Persistent trigeminal artery supply to an intrinsic trigeminal nerve arteriovenous malformation: a rare cause of trigeminal neuralgia.

Infratentorial arteriovenous malformations (AVM) associated with the trigeminal nerve root entry zone are a known cause of secondary trigeminal neuralgia (TN). The treatment of both TN and AVM can be challenging, especially if the AVM is embedded within the trigeminal nerve. A persistent trigeminal artery (PTA) can rarely supply these intrinsic trigeminal nerve AVM. We present a 64-year-old man with TN from a right trigeminal nerve AVM supplied by a PTA variant. The patient underwent microvascular decompression and a partial resection of the AVM with relief of facial pain symptoms. His residua

Peripheral nerve stimulation for trigeminal neuropathic pain.

Facial pain is a complex disease with a number of possible etiologies. Trigeminal neuropathic pain (TNP) is defined as pain caused by a lesion or disease of the trigeminal branch of the peripheral nervous system resulting in chronic facial pain over the distribution of the injured nerve. First line treatment of TNP includes management with anticonvulsant medication (carbamazepine, phenytoin, gabapentin, etc.), baclofen, and analgesics. TNP, however, can be a condition difficult to adequately treat with medical management alone. Patients with TNP can suffer from significant morbidity as a resul

[Primitive intracranial trigeminal nerve germinoma. Case report].

A 22-year-old man presented headache, asthenia, body weight loss and trigeminal hypoesthesia worsening quickly. Radiological analysis showed an enhanced lesion that originated from the cavernous sinus and extended into the Meckel cave, owing to the fifth cranial nerve's course. The lesion was explored by a temporo-pterional approach and was partially removed. On the basis of histological analysis and negativity of tumor marker levels in serum and cerebrospinal fluid (alpha-fetoprotein alpha, human beta-chorionic gonadotropin), a primitive non-secreting intracranial germinoma was diagnosed. Infratemporal fossa mucocele with trigeminal nerve compression.

We present a rare case of an infratemporal fossa mucocele with facial paresthesia in a 36-year-old man. Both computerized tomography scans and magnetic resonance imaging confirmed a cystic lesion occupying the left infratemporal fossa. Marsupialization using transmaxillary and endoscopic intranasal approaches was performed to drain the mucocele into the nasal cavity and the application of mitomycin C to preserve the drainage of mucocele, which may be a good surgical option for the complete excision of infratemporal fossa cystic lesions without causing postoperative morbidity.

Functional sensory recovery after trigeminal nerve repair.

PURPOSE: The aim of this study was to estimate the proportion of subjects who achieved functional sensory recovery (FSR) 1 year after inferior alveolar or lingual nerve repair and to identify risk factors associated with failure to achieve FSR. METHODS: Using a retrospective cohort study design, we developed a sample composed of subjects who underwent lingual or inferior alveolar nerve repair. Eligible subjects had at least 1 postoperative visit. For subjects having bilateral nerve repair, 1 side was selected randomly for analysis. Predictor variables were categorized as demographic, anatomic

Trigeminal nerve repetitive stimulation in myasthenia gravis.

The aim of this study was to evaluate the utility of repetitive nerve stimulation (RNS) of the trigeminal nerve in assessing patients with myasthenia gravis (MG). In 26 normal controls and 21 patients with myasthenia gravis (MG), 2-Hz repetitive stimulation of the trigeminal nerve was performed using a monopolar needle for percutaneous nerve stimulation and recording over the surface of the masseter. In the MG patients, repetitive stimulation of the ulnar, spinal accessory, and facial nerves was also performed. The mean percent decrement in the compound muscle action potential (CMAP) amplitude

Pictorial review: Trigeminal nerve: anatomy and pathology.

MRI is the imaging modality of choice when trigeminal nerve pathology is suspected. Most lesions are readily recognizable if appropriate imaging sequences are performed. Routine cranial MRI sequences augmented by a three-dimensional gradient echo sequence such as FISP (fast inflow with steady-state precession) are sufficient to demonstrate most pathological processes involving the trigeminal nerve and nucleus. Intravenous gadolinium-DTPA occasionally provides additional diagnostic information. MRI is particularly useful in planning the management of those conditions where surgical or medical i

Trigeminal Nerve Schwannoma of the Cerebellopontine Angle.

Introduction  Large and even moderate sized, extra-axial cerebellopontine angle (CPA) tumors may fill this restricted space and distort the regional anatomy. It may be difficult to determine even with high resolution magnetic resonance imaging (MRI) if the tumor is dural-based, or what the nerve of origin is if a schwannoma. While clinical history and exam are helpful, they are not unequivocal, particularly since many patients present with a myriad of symptoms, or conversely an incidental finding. We present an atypical appearing, asymptomatic CPA tumor, ultimately identified at surgery to be

Granular cell tumor of the trigeminal nerve.

Male breast cancer is a rare disease and it differs from breast cancer in women by some characteristics. The incidence of the disease has increased in the last 25 years. The records of male patients who underwent surgery for breast cancer between 2007 and 2017 were retrospectively reviewed in a tertiary care hospital in Istanbul, Turkey. The patients' ages, background, family history, clinical features, histopathological features of the tumour, its stage, the treatment and the survival were investigated. SPSS 15.0 for Windows programme was used for statistical analysis.Survival analysis was pe

Endoscopic vascular decompression of the trigeminal nerve.

BACKGROUND: Microvascular decompression is an effective method of treatment in trigeminal neuralgia. It may fail if a compressing vessel is overlooked during surgery. The endoscope has an edge over the microscope in visualizing such conflicts. MATERIALS AND METHODS: This is a prospective study of 51 patients. Preoperative computed tomography and magnetic resonance imaging scans were performed in all the cases. A 4 − 5 cm retroauricular skin incision was made and an about 3 cm craniectomy was performed. A 0° 4 mm telescope supported by the holder was used after the dural opening. A 2 by 6 cm s

Benign triton tumor of the trigeminal nerve.

Benign triton tumors (neuromuscular hamartomas) are rare neoplasms composed of well-differentiated striated muscle fibers admixed with peripheral nerve fibers. To the best of our knowledge, this is the third case reported in the world literature of a benign triton tumor involving cranial nerve V (trigeminal nerve) and the first in the radiology literature. The previous reports of this lesion have focused on its unusual pathology and not on the imaging findings. In light of the imaging appearance of this lesion, we consider that the imaging findings may suggest this uncommon diagnosis.

Percutaneous balloon compression of the trigeminal nerve.

Balloon compression is a simple and effective percutaneous approach for the treatment of classic trigeminal neuralgia or trigeminal neuralgia secondary to multiple sclerosis. The operation injures large myelinated fibers, removing the "trigger" to the presumed ephaptic transmission of pain. Because unmyelinated fibers, which mediate the corneal reflex, are preserved, compression may be of advantage in the treatment of first division pain, since the corneal reflex is mediated by the small unmyelinated fibers. In the author's series of 141 consecutive patients treated by balloon compression, ove

Zoster sine herpete of the trigeminal nerve.

Zoster sine herpete infection that involves the trigeminal nerve is rarely reported. The present case details a case of varicella zoster virus infection of the mandibular division of the left trigeminal nerve without evidence of a vesicular eruption. The diagnosis was established on clinical findings and was supported by the demonstration of an IgG antibody titer of > 1:640 during the acute phase of the disease.
