Thoracolumbar flexion-distraction injuries combined with vertebral body fractures.

Surgery was performed on 25 patients with combination injuries (flexion-distraction injury plus vertebral body fracture): 8 patients with anterior-column failure (compression) and 17 patients with anterior-column plus middle-column failure (burst). Patients with compression received posterior instrumentation and underwent fusion; patients with burst received posterior instrumentation and later underwent anterior decompression and fusion. Eleven patients in the burst group had a neurologic deficit. Single dural tears were discovered in 7 patients during the posterior-instrumentation procedure. …

Percutaneous vertebroplasty for severe osteoporotic vertebral body compression fractures.

PURPOSE: To determine the efficacy of percutaneous vertebroplasty in treating severe vertebral body compression fractures, or vertebra plana, in patients with osteoporosis. MATERIALS AND METHODS: In 155 patients, 310 percutaneous vertebroplasties were performed during 25 months and 15 days. Of these, 37 patients (27 women, 10 men; mean age, 73.6 years) underwent 48 vertebroplasties for severe osteoporotic vertebral body compression fractures. The fractures were defined as vertebrae that have collapsed to less than one-third of their original height. Imaging and clinical features were analyzed…

Vertebral body fractures in child abuse. Radiologic-histopathologic correlates.

RATIONALE AND OBJECTIVES: Vertebral injuries are rarely reported sequelae of child abuse, and little is known concerning the mechanisms of injury and healing. A preliminary investigation of these issues included correlating radiologic and histologic findings in children with vertebral injuries who died of complications relating to physical abuse. METHODS: Ten vertebral body fractures from four abused infants and young children were studied radiologically and histopathologically. RESULTS: Infants ranged in age from 7 to 36 months (mean, 21 months). Three patients died of associated head injur…

Cervical spinal stenosis: determination with vertebral body ratio method.

Transient bilateral sensory and motor symptoms after trauma, including complete paralysis, have been identified in patients with cervical spinal stenosis. Radiographs of 23 patient athletes with cervical spinal neurapraxia were used for measurement of the cervical spinal canal. Two methods of measurement were used. In the conventional method, sagittal diameter is measured from the posterior surface of the vertebral body to the nearest point of the corresponding laminar line. In the ratio method, the sagittal diameter of the spinal canal is divided by the sagittal diameter of the corresponding …

Vertebral body collapse after spine stereotactic body radiation therapy: a single-center institutional experience.

BACKGROUND: Spine stereotactic body radiation therapy (SBRT) for the treatment of metastatic disease is increasingly utilized owing to improved pain and local control over conventional regimens. Vertebral body collapse (VBC) is an important toxicity following spine SBRT. We investigated our institutional experience with spine SBRT as it relates to VBC and spinal instability neoplastic score (SINS). PATIENTS AND METHODS: Records of 83 patients with 100 spinal lesions treated with SBRT between 2007 and 2022 were reviewed. Clinical information was abstracted from the medical record. The primary …

Sagittal Spinal Alignment 2 Years After Vertebral Body Tethering.

STUDY DESIGN: Retrospective cohort. OBJECTIVE: To investigate the change of sagittal spinal alignment after anterior vertebral body tethering (VBT) in skeletally immature patients with idiopathic scoliosis using pelvic incidence-based measures of alignment. SUMMARY OF BACKGROUND DATA: VBT is a non-fusion option for the treatment of idiopathic scoliosis, though concern exists for derangement of the sagittal plane with skeletal growth. METHODS: A retrospective analysis was conducted on 87 patients with thoracic idiopathic scoliosis who underwent VBT. Preoperative and 2-year postoperative radi…

Incidence of Tether Breakage in Anterior Vertebral Body Tethering.

BACKGROUND: Thoracic anterior vertebral body tethering (TAVBT) is an emerging treatment for adolescent idiopathic scoliosis. Tether breakage is a known complication of TAVBT with incompletely known incidence. We aim to define the incidence of tether breakage in patients with adolescent idiopathic scoliosis who undergo TAVBT. The incidence of tether breakage in TAVBT is hypothesized to be high and increase with time postoperatively. METHODS: All patients with right-sided, thoracic curves who underwent TAVBT with at least 2 and up to 3 years of radiographic follow-up were included. Tether break…

Vertebral body tethering for adolescent idiopathic scoliosis: a review.

