Timing of surgery in valvular heart disease: prophylactic surgery vs watchful waiting in the asymptomatic patient.

In the absence of randomized controlled trial data, the management of patients with severe valvular heart disease without symptoms, ventricular dysfunction, or other identified triggers for surgery is controversial. In this review, we frame the debate between prophylactic surgery vs close follow-up until triggers occur (watchful waiting) for severe aortic stenosis and degenerative mitral regurgitation (MR), the 2 conditions for which the pros and cons of these approaches are best articulated. Classic high-gradient severe aortic stenosis is generally accurately diagnosed. In asymptomatic patien…

MRI Monitoring of Locally Advanced Rectal Cancer in Watch and Wait Patients.

BACKGROUND: Watch and wait (W&W) is an alternative strategy for locally advanced rectal cancer (LARC) (T3-4/N+) patients. MRI-based evaluation of clinical complete response (cCR) and post-cCR disease monitoring remain uncertain. This study aims to investigate the association between MRI feature changes and survival outcomes in these patients. METHODS: LARC Patients achieving cCR after neoadjuvant chemoradiotherapy and opting for W&W were included. MRI features were recorded from pretreatment to the follow-up period, including tumor bed scarring on post-treatment T2-weighted imaging (T2WI), ma

Organ preserving watch-and-wait strategy in the treatment of rectal cancer Brno.

Watch-and-wait (WW) strategy offers an alternative to radical resection with total mesorectal excision (TME) in selected patients with distal rectal adenocarcinoma after achieving complete clinical response (cCR) to neoadjuvant therapy. This approach is based on intensive follow-up, where a multidisciplinary team, especially the surgeon, is confronted with a demanding follow-up regimen including repeated anorectoscopies, per rectum examinations and magnetic resonance imaging. The prediction of pathological complete response in cCR is particularly problematic. The risk of recur-rence (regrowth)

Is watch and wait still acceptable for patients with low-grade follicular lymphoma?

Follicular lymphoma (FL) represents more than 20% of all non-Hodgkin lymphomas worldwide and approximately 30% of the non-Hodgkin lymphomas diagnosed in the United States. Although occasionally localized at the time of diagnosis, most patients have disseminated disease. However, patients are frequently asymptomatic, and this, in combination with a long median survival, led to the initial studies of observing asymptomatic patients without initial therapy, ie, "watch and wait." Since the initial report of watch and wait as a treatment strategy for patients with low-grade FL, our understanding of

The Risk of Distant Metastases in Patients With Clinical Complete Response Managed by Watch and Wait After Neoadjuvant Therapy for Rectal Cancer: The Influence of Local Regrowth in the International Watch and Wait Database.

BACKGROUND: Nearly 30% of patients with rectal cancer develop local regrowth after initial clinical complete response managed by watch and wait. These patients might be at higher risk for distant metastases. OBJECTIVE: This study aimed to investigate risk factors for distant metastases using time-dependent analyses. DESIGN: Data from an international watch and wait database were retrospectively reviewed. Cox regression analysis was used to determine risk factors for worse distant metastases-free survival. Conditional survival modeling was used to investigate the impact of risk factors on the

Watchful waiting as a strategy to reduce low-value spinal imaging: study protocol for a randomized trial.

BACKGROUND: Patients with acute low back pain frequently request diagnostic imaging, and clinicians feel pressure to acquiesce to such requests to sustain patient trust and satisfaction. Spinal imaging in patients with acute low back pain poses risks from diagnostic evaluation of false-positive findings, patient labeling and anxiety, and unnecessary treatment (including spinal surgery). Watchful waiting advice has been an effective strategy to reduce some low-value treatments, and some evidence suggests a watchful waiting approach would be acceptable to many patients requesting diagnostic test…

Methotrexate-associated lymphoproliferative disorders: management by watchful waiting and observation of early lymphocyte recovery after methotrexate withdrawal.

No optimum treatment of iatrogenic immunodeficiency-associated lymphoproliferative disorders due to methotrexate in patients with rheumatoid arthritis (MTX-LPD) has yet been established, although MTX withdrawal is known to have a substantial effect on tumor regression. Here, we retrospectively analyzed 20 cases of MTX-LPD. Tumor shrinkage occurred in 18 of 20 cases, but only following MTX withdrawal. This tumor regression ratio was considerably better than in previous reports, and appeared due to longer "watchful waiting." Lymphocyte recovery at 2 weeks after MTX withdrawal was significantly h…

A Case of Chylothorax without Organic Changes in the Peripancreatic Region Following Pancreatitis Managed with Watchful Waiting.

A 77-year-old man was referred for an evaluation of persistent left-sided pleural effusion that progressively increased after treatment for acute alcoholic pancreatitis, which had been managed without any intra-abdominal procedures. Thoracentesis yielded a milky pleural fluid with markedly elevated triglyceride levels, consistent with a diagnosis of chylothorax. Although lymphangiography failed to identify the exact site of chyle leakage, chylothorax was considered to be associated with pancreatitis, as no alternative causes were identified. Chylothorax can develop after pancreatitis, even in

Case report on complex jewelry foreign body ingestion in pediatric population: When watchful waiting prevails over intervention.

