Pilot study on cannabis-induced alterations in platelet function: implications for transfusion medicine.

Platelet transfusions are essential in the management of thrombocytopenia, bleeding disorders, and hematologic malignancies. With cannabis use rising worldwide, its impact on donor platelet quality and transfusion efficacy remains poorly understood. This study investigated the effects of cannabis joint extracts (CJE) on platelet activation, pro-coagulant phenotype, mitochondrial function, and cytokine/chemokine release, with implications for transfusion safety. Human platelets were exposed in vitro to increasing concentrations of two CJE with distinct cannabinoid profiles: Orchid (O-CJE, THC 1

Transfusion medicine and solid organ transplant - Update and review of some current issues.

Transfusion medicine holds a place of prime importance in organ transplant surgeries. There is a huge demand of organs worldwide with long waiting periods before the organ is available for transplant. Currently the dependency on ABO and HLA matching has decreased considerably with the use of modern immunosuppressant drugs and transplant techniques. The greatest advance in clinical implementation of ABO-incompatible transplants came about through desensitization and isoagglutinin elimination techniques with immunoadsorption and anti-CD20 antibodies becoming the norm, and spleenectomy fading out…

Best practices adopted in Argentina to promote hemotherapy technicians as professionals in transfusion medicine.

From the beginning of the use of blood for therapeutic purposes, technicians in Argentina played an important role. The scope of the hemotherapy technician's activities includes participation in all processes of transfusion medicine. Hemotherapy technicians' workplaces are hemotherapy services in all their complexity. Current training in Argentina considers the hemotherapy technician no longer as an assistant, but as a professional in the health area. The creation of the Bachelor's Degree in Hemotherapy and Immunohematology responds to the need to professionalize human resources in health, rai…

Red cell transfusion benefits in oncology, haematology and palliative medicine populations: a narrative review.

Due to the heterogenous nature of the palliative medicine patient population, assessment of benefit, and thus choice of appropriate patient for consideration of transfusion, can be challenging. This can be confounded by the use of both liberal and restrictive transfusion thresholds. The multifactorial nature of many symptoms of anaemia, particularly in patients with advanced malignancy, can further complicate. As such, there is a paucity of data supporting the subjective, objective and clinical benefit of red cell transfusion in the palliative medicine setting. This narrative review summarises

The State of Transfusion Medicine in Low- and Middle-income Countries: A Critical Narrative Review.

An annual global shortfall of over 100 million blood units is borne almost entirely by low- and middle-income countries (LMICs), with measurable consequences for patient survival. Despite sustained advocacy, the evidence on transfusion medicine across these settings has not been systematically and critically reviewed. A narrative review was conducted across 9 electronic databases, applying strict LMIC-specific, date-bound eligibility criteria. Studies conducted in LMIC settings addressing any aspect of transfusion medicine and published within the preceding 15 years were eligible. Seven studie…

Transfusion Medicine in a Multidisciplinary Approach to Morbidly Adherent Placenta: Preparing for and Preventing the Worst.

The increasing incidence of placenta accreta has paralleled the rise in its greatest risk factor: cesarean delivery. In placenta accreta, the abnormal invasion of the chorionic villi into the myometrium prevents separation of the placenta at delivery, and the myometrium is unable to contract to prevent hemorrhage. Spontaneous uterine rupture and hemoperitoneum may also occur in the setting of placenta percreta. The average blood loss during a delivery complicated by placenta accreta is 2 to 5 L, compared to less than 0.5 L for a normal spontaneous vaginal delivery and less than 1 L for a cesar

Red blood cell transfusions for preterm infants: the role of evidence-based medicine.

Increasingly clinicians attempt to base decisions regarding patient management on the results of clinical studies in addition to expert opinion and their own practical experience. In this article, the author reviews the published studies available to assist clinicians to make evidence-based decisions in three topics related to small volume red blood cell (RBC) transfusions for preterm infants; namely, studies examining the effects of RBC transfusions on possible symptoms of anemia such as tachypnea, apnea or other cardiorespiratory irregularities, studies investigating the collection and trans
