthodontic reasons. MATERIALS AND METHODS: Eleven caries and defect-free human inferior deciduous incisors were extracted for orthodontic reasons and the cementoenamel junction was investigated by scanning electron microscopy (SEM). The types of tissue interrelations were classified in four possible categories: 1) cementum and enamel edge-to-edge, 2) cementum overlapped by enamel, 3) enamel overlapped by cementum, 4) presence of exposed dentin between enam

[Die externe zervikale Resorption bei Oberkiefer-Eckzähnen].

External cervical resorptions (ECRs) are challenging dental hard tissue lesions. They are frequently missed due to their asymptomatic progression. Three-dimensional imaging and increased awareness within the dental community have likely contributed to the observed rise in ECR prevalence over the recent years. While the exact etiology of ECR remains unclear, numerous risk factors have been identified, encompassing both physical and chemical factors. However, idiopathic cases have also been documented. Literature highlights the propensity of ECRs to occur in anterior teeth and molars, often link

Novel Surgical Approach for Root Coverage Using Coronally Advanced Flap in Conjunction With Transposition of Periosteal Pedicle Graft.

INTRODUCTION: Gingival recession is a complex phenomenon with multifactorial etiology. It is defined as the apical migration of the soft tissue margin beyond the cemento-enamel junction, thereby exposing the root surface. It results in the destruction of both soft and hard tissues. CASE PRESENTATION: Three patients with buccal gingival recession defects underwent surgical treatment consisting of transposition of a periosteal pedicle in conjunction with the coronally advanced flap (CAF) technique. As the cambium layer of the periosteum has greater osteoblastic potential than the fibrous layer,

European Society of Endodontology position statement: External Cervical Resorption.

This Position Statement represents the consensus of an expert committee convened by the European Society of Endodontology (ESE) on External Cervical Resorption (ECR). The statement is based on current clinical and scientific evidence as well as the expertise of the committee. The primary aim is to provide a current authoritative position on the aetiology, histopathology, clinical presentation and management of ECR, and also to highlight areas where there is minimal evidence. Previously published review articles provide more detailed background information and the basis for this position statem

Idiopathic cervical resorption: a diagnostic dilemma.

UNLABELLED: Idiopathic cervical resorption is a rare form of external resorption, usually with no external signs, and can be misdiagnosed as dental caries or other types of tooth resorption. Here we report a case in which, during routine radiography, an asymptomatic lower right molar presented with a radiolucency at the cervical region, with no obvious aetiologic factor identified. CLINICAL RELEVANCE: This paper emphasizes the aetiopathogenesis and differential diagnosis of this rare and asymptomatic form of pathologic resorption, which can be encountered in dental practice.

Influence of Dentoalveolar Ankylosis on the Biomechanical Response of a Single-rooted Tooth and Surrounding Alveolar Bone: A 3-dimensional Finite Element Analysis.

INTRODUCTION: Dentoalveolar ankylosis necessarily accompanies the loss of periodontal ligament (PDL), which might alter the biomechanical response of the tooth. The purpose of this study was to investigate the influence of dentoalveolar ankylosis on a single-rooted tooth and the surrounding alveolar bone structures in the biomechanical standpoint. METHODS: A basic model of an intact maxillary central incisor and the surrounding bone structures was chosen for the numeric analysis. From this basic model, 6 different models were further developed by combining 3 types of endodontic status (an int

Healing and prognosis of teeth with intra-alveolar fractures involving the cervical part of the root.

Healing and long-term prognosis of 94 cervical root fractures were evaluated. The teeth were divided into two groups according to type of fracture: transverse fractures limited to the cervical third of the root (51 incisors) and oblique fractures involving both the cervical and middle parts of the root (43 incisors). Neither the frequency nor the type of fracture healing differed significantly between the two groups. In the material as a whole, healing of the fracture with hard tissue formation was observed in 17 teeth (18%), and healing with interposition of periodontal ligament (PDL) and, in

A new concept regarding the origin of the junctional and crevicular epithelium in rat molars.

