An overview is given of the pathogenic mechanisms involved in Helicobacter pylori-associated duodenal and gastric ulceration. Special attention is given to the role of microbial virulence factors, the effects on gastric acid secretion and the development of 'gastric type' metaplasia in the duodenal bulb. Cure of the infection essentially eliminates ulcer recurrence. Cure also prevents ulcer-related bleeding. Antibiotic regimens providing a reliable high cure rate are briefly discussed. Finally the questions about ulcer disease which remain unanswered are summarized.

Patients with wounds and cutaneous ulcers have been a costly but low priority group. During recent years, greater attention has been focused on wound problems, partly as the result of the formation of interest groups and the establishment of wound treatment centres, e.g. in Britain and the USA. In June this year, the first wound treatment centre in Scandinavia is to open in Copenhagen. Research and education in this field give promise of modernised treatment for patients with chronic non-healing wounds and ulcers.

Refractory or intractable ulcer is defined as an ulcer that fails to heal completely after eight to twelve weeks, despite appropriate treatment with a modern antiulcer therapy in a compliant patient. Refractory ulcer should be suspected in individuals diagnosed to have peptic ulcer if their symptoms persist longer than usual: occurrence of complications or simply their ulcers fail to heal, since up to 25% of such patients remain asymptomatic. Conditions associated with refractory ulcer include noncompliance, continuous consumption of nonsteroidal anti-inflammatory drugs, acid hypersecretion, s…

Peptic ulcer disease affects between 5 percent and 10 percent of Americans during their lifetime. There are a variety of predisposing factors, including nonsteroidal anti-inflammatory drug (NSAID) use, Helicobacter pylori infection, tobacco use and, possibly, certain dietary practices. While H2-receptor antagonists remain the mainstay of therapy, the selection of an antiulcer drug should be individualized. Famotidine is preferred in persons who use alcohol, sucralfate may be preferred for maintenance therapy in smokers and omeprazole is useful in hypersecretors of acid and in patients with res…

Fungal gastric ulcer is a relatively frequent from of gastric mycosis coexisting in 20-30% of cases with peptic ulcer. In such cases fungi were present in patients without other disease and in those with concomitant diseases such as neoplasms, polyps of the stomach, haematological diseases, and in patients treated with cytostatics and H2-blockers. Fungal ulcer differ from common ulcers in greater size and tendency for bleeding. In certain situations "fungal ulcer" requires routine antimycotic treatment especially if operation is planned.

Non-ulcer dyspepsia (NUD) includes functional forms, related to secretory and/or motor disorders, but also refers to forms with gastritis and/or duodenitis (erosive or not, Helicobacter pylori positive or not), as well as to idiopathic forms. NUD pathophysiology is multifactorial. Secretory abnormalities, H. pylori infection and in particular digestive and interdigestive disorders of gastrointestinal motility are often detected in NUD patients, but psychological, social and environmental factors can be also involved in NUD pathogenesis. With regard to symptom genesis, there is still no convinc…

The purpose of this paper is to evaluate the frequency and some of the clinical and epidemiological characteristics of asymptomatic gastric ulcer, found in persons who attend the Center for Control of Gastrointestinal Cancer of San Cristóbal, Venezuela, as part of a gastric cancer mass screening program. 350 patients with active peptic ulcer were found, 176 duodenal and 174 gastric, diagnosed by means of endoscopy. Of the patients with gastric ulcers, 67 (38.5%) had no symptoms. This group of asymptomatic patients were analyzed regarding age, sex, place of residence, type of work and also the …

This is a case report on tuberculosis of the stomach in a 70-year-old woman who, submitted to an endoscopic examination for uncharacteristic upper abdominal complaints, revealed multiple ulcers in the aboral part of the stomach, together with a thickening of the gastric wall. Histologically, caseating epithelioid cell granulomas were found, and only a single acid-fast tubercle (TB rod). As a further specific manifestation, tuberculous spondylitis developed, which was treated surgically. Under anti-tuberculous treatment, the gastric ulcers healed. The clinical-endoscopic picture, the pathogenes…

The authors present data of the clinical X-ray and morphological diagnostics of malignant gastric ulcers revealed in 96 patients. The total number of patients with gastric ulcer operated upon was 642. The morphological findings may serve the only reliable criterium of malignancy. Results of the operative treatment of such a malignant ulcer of the stomach are dependent on the spread of malignant invasion in the gastric wall. Treatment of patients with the involved mucosa only gave 5-year survival in 89.5%. Spread of the malignant invasion throughout the whole thickness of the gastric wall--50.0…

Thirty-four patients presenting with gastric perforation over a ten year period were studied, retrospectively. Five patients (14%) died either as a result of delay in treatment or wrong diagnosis. In addition, 2 patients (6%) died from postoperative complications. Twelve patients (38%) were on potentially ulcerogenic drugs. Patients who had either simple closure of ulcers (n = 17) or partial gastrectomy (n = 6) had few symptoms at follow-up, but patients who underwent truncal vagotomy and pyloroplasty (n = 7) had poorer results. There were 2 recurrent ulcers following simple closure.