PURPOSE: Adolescent Idiopathic Scoliosis (AIS) remains the most common type of pediatric scoliosis, mostly affecting children between ages 10 and 18. Vertebral body tethering (VBT) offers a non-fusion alternative to the gold standard spinal fusion that permits flexibility and some growth within instrumented segments. This article will serve as a comprehensive literature review of the current state-of-the-art of VBT in relation to radiographic and clinical outcomes, complications, and the learning curve associated with the procedure. METHODS: A systematic literature review was conducted on Pub…

Vertebral Body Osteolysis 6 Years After Cervical Disk Arthroplasty.

Cervical arthroplasty is an accepted and widely performed surgical intervention with usually favorable outcomes. We report a rare case of a 37-year-old woman who presented with vertebral body osteolysis, 6 years after two-level cervical arthroplasty (C5-C6 and C6-C7). The patient showed no initial complications, but at year 6 she presented with neck and radicular arm pain. Diagnostic imaging revealed osteolysis of the vertebral body C6. The patient subsequently underwent removal of the two prostheses and C6 corpectomy, followed by bridging C5 to C7 and anterior fixation. During surgery, the ma…

Fluoroscopy Guided Percutaneous Transpedicular Biopsy of Vertebral Body Lesion.

BACKGROUND: The common causes of vertebral body lesion are metastasis, infection, primary malignancies or osteoporotic fractures. Histopathological examination is necessary to confirm the diagnosis. There are different approaches to collect the biopsy samples and they have different adequacy and accuracy rates and also possible complications. This study aims to determine adequacy, accuracy and safety of the fluoroscopy guided percutaneous transpedicular biopsy of the vertebral body lesion. METHODS: This is retrospective review of all the patients who underwent fluoroscopy guided percutaneous …

Cement Leakage into Adjacent Vertebral Body Following Percutaneous Vertebroplasty.

Percutaneous vertebroplasty (PV) is a minimally invasive procedure for osteoporotic vertebral compression fractures that fail to respond to conventional conservative treatment. It significantly improves intolerable back pain within hours, and has a low complication rate. Although rare, PV is not free of complications, most of which are directly related to cement leakage. Because of its association with new adjacent fracture, the importance of cement leakage into the adjacent disc space is paramount. Here, we report an interesting case of cement leakage into the adjacent upper vertebral body as…

[Corrective growth after vertebral body fracture in the adolescence].

We present the case of a 13-year-old female patient who sustained an incomplete burst fracture of the 1st lumbar vertebral body and an impression fracture of the 3rd vertebral body. Using a dorsal instrumentation the kyphosis of the thoracolumbar junction was treated. The radiographical follow-up indicated a correction of the involved vertebral bodies. However, complete restoration of the height of the vertebral bodies was not achieved. In view of this long-term follow-up the potential for correction of the adolescent thoracolumbar junction spine is discussed.

Spinal intraosseous arteriovenous fistula in the fractured vertebral body.

A 68-year-old woman presenting with progressive paraparesis was diagnosed with an AVF involving a previously fractured L1 vertebral body on which feeders from multiple segmental arteries converged. The most distinctive feature in our patient was that the fistula was located in the vertebral body. Transarterial embolization of the segmental arteries with coils and glue resulted in total obliteration of the fistula, which in turn resulted in symptom improvement.

Minimally invasive treatment of osteoporotic vertebral body compression fractures.

BACKGROUND CONTEXT: Seven hundred thousand osteoporotic compression fractures occur yearly. Approximately 260,000 lead to severe pain, and 1 … cant improvement in symptoms and function after kyphoplasty or vertebroplasty. Kyphoplasty improves vertebral body height and kyphotic alignment by 50%, if performed within 3 months of the onset of the fracture collapse. CONCLUSIONS: Vertebroplasty and kyphoplasty both have roles in the treatment of painful osteoporotic compression fractures. Only kyphoplasty h …

Chronic obstructive pneumonia caused by a vertebral body osteophyte.

Osteophytes associated with spondylosis have been implicated as a cause of multiple extraspinal manifestations. Symptoms are more likely to occur with the large osteophytes associated with diffuse idiopathic skeletal hyperostosis. In the thoracic region, osteophytes have been reported infrequently as a cause of extraspinal complications. We report a case in which an anterior thoracic vertebral osteophyte was responsible for chronic obstructive pneumonia due to obstruction of the right main stem bronchus. The patient's condition improved considerably after surgical resection of the compressing
