The Ka'u District of Hawaii is exposed to sulfurous air pollution called vog from the ongoing eruption of Kilauea Volcano. Increased volcanic activity in 2008 prompted an indoor air quality assessment of the district's hospital and schools. All indoor sulfur dioxide concentrations were above the World Health Organization's average 24-hour recommendation. Indoor penetration ratios were up to 94% of ambient levels and dependent upon building construction or the use of air-conditioning. Health-promotion efforts for vulnerable populations at the hospital and schools are under way to improve indoor…

Health-related quality of life (HRQoL) is a key outcome in managing people with HIV. This study examined how sociodemographic factors relate to HRQoL. People with HIV from a Dutch treatment center completed a questionnaire assessing eight HRQoL domains: general health, anxiety and depression, social support, stigma, sexuality problems, self-esteem, sleeping difficulties and side effects. Sociodemographic factors included gender, age, education, employment, religion, marital status, sexual orientation, sex of sexual partners, and year of HIV diagnosis. Multivariable regression assessed associat

RESULTS: Eleven statements were generated: six on features and risk factors of PCC in clinically vulnerable populations, two on diagnosis and management, and three on prevention. Chronic fatigue was identified as the most frequent presentation of PCC in PWH and RD populations. A different case definition of PCC is required for RD population, as symptoms of PCC and au …

BACKGROUND: The high demand for oral health care services among vulnerable individuals combined with limited available resources requires a rethinking of the provision of oral health services. This study aimed to determine the usefulness of the dental panoramic radiograph (DPR) as an imaging tool to assess oral health and prioritize dental interventions in vulnerable patients.
METHODS: This observational and retrospective study evaluated charts of patients who attended Public Health Dental Clinics (PHDC), Alberta Health Services (AHS), in Calgary, Canada, between January 2018 and December 201

BACKGROUND: Studies investigating vulnerable populations have shown that work-related musculoskeletal disorders have a negative impact on quality of life. However, no study has examined the body regions commonly affected by work-related musculoskeletal disorders in vulnerable populations.
OBJECTIVE: To describe the body regions commonly affected by work-related musculoskeletal disorders in vulnerable populations.
METHODS: Cross-sectional study. We used the ABEP questionnaire, the World Health Organization Quality of Life, the Nordic Musculoskeletal Disorders Questionnaire, the Perceived Stre…

BACKGROUND: The Centers for Medicare & Medicaid Services Bundled Payments for Care Improvement (BPCI) initiative tested whether episode-based payment models could reduce Medicare payments without harming quality. Among patients with vulnerabilities, BPCI appeared to effectively reduce payments while maintaining the quality of care. However, these findings could overlook potential adverse patient-reported outcomes in this population.
RESEARCH DESIGN: We surveyed beneficiaries with 4 characteristics (Medicare-Medicaid dual eligibility; dementia; recent institutional care; or racial/ethnic minor

Research assistants (RAs) play a variety of roles that are critical in making research happen and in determining its quality and effectiveness. Yet their locus of power in the production of knowledge stands in sharp contrast to their relative powerlessness in the hierarchical research organization. This article explores the experiences of RAs engaged in a randomized controlled longitudinal field trial of a Housing First intervention for individuals experiencing homelessness and mental illness in Toronto. They encountered several unexpected effects of navigating the power ascribed to them by bo

AIM: This paper critically examines the Australian College of Nursing's proposed socialised healthcare model and the potential of nurse-led socialised care as a paradigm shift for addressing the intersection of medical need and social distress among vulnerable populations experiencing homelessness, domestic violence, poverty, and other disadvantage.
APPROACH: This is a position paper. It offers a theoretical commentary and critical reading that situates the proposed socialised healthcare model within frameworks of social determinants of health, health equity, and socially responsive care deli…

The emergency department evaluates many patients with undiagnosed cognitive impairment and presents an opportune setting to facilitate early detection and referral to memory care specialists. We evaluated a novel care navigation pathway that facilitated referrals of ethnoculturally diverse individuals with suspected cognitive impairment from geriatric professionals embedded in the emergency department to dementia specialist care. We compared rates of referrals and successful appointment attendance for patients in this pathway compared to patients in a traditional primary care provider referral

BACKGROUND: Duchenne muscular dystrophy (DMD) is the most common fatal pediatric neuromuscular disorder, resulting in premature morbidity and mortality. Multidisciplinary care and new therapies have prolonged quantity and quality of life, for which many families remain dependent on Medicaid for access to standards of care. Proposed Medicaid funding cuts would disproportionately harm children with complex medical conditions, including DMD, jeopardizing their access to essential services and specialized care. Children with DMD rely on comprehensive multidisciplinary care, including neurology, ca

Background: Even 3 years into the COVID-19 pandemic, questions remain about how to safely and effectively vaccinate vulnerable populations. A systematic analysis of the safety and efficacy of the COVID-19 vaccine in at-risk groups has not been conducted to date. Methods: This study involved a comprehensive search of PubMed, EMBASE, and Cochrane Central Controlled Trial Registry data through 12 July 2022. Post-vaccination outcomes included the number of humoral and cellular immune responders in vulnerable and healthy populations, antibody levels in humoral immune responders, and adverse events.…

The scope of this study was to analyze the trend, spatialization and circumstances associated with violence against vulnerable populations in Brazil between 2009 and 2017. Descriptive analysis, spatialization and polynomial regression were used. A total of 1,116,331 cases of violence were notified, with an increase of 667.5% over the period. In all populations under research there was a predominance of physical violence against women with low levels of schooling, perpetrated by men in residences. Sexual and psychological violence was highlighted in female and LGBT populations. Negligence, howe…

Mass care on an epic scale - we thought we had seen it all during Hurricanes Katrina, Rita, Ike and then Sandy. However, catastrophic scales reached a new level during Hurricane Harvey. How did the response and recovery to Harvey differ from previous hurricane events and what lessons were (and were not) learned? This paper examines the impact on the delivery of medical care to the vulnerable populations in Harvey's path.

BACKGROUND: Supplementary feeding may help food insecure and vulnerable people by optimising the nutritional value and adequacy of the diet, improving quality of life and improving various health parameters of disadvantaged families. In low- and middle-income countries (LMIC), the problems supplementary feeding aims to address are entangled with poverty and deprivation, the programmes are expensive and delivery is complicated.
OBJECTIVES: 1. To summarise the evidence from systematic reviews of supplementary feeding for food insecure, vulnerable and malnourished populations, including children

To address barriers to adequate engagement in medical care among people living with HIV, Wisconsin's AIDS/HIV Program created a new position, the Linkage to Care (LTC) Specialist. Specialists provide intensive, short-term case management and patient navigation services for small caseloads of individuals at high risk of disengaging with medical care. Clients are eligible if they are newly diagnosed with HIV or new to medical care, recently released from incarceration, recently out of care, nonadherent to scheduled medical care visits, or have detectable viral load while in care.
