Hyperventilation and vertigo.

An electronystagmographic study was conducted on 19 normal subjects, in order to observe whether the subjective sensation of dizziness provoked by hyperventilation could be confirmed objectively by nystagmus. Each of them had two electronystagmograms, the first being a routine ENG and the second a repetition of the first, but with additional periods of 90 sec. of hyperventilation at certain precise pre-determined moments of the test. Hyperventilation was not shown to have significant effect on the slow phase of post caloric nystagmus; however, it increased significantly (p = 0.061) the number

[Special forms of vertigo. Which therapy for which type of vertigo?].

Paroxysmal vertigo, permanent vertigo, positional and postural vertigo, and transient vertigo are special forms, which are differentiated by their temporal course. Paroxysmal vertigo, together with hearing impairment and noises in the ear, is typical of Menière's disease. Persistent vertigo often occurs after the loss of a peripheral vestibular end organ (e.g. after trauma of infection). In the case of positional and postural vertigo, a differentiation must be made between benign paroxysmal positional vertigo due to "wandering" otoliths, and orthostatic vertigo, which occurs on changing rapidl…

Vascular vertigo: updates.

Discriminating strokes in patients with acute dizziness/vertigo is challenging especially when other symptoms and signs of central nervous involvements are not evident. Despite the developments in imaging technology over the decades, a significant proportion of acute strokes may escape detection on imaging especially during the acute phase or when the lesions are small. Thus, small strokes causing isolated dizziness/vertigo would have a higher chance of misdiagnosis in the emergency department. Even though several diagnostic algorithms have been advanced for acute vascular vertigo, we still aw…

Acute continuous vertigo.

Acute continuous vertigo presentations are among the most feared presentations in medicine. Although a self-limited disorder is the typical cause, a life-threatening stroke can also occur. Differentiating a self-limited disorder from a life-threatening stroke can be a challenge. Routinely collected information--such as stroke risk factors and findings on the general neurologic examination--is not likely to enable the clinician to discriminate between these causes. A focused oculomotor examination is a necessary component of the assessment, but is underused in routine care. The author describes…
