Suicide Risk Evaluations and Suicide in the Veterans Health Administration.

IMPORTANCE: The Veterans Health Administration (VHA) implemented the Comprehensive Suicide Risk Evaluation (CSRE) in 2019 to standardize suicide risk assessment across the health care system. Identifying CSRE responses associated with suicide could inform risk management and prevent suicide. OBJECTIVE: To identify CSRE responses associated with subsequent suicide. DESIGN, SETTING, AND PARTICIPANTS: This cohort study examines acute (within 30 days) and chronic (within 365 days) suicide risk after 269 374 CSREs were administered. Participants included US VHA patients undergoing CSRE evaluation…

Insomnia in the Veteran Population: A Sleep Health and Wellness Intervention.

The purpose of this evidence-based project (EBP) was to determine if an evidence-based sleep health and wellness intervention improved sleep in veterans self-reporting a history of insomnia. Insomnia can negatively affect an individual's physical and psychological well-being, as well as increase health-care costs and decrease the overall quality of life. The intervention utilized a combination of insomnia treatments, delivered to two American Legion veteran participant groups: an Alabama American Legion Retreat and individuals at an American Legion Post site. The holistic-focused modalities us

The mental health of sons and daughters of Australian Vietnam veterans.

BACKGROUND: War service increases the risk of post-traumatic stress disorder (PTSD) to combatants, and has been shown to increase the risk of PTSD in their offspring. The extent to which there is an excess compared with the general population is not yet established, nor whether PTSD increases the risk of other psychiatric problems. METHODS: A national sample of 133 sons and 182 daughters of a cohort of 179 Australian Vietnam veterans' families were assessed in person, using structured psychiatric interviews. The prevalence of trauma exposures, DSM-IV (Diagnostic and Statistical Manual of Ment

Understanding the health of veterans who are homeless: A review of the literature.

The United States Department of Housing and Urban Development estimates that almost 50,000 veterans are homeless on any given night. Homeless veterans are at greater risk of health disparities than their housed counterparts due to the multifactorial nature of their health and social needs. The Department of Veterans Affairs, in collaboration with more than a dozen other federal agencies, has concentrated efforts to improve the health of this vulnerable population while enacting a plan to eliminate veteran homelessness within the near future. Understanding the unique health needs of veterans wh

Variations in the use of cardiac procedures in the Veterans Health Administration.

BACKGROUND: Considerable variability exists in the use of cardiac procedures for patients with heart disease. One cause for this variability is the availability of local facilities to perform these procedures. This study was initiated to identify health system features that are related to rates of catheterization, percutaneous coronary angioplasty, and coronary artery bypass graft surgery in the Veterans Affairs health care system in which structured referral systems are intended to compensate for variation in local resource availability. METHODS: Medical records of 30,901 patients admitted t

Prevalence and multi-morbid correlates of homelessness among veterans with HIV infection nationally in the veterans health administration.

This study sought to evaluate the risk of homelessness among HIV positive users of Veterans Health Administration (VHA) services. National VHA administrative data from Fiscal Year (FY) 2012 (N = 5.4 million) were used to: (1) evaluate the unadjusted risk of homelessness among veterans with HIV; (2) identify common multi-morbid correlates of both HIV diagnosis and homelessness; and (3) to assess the independent risk of homelessness among veterans with HIV after adjusting for risk factors common to both conditions. Veterans with HIV were at substantial risk of homelessness (OR = 4.23 (95% CI 4.0…

Mental Health Impact of Homecoming Experience Among 1730 Formerly Deployed Veterans From the Vietnam War to Current Conflicts: Results From the Veterans' Health Study.

We examined the effects of homecoming support on current mental health among 1730 deployed veterans from Vietnam, Iraq/Afghanistan, Persian Gulf, and other conflicts. The prevalence of current posttraumatic stress disorder (PTSD) was 5.4%, current depression was 8.3%, and 5.4% had suicidal thoughts in the past month. Overall, 26% of veterans had low homecoming support, which was more prevalent among Vietnam veterans (44.3%, p < 0.001). In multivariable logistic regressions, controlling for demographics, combat exposure, number of deployments, trauma history, and operational theater, low postde

Consistent Honor, Persistent Disadvantage: American Indian and Alaska Native Veteran Health in the National Survey of Veterans.

Objective: To examine self-rated health and activities of daily living (ADLs) limitations among American Indian and Alaska Native (AI/AN) veterans relative to white veterans. Methods: We use the 2010 National Survey of Veterans and limit the sample to veterans who identify as AI/AN or non-Hispanic white. We calculated descriptive statistics, confidence intervals, and used logistic regression. Results: AI/AN veterans are younger, have lower levels of income, and have higher levels of exposure to combat and environmental hazards compared to white veterans. We found that AI/AN veterans are signif

Exploring PrEP Uptake in Young Women Veterans: Insights From a Detailed Chart Review Within the Veterans Health Administration.

INTRODUCTION: Uptake of HIV pre-exposure prophylaxis (PrEP) within the US Veterans Health Administration (VHA) remains modest, especially among younger women. Younger women face a disproportionately high risk of HIV infection due to biological, behavioral, and structural factors and experience the greatest barriers to PrEP access compared with older women. To examine gaps in PrEP delivery for this high-risk group, we conducted in-depth chart reviews of patients younger than 26 years identified as female at birth. METHODS: Among all VHA patients prescribed PrEP from 2012 to 2022, 24 were young…

Intimate partner violence in United States military veterans: Results from the National Health and Resilience in Veterans Study.

Intimate partner violence (IPV) is a significant concern among military personnel and veterans, yet few studies have examined its prevalence and correlates in nationally representative samples of veterans. This study aimed to address this gap by identifying the prevalence of IPV and factors associated with lifetime physical IPV perpetration, expanding on prior research by including a broader range of traumatic experiences (e.g., military sexual trauma) and psychiatric variables (e.g., nicotine dependence, non-suicidal self-injury). Results revealed that, of 2376 veterans surveyed, 208 (weighte