The substitution of the internal enamel epithelium by a squamous epithelium during tooth eruption has been studied histologically in rat molars. Just prior to eruption, cytolysis of the connective tissue covering the cusp tip determines hyperplasia in the oral epithelium. A conspicuous infiltration results. Infiltrated undifferentiated cells form distinct cords which seem to be attracted by the internal enamel epithelium. They reaggregate when they come into its contact and form an underlying epithelium. The process starts in the superficial part and progressively extends towards the cervix. F

Root and root canal morphology in maxillary second molar with fused root from a native Chinese population.

INTRODUCTION: Root fusion is an anatomic variation in maxillary second molars (MSMs); however, the nature of this canal morphology as it relates to its root anatomy has not been fully clarified. The purpose of this study was to investigate the relationship between features of fused roots and root canal anatomy in MSMs using micro-computed tomographic imaging. METHODS: One hundred eighty-seven extracted MSMs were scanned with the μCT50 (Scanco Medical, Bassersdorf, Switzerland), and their root and canal morphology was classified and analyzed using the classifications proposed by Yang and Vertu

External multiple invasive cervical resorption with subsequent arrest of the resorption.

A rare, the uncommon, case of multiple invasive cervical resorption (MICR) with subsequent arrest of the resorption involving eight teeth of the mandible (tooth 46 to tooth 32) is presented. This case is noteworthy because of the characteristics of its clinical evolution. There have only been a few cases in the literature were found presenting involvement of such a large number of teeth. Furthermore, no reports on cases of MICR with arrest of the resorption, as described in this report, were found.

[Analysis of etiological factors involved in noncarious cervical lesions].

PURPOSE: The etiopathology of noncarious cervical lesions (NCCL) is multifactorial and still not fully understood. Tooth wear is defined as loss of dental hard tissue by a chemical or mechanical process that does not involve bacteria. This form of tooth surface loss includes attrition, abrasion, erosion, and abfraction. Noncarious cervical lesions represent loss of tooth structure at the cementoenamel junction. The purpose of this clinical study of NCCL was to analyze the etiology in relation to age and to identify the most important risk factors associated with cervical lesions, as well as pa

Coronally repositioning the cemento-enamel junction to address gingival margin discrepancies.

BACKGROUND: Gingival margin discrepancies, especially in the esthetic zone, can pose a challenge for the restorative dentist. Surgical clinical crown lengthening and orthodontic tooth movement are most often used to treat these situations. In this report, an additional surgical technique will be discussed with its indications for use. METHODS: Three cases are presented, each with a different type of gingival margin discrepancy. The surgical technique used was the same for all cases. It involved establishing a new, more coronally positioned cemento-enamel junction (CEJ) through the use of a fi

External cervical resorption: part 2 - management.

Effective management of external cervical resorption (ECR) depends on accurate assessment of the true nature and accessibility of ECR; this has been discussed in part 1 of this 2 part article. This aim of this article was firstly, to review the literature in relation to the management of ECR and secondly, based on the available evidence, describe different strategies for the management of ECR. In cases where ECR is supracrestal, superficial and with limited circumferential spread, a surgical repair without root canal treatment is the preferred approach. With more extensive ECR lesions, vital p

Multiple idiopathic cervical resorption: a diagnostic dilemma.

Multiple idiopathic cervical resorption is an uncommon condition, the etiology of which remains unknown. The case of a 36-year-old woman with idiopathic resorption in the cervical areas of multiple teeth is presented. Her medical history was noncontributory, but her dental history was valuable in reaching a diagnosis. Hematologic and endocrinal investigations were unremarkable, but a computed tomography scan revealed relevant information. The progression of the lesion and involvement of multiple teeth without any pattern or chronology are challenges faced when diagnosing and treating this cond

Playing wind instruments as a potential aetiologic cofactor in external cervical resorption: two case reports.

AIM: To present two cases of external cervical resorption (ECR) on maxillary incisors, in which the primary aetiologic factor is suggested to be pressure trauma by frequently playing wind instruments. SUMMARY: The exact aetiological spectrum of ECR is still poorly understood. For resorption to occur, a defect in the cementum layer (trigger) is a likely prerequisite. Whilst the mechanism for continuation (stimulus) is still unclear, knowledge of potential predisposing factors is important in assessing patients at risk. Pressure generated by playing wind instruments could present an aetiologica
