Sulfamethoxazole-trimethoprim may be an alternative or adjunctive treatment for Wegener's granulomatosis, as suggested by our experience with six patients. Two had limited Wegener's granulomatosis; one of these achieved remission with sulfamethoxazole-trimethoprim alone, and the second achieved remission with sulfamethoxazole-trimethoprim and prednisone. Four patients presented with sinus, pulmonary, and renal involvement. One patient initially treated with sulfamethoxazole-trimethoprim developed worsening renal function requiring cytotoxic therapy. Two patients initially treated with cytotoxi…

Transferable resistance to classical Cephalosporins emerged three years ago among Enterobacteriaceae simultaneously in several hospitals and was associated with resistance to other betalactam drugs, i.e. with a transmissible TEM-like betalactamase. A systematic survey of incidence of R plasmids in Proteus sp. lead, in two district PHLs, to demonstration of transferable Azlocillin resistance. This resistance was most frequently associated with that for Carbenicillin, but also strains susceptible to CAR but resistant to AZL have been isolated. Trimethoprim resistance was in this country first de…

A retrospective comparison between clinical experience with trimethoprim-sulfamethoxazole (TMP-SMZ) and predictive laboratory investigations demonstrates that the most accurate predictions concern antibacterial activity of the drug against individual organisms. With regard to therapy for some relatively severe infections, clinical experience is limited and further research is needed. Some such studies that were not feasible in the past are now possible as a result of the availability of trimethoprim (TMP) as a single agent and of parenteral forms of both TMP and TMP-SMZ. The infections reviewe…

Twenty-three patients with typhoid fever diagnosed by blood culture were treated with co-trimoxazole (trimethoprim/sulphamethoxazole, 1 :5) at a dosage of 10 mg trimethoprim/kg/day in two divided daily doses for 10 days. Twenty-two patients responded, the mean period to defervescence being three days and to relief of toxaemia being 1-7 days. Another nineteen patients with similarly proven typhoid were treated with chloramphenicol (100 mg/kg/day) in four divided daily doses for 10 days. Eighteen patients responded, defervescence occurring in an average of 4-1 days and relief of toxaemia in a me…

A patient with Whipple's disease responding rapidly to sulphamethoxazole-trimethoprim is described. In spite of initial folate deficiency, the drug was well tolerated for one year. It is proposed that a sulphonamide combined with trimethoprim should be the drug of first choice in the treatment of Whipple's disease. Repeated jejunal biopsy is suggested as the best way to record the effectiveness of treatment.

One hundred and three children with proved typhoid fever were treated with trimethoprim-sulphamethoxazole, and the results compared with those of a further 40 children treated with chloramphenicol. The bacteriological response to trimethoprim-sulphamethoxazole was unsatisfactory. From this study it seems that at present chloramphenicol is still the treatment of choice for typhoid fever. In view of the haematological changes occurring during therapy with trimethoprim-sulphamethoxazole caution is necessary and monitoring of the blood picture advisable, even at the recommended dose.
