Choledochal cysts (CDCs) and biliary atresia (BA) are rare pediatric hepatobiliary anomalies that require surgical intervention due to increased risk of malignancy and liver failure, respectively. The underlying disease and operative procedures place patients at risk for long-term complications, which may continue to affect them into adulthood. Lack of a transitional care model in the health-care system potentiates the challenges they will face following aging out of their pediatric providers' care. We sought to elucidate the long-term complications and challenges patients with CDCs and BA fac…

Background: The transition from hospital to home for older individuals can be complicated, as they are more likely to have complex health and/or social care needs. Several published studies have outlined positive outcomes from pharmacist-driven transition of care programs. At our four geriatric primary care clinics affiliated with a large academic medical center, there is no medication reconciliation process to evaluate a patient's medications after being discharged from the hospital to home. Objective: The objective of this pilot program was to demonstrate the need for a pharmacist-led transi

Healthcare systems in Canada and elsewhere have identified the need to develop methods to effectively and safely transition appropriate cancer survivors to primary care. It is generally accepted that survivors with a low risk of adverse events, including recurrence and toxicity, should be more systematically identified and offered transition. There remains a lack of clarity about what constitutes an appropriate profile that would assist greater application in practice. To address this gap, we examined the clinical profiles of patients that were transitioned from a large regional cancer centre

Cystic Fibrosis is a chronic disease affecting multiple systems, including the GI tract. Clinical manifestation in patients can start as early as infancy and vary across different age groups. With the advent of new, highly effective modulators, the life expectancy of PwCF has improved significantly. Various GI aspects of CF care, such as nutrition, are linked to an overall improvement in morbidity, lung function and the quality of life of PwCF. The variable clinical presentations and management of GI diseases in pediatrics and adults with CF should be recognized. Therefore, it is necessary to

Objective: To investigate the association between functional status and post-acute care (PAC) transition(s). Methods: Secondary analysis of 2013-2014 Medicare data for individuals aged ≥66 years with stroke, lower extremity joint replacements, and hip/femur fracture discharged to one of three PAC settings (inpatient rehabilitation facilities, skilled nursing facilities, and home health agencies). Functional scores were co-calibrated into a 0-100 scale across settings. Multilevel logistic regression was used to test the partition of variance (%) and the probability of PAC transition attributed

Global efforts are underway to develop, implement and test health care transition (HCT) models of care. Most studies have focused on the transfer of care models. In contrast, the nurse-led interdisciplinary HCT model, Movin' On Up, provides comprehensive HCT services beginning in early adolescence. A retrospective analysis was conducted of data extracted from HCT records of 146 adolescents and emerging adults with spina bifida (with a mean age of 13.91 years) who were provided services in the Movin' On Up HCT program. Data were categorized based on the Health Care Transition Research Consortiu

BACKGROUND AND AIMS: The British Society of Gastroenterology recommend use of structured transition programs to improve control of chronic gastrointestinal disease in adolescents. We sought to determine the impact of attending a structured transition program on engagement of patients with inflammatory bowel disease (IBD) services and disease outcomes.
METHODS: We performed a retrospective multicenter study of patients with IBD prior to and post-transfer to adult services. Patients were grouped into those who attended a structured transition program and those who received standard care, transf

OBJECTIVES: In young adults with inflammatory bowel disease (IBD), the time following transfer to adult care is high-risk for adverse outcomes. We used quality improvement (QI) methods to standardize care, decrease variation, and improve preparation of young adults during the transition/transfer process.
METHODS: We created the IBD Transition Care Index (TCI), a list of 10 variables whose completion was felt to represent a more comprehensive transition/transfer process. Variables were organized into three domains: Disease Control/Physical Health, Psychosocial Well-being, and Transition/Transf

BACKGROUND: The literature on transitional care in anorectal malformation (ARM) and Hirschsprung's disease (HD) is diverse and heterogeneous. There is a lack of standards and guidelines specific to transitional care in these conditions. We aim to establish and systematically categorize challenges and solutions related to colorectal transition care.
METHODS: Systematic review of qualitative studies from MEDLINE, EMBASE, PubMed and Scopus databases (2008-2022) was conducted to identify the challenges and solutions of healthcare transition specific to ARM and HD. Thematic analyses are reported w…

The pilot transition from outsourced to in-house hemodialy sis (HD) services at a 274-bed acute care hospital resulted in significant cost savings, operational improvements, and enhanced patient care. By adopting an advanced in-house HD model, the pilot hospital reduced per-treatment costs by 89.2%, eliminated outsourced vendor-imposed penalties, and achieved a 0% turnover rate among nephrology nurses. A structured, multidisciplinary strategy streamlined workflows and improved staff engagement. The pilot's success catalyzed system-wide expansion over four years across eight hospitals, demonstr

BACKGROUND: Peripheral arterial disease (PAD) affects between 3% and 10% of the global population. Transitional care programmes (TCPs) aim to ensure continuity between hospital-based acute care and primary healthcare. Nurses play a pivotal role in supporting this transition. TCPs may reduce hospital readmissions, emergency department (ED) attendances and improve patients' quality of life (QoL). This scoping review examines whether nurse-led TCP improves the management of patients undergoing lower limb vascular surgery, including the effectiveness of remote (telehealth) delivery.
METHODS: This…

Approximately 25% of inflammatory bowel disease (IBD) cases are diagnosed in childhood, with distinct characteristics compared to adult-onset disease. Transitioning from pediatric to adult care is a critical phase requiring structured planning. A literature review of PubMed, Scopus, and Web of Science articles from the past 6 years highlights that structured transition programs improve adherence, reduce relapse rates, and lower hospitalizations, yet many centers lack formal protocols. Key barriers include inadequate provider training, poor communication between pediatric and adult teams, and l

AIMS: Gait speed has been advocated as a marker of vulnerability, but its discriminatory utility and predictive ability in frail older people in a post-acute community-based rehabilitation program has not been extensively investigated. The aims of this research were to examine whether there was a meaningful improvement in gait speed in post-acute care patients, and to determine whether gait speed predicted outcomes at 6-month follow up.
METHODS: In a prospective cohort study, 351 older persons admitted to a transition care program were comprehensively assessed using the interRAI Home Care ins

Social determinants of health (SDOH) influence transition of care (TOC). The complex interactions of SDOH-such as those between education and economics-make research and interventions in this area challenging. However, evidence suggests that SDOH can help identify those at risk of poor TOC outcomes. High-risk groups include people with public or no insurance, those with low income or limited social support, and those who live in disadvantaged neighborhoods. Interventions such as improved health care access and enhanced social support may offer meaningful impact.

OBJECTIVES: Little is known about the experience of adolescents and young adults (AYA) with disorders of gut-brain interaction (DGBI) who transition from pediatric to adult gastroenterology care. In this two-part study, we used quantitative and qualitative methods to: (1) assess incidence of optimal versus suboptimal transitions of care for AYA with DGBI, (2) characterize health and quality of life effects of the transition, and (3) identify barriers and facilitators for optimal transition of care.
METHODS: In Part 1, we conducted a retrospective review of AYA referrals to our adult neurogast
