BACKGROUND: Labyrinthitis is an inner ear disorder of unknown incidence, characterized by sudden hearing loss and concurrent vertigo. Cohort studies of patients diagnosed with labyrinthitis are nonexistent. This study aims to describe the clinical characteristics and prognosis of patients diagnosed with idiopathic labyrinthitis.
METHODS: Patients with labyrinthitis in the absence of a clear viral, bacterial, or autoimmune pathogenesis were retrospectively identified from electronic patient files. Symptoms at presentation and results from vestibular testing were retrieved. The 9-item Vestibula

An intravestibular lipoma was discovered during destructive surgery for disabling vertigo in a case diagnosed as delayed endolymphatic hydrops. The author recommends that in cases that resemble Meniere's disease but have unusual symptoms, the existence of other lesions within the inner ear should be suspected.

Horizontal optokinetic after-nystagmus (OKAN) was studied in normal volunteers and in patients with either unilateral or bilateral loss of labyrinthine function. Optokinetic stimulation in normal volunteers produced OKAN in the same direction as preceding optokinetic nystagmus (OKN) with a duration of 45-90 seconds and a slowly decreasing slow phase eye velocity. Similar stimulation in the bilateral labyrinthine loss group produced markedly diminished or no OKAN. In the unilateral group when the optokinetic stimulus produced OKN with fast phase to the affected ear, OKAN was markedly diminished

An automatic, computerized system for analysis of the ocular movements in nystagmus, slow pursuit and saccades is presented. The performance of the system has been tested in healthy subjects, as well as in patients with peripheral or brainstem audio-vestibular disorders. The automatized procedure enables an extended application of opto-motor tests in the clinical examination of patients with suspected retrocochlear and brainstem involvement.

Seventy-one patients with an unilateral caloric hypofunction were examined in order to establish the correlation with cochlear function. We divised our cases following the subjective clinical syndromes. Groupe A (12 patients) with a history of only one crise of vertigo. Groupe B (13 patients) with repeated vertiginous attacks. Groupe C (21 patients) with a positional vertigo only. Groupe D (15 patients) with rather undefinite complaints. Groupe E (7 patients) with no vertigo at all. We believe to discern the following syndromes in our cases of unilateral vestibular hypofunction: 9 cases with a

The aim of this paper was evaluation of vertigo frequency in otosclerotic patients (group I N = 64) in comparison with control group (group II N = 20) and in people after stapedectomy (group III N = 64). Furthermore electronystagmographic recording spontaneous nystagmus, positional nystagmus were made. Computer analyses of ENG-recording and automatic calculation of directional preponderance and canal paresis were used. Vertigo was in 12% otosclerotic patients while in equal of age control group only in 5% subjects. Spontaneous nystagmus in 20% and positional nystagmus in 27% were recorded in o

The neurosensory structures within the vestibular labyrinth of the inner ear are selectively irradiated with ultrasonic energy to alleviate the vertiginous symptoms of Meniere disease. However, correct application and coupling of the ultrasonic transducer to the inner ear is essential for maximum transmission of energy into the vestibule. The importance of assessing the results of long-term postoperative treatment is demonstrated by a change in the success rate that decreased by approximately 10% when the results obtained within the first 12 months of treatment were compared with those after f

Perilymph fistulas may manifest themselves either by otologic symptoms, as in cases of small fistulas, or primarily through neurologic symptoms, as with large fistulas. A case history is presented of a 3-year-old girl with recurrent bouts of meningitis who was found to have a large perilymph fistula. She had a right undeveloped cochlea and labyrinth, as well as multiple defects of the medial wall of her middle ear. The only way of preventing the recurrent bouts of bacterial meningitis in cases of large perilymph fistulas is surgical repair. Our conclusion is that, considering the problems invo

Perilymph fistula is caused by changes of cerebrospinal fluid pressure and/or middle ear pressure. For diagnosis, history taking is extremely important in regard to whether the occurrence of symptoms is related to physical exertion, such as straining, nose blowing, sneezing etc. A variety of symptoms are due to pathologic changes of the membranous labyrinth. Exploratory tympanotomy is needed to verify the occurrence of leakage. However, perilymph fistula cannot be excluded, even if leakage is not observed. Management consists of absolute rest and closure of the fistula. If dizziness or vertigo

This article reviews the interrelationship between panic disorder and vestibular function. There is a possibility of both somatopsychic and psychosomatic interactions between panic and the vestibular system. Another possibility is that vestibular dysfunction could be associated with certain mental disorders, including panic disorder, as a nonspecific marker. Somatopsychic interactions are suggested by findings of high prevalence of vestibular dysfunction in selected patients with panic disorder, by the occurrence of "space and motion phobia" in patients with panic disorder, and by the report o

At its recent spring meeting, the American Neurotologic Society devoted a portion of its program to a symposium on staging Meniere's disease and reporting results. One of the problems discussed, was which preoperative audiogram should be used for reporting results of any treatment. I think that it is very important that we, as otologists, try to look at Meniere's disease or any inner ear dysfunction in a more scientific way. Some of you may feel, on initial evaluation of this material, that it is too cumbersome to be practical in your daily clinical work. However, we feel we have put forth a r

OBJECTIVE: To describe a case of traumatic pneumolabyrinth and subsequent hearing recovery after ejection of air with transcanal endoscopic surgical exploration.
PATIENTS: A 38-year-old man was struck by his child while cleaning his ear with an ear pick made of bamboo, which penetrated deep into the left ear canal. Severe vertigo with vomiting and left hearing impairment ensued. In addition, high-resolution computed tomography demonstrated an air density within the vestibule.
INTERVENTIONS: Exploratory tympanotomy was performed endoscopically a day after the injury and air was ejected from t

The connections between the two vestibular nuclei permit vestibular compensation after unilateral lesions. The interactions between the vestibular and the visual and proprioceptive system can support vestibular compensation. Our physical training program for the treatment of vertigo is based on these mechanisms. The training consists of fixation exercises, smooth pursuit and motor learning by proprioceptive cognition. The results are very convincing.
