Impact of Device-Associated Infection on Trauma Patient Outcomes at a Major Trauma Center.

BACKGROUND: Catheter-associated urinary tract infection (CAUTI) and ventilator-associated pneumonia (VAP) are considered performance measures. We analyzed the incidence, prevalence, and risk of CAUTI and VAP in trauma patients, as well as the demographic and injury factors related to these infections and their relative risks of negative outcomes (prolonged length of stay [LOS], sepsis, and death). METHODS: Trauma registry data were analyzed (age >18 y; LOS >24 h) from January 1, 2007, to December 31, 2011. Demographics and injury location, severity, and type were analyzed relative to outcomes

Infectious complications of noncombat trauma patients provided care at a military trauma center.

Infectious complications are reported frequently in combat trauma patients treated at military hospitals. Infections in 4566 noncombat related trauma patients treated at a military trauma center were retrospectively reviewed from 1/2003 to 5/2007 using registry data. Burns, penetrating, and blunt trauma accounted for 17%, 19%, and 64%, respectively; the median age was 38 and 22% were female. Pulmonary infections were present in 4.2% of patients, 2.4% had cellulitis and wound infections, 2.2% urinary infections, and 0.7% sepsis. On univariate analysis, infected patients were more likely to be a

The efficacy of a two-tiered trauma activation system at a level I trauma center.

BACKGROUND: By using current American College of Surgeons trauma center triage criteria, 52% of patients transported to our level I trauma center are discharged home from the emergency department (ED). Because the majority of our trauma transports were based solely on mechanism of injury, we instituted, in 1990, a two-tiered trauma team activation system. Patients are triaged into major and minor trauma alert categories based on prehospital provider information. For minor trauma patients, respiratory therapy, operating room staff, and blood bank do not respond. The current study evaluated this

Assessment of severity, causes, and outcomes of hospitalized trauma patients at a major trauma center.

BACKGROUND AND AIM: Trauma is a major cause of hospitalization in both civilians and military personnel (MP). It constitutes one of the most common causes of work layoffs, lowered efficiency, and handicaps in the civilian as well as the military population. Assessment of injury parameters and severity is useful. It allows for grading, grouping, classification, and assessment of outcomes of trauma patients for comparison with world standards. This study aimed to assess the Injury Severity Scores (ISS), recognize the source, and types of injuries leading to hospitalization of civilian and milita

[Closed traumas of the thorax. Assessment of two years activity at the Dakar Trauma Center].

Two years after opening the Dakar Trauma Center has received 179 cases of blunt chest trauma without prehospital treatment. Road accidents were the leading cause. The initial mortality was 29 patients (16.20%) and 150 patients arrived alive and were treated. The initial mortality was primarily due to the associated extrathoracic injuries mainly hemorrhagic with hemoperitoneum in 44.82% and brain hemorrhage in 24.14% of cases. Secondly, it was due to the thoracic injuries with 41.37% of anterior flail chest and massive hemothorax in 65.51% of cases. Among the 150 patients treated, 42% had extra

Analysis of ECMO usage in trauma patients at a major level 1 trauma center.

Extracorporeal membrane oxygenation (ECMO) has been used for decades in patients where adequate oxygenation could not be achieved using traditional efforts. Data is emerging regarding its use in trauma. Retrospective data was collected from the ECMO database at an urban Level 1 trauma center between 2020 and 2024. Inclusion data was age > 17 years, trauma, and ECMO utilization during admission. Outcomes extracted included patient demographics, mechanism and type of injury, type of ECMO, ECMO days, hospital length of stay (LOS), intensive care LOS, date of injury to ECMO time, P/F ratio, ISS, c

Food Insecurity Is Common in the Orthopedic Trauma Population at a Rural Academic Trauma Center.

BACKGROUND: Food insecurity is an increasingly recognized public health issue. Identifying risk factors for food insecurity would support public health initiatives to provide targeted nutrition interventions to high-risk individuals. Food insecurity has not been investigated in the orthopedic trauma population. METHODS: From April 27, 2021 to June 23, 2021, we surveyed patients within six months of operative pelvic and/or extremity fracture fixation at a single institution. Food insecurity was assessed using the validated United States Department of Agriculture Household Food Insecurity quest

Secondary overtriage of pediatric neurosurgical trauma at a Level I pediatric trauma center.

The authors looked at all of the pediatric patients with a head injury who were transferred from other hospitals to their own over 12 years and tried to identify factors that would allow patients to stay closer to home at their local hospitals and not be transferred. Many patients with isolated, nondisplaced skull fractures or negative CT imaging likely could have avoided transfer. While hospitals should be cautious, this may help families stay closer to home.

Criteria for the qualification of geriatric patients to the trauma center.

BACKGROUND: The growth of the geriatric population, resulting in an exponential increase in the number of injured older people, determines the necessity of ensuring adequate access to specialized units such as trauma centers (TCs). The specificity of pathophysiological processes that progress with age, worsening the body's response to trauma, makes it difficult to develop an optimal triage protocol for geriatric patients, reducing high undertriage, i.e., underestimation of injuries and referral to lower-reference units. AIM: To verify the TC qualification criteria for 65+ patients and analyze…

The effects of Hurricane Sandy on trauma center admissions.

BACKGROUND: Hurricane Sandy was a particularly unusual storm with regard to both size and location of landfall. The storm landed in New Jersey, which is unusual for a tropical storm of such scale, and created hazardous conditions which caused injury to residents during the storm and in the months following. This study aims to describe differences in trauma center admissions and patterns of injury during this time period when compared to a period with no such storm. METHODS: Data were collected for this study from patients who were admitted to the trauma center at Morristown Medical Center dur

Factors affecting trauma center utilization in Illinois.

The Illinois Trauma System has served as a prototype for many of the advanced emergency medical services systems developed during the past decade. The Illinois Trauma Study uses a cross-sectional research design to assess the trauma patient utilization patterns at hospitals in Illinois. Data were collected from 33 Trauma Centers and 43 nonsystem hospitals for the time interval July 1973-June 1974. Results indicated that 45.5 per cent of patients admitted with specified trauma conditions were hospitalized at Trauma Centers. When adjustment is made for differences in the size of hospitals, a 50 …

Differences in Characteristics of Trauma Between General Trauma and Suicidal Trauma-Trauma Mechanism, Injury Site, and Severity: A Single-Center Study.

Background: Among patients with trauma, those with self-harm exhibit different characteristics than those who experience general accidents. Unstable vital signs following a severe injury often limit accurate imaging and injury assessment during initial treatment, rendering decision-making challenging for definitive care. Identifying correlations between damage area and severity can improve predictions and treatment decisions. We identify differences in characteristics of trauma between patients who experienced general accidents and those who attempted suicide. Methods: This study investigates

Operative Rib Fixation at a Rural Trauma Center.

INTRODUCTION: Operative rib fixation is becoming more common in the management of rib fractures, with studies showing benefits of surgery in decreasing time on mechanical ventilation, ICU and total hospital length of stay, and mortality. This paper will review the data from our institution to determine if these benefits were seen in our trauma population. METHODS: A retrospective review of the institution's trauma database was conducted, including operative patients (n = 36), control patients (nonoperative patients from October 2018 to October 2019, n = 207), and selected control patients bas