Foreign body ingestion (FBI) is a frequent pediatric emergency that often requires immediate, invasive endoscopic or surgical extraction when the object is large, multi-linked, or has sharp components. This case report highlights the safety and efficacy of an evidence-based watchful waiting protocol for a complex jewelry ingestion in an asymptomatic older child. An asymptomatic 8-year-old male presented to the emergency department 2 hours after accidentally swallowing a multi-linked metallic bracelet. Initial abdominal radiography 2 hours post-ingestion localized the radiopaque object to the p…

Adenotonsillectomy vs Watchful Waiting in Pediatric Mild to Moderate Obstructive Sleep Apnea: The KATE Randomized Clinical Trial.

IMPORTANCE: Adenotonsillectomy (ATE) is the standard procedure to treat children with obstructive sleep apnea (OSA). OBJECTIVE: To investigate whether ATE is more effective than WW for treating otherwise healthy children with mild to moderate OSA. This randomized clinical trial (RCT) of young children investigated the benefit of ATE compared with watchful waiting (WW) after 3 years. DESIGN, SETTING, AND PARTICIPANTS: The Karolinska Adenotonsillectomy (KATE) RCT was conducted between 2014 and 2020 with a 3-year follow-up and included children aged between 2 to 4 years with an Obstructive Apne…

Comparing Early Intervention to Watchful Waiting: A Review on Risk Stratification and Management in Asymptomatic Aortic Stenosis.

Aortic stenosis is a progressive condition with substantial implications for morbidity and mortality. In recent years, attention has shifted toward risk stratification and the development of individualized management plans to optimize treatment outcomes. The management of asymptomatic patients has become a topic of significant controversy, as emerging studies challenge traditional watchful waiting guidelines and propose the potential benefits of early intervention. While early intervention may reduce overall morbidity and mortality in this patient population, the associated procedural risks re…

When less is more: Updates in active surveillance and watchful waiting in the management of prostate cancer.

BACKGROUND: Prostate cancer is the second most common cancer among men globally. A range of management options are available for prostate cancer, including surgery, radiation therapy, hormone therapy, chemotherapy, or surveillance. Conservative strategies include active surveillance and watchful waiting, which differ in their intent. OBJECTIVE: We provide a targeted instructive management algorithm for improving understanding of conservative strategies in prostate cancer. DISCUSSION: Active surveillance involves close monitoring with curative intent when there is evidence of disease progress…

Can urodynamic assessment of outflow obstruction predict outcome from watchful waiting?--A four-year follow-up study.

OBJECTIVE: One of the most common "treatment" alternatives in suspected outflow obstruction due to bladder outlet obstruction (BOO) is watchful waiting (WW). The aim of this study was to see whether there were any differences in outcome between patients with slight, moderate or severe obstruction due to BOO as classified by transrectal ultrasound (TRUS) and urodynamics. MATERIAL AND METHODS: Thirty-seven men with lower urinary tract symptoms (LUTS) and suspected BOO were included. All of the patients were investigated by a routine investigation schedule, including TRUS and urodynamics with pr…

The cost-effectiveness of early surgery versus watchful waiting in the management of small abdominal aortic aneurysms.

PURPOSE: The purpose of this study was to compare the relative cost-effectiveness of two clinical strategies for managing 4 to 5 cm diameter abdominal aortic aneurysms (AAAs): early surgery (repair 4 cm AAA when diagnosed) versus watchful waiting (monitor AAA with ultrasound size measurements every 6 months and repair if the diameter reaches 5 cm). METHODS: We used a Markov decision tree to compute the expected survival in quality-adjusted life years (QALYs) for each strategy, based on literature-derived estimates for the probabilities of different outcomes in this model. We determined hospit…

Living with Cancer under watch and wait: a qualitative systematic review and thematic synthesis.

OBJECTIVE: Watchful waiting, sometimes referred to as active surveillance or observation, is used across a range of cancer contexts to avoid or delay unnecessary treatment and its associated harms. Although this approach may reduce overtreatment, it can also place patients in the unusual position of living with a known cancer that is being monitored rather than immediately treated. This may have important psychological and social consequences. The aim of this review was to synthesize qualitative research exploring patients' experiences of watchful waiting across cancer contexts. METHODS: We c…

"Watch-and-Wait" or "Lost to Follow-Up"? Adherence to Nonoperative Management of Rectal Cancer.

INTRODUCTION: Due to potential for local regrowth in patients undergoing nonoperative management (NOM) of rectal adenocarcinoma, current guidelines recommend offering close surveillance in a "watch-and-wait" paradigm. These regimens require many visits, potentially posing significant burden on patients. There is a paucity of data regarding optimal frequency of examinations. We sought to determine adherence to recommended surveillance, identify predictors of poor adherence, and evaluate its association with on oncologic outcomes during the first 2 y after clinical complete response (cCR). Duodenal neuroendocrine neoplasms with unexpectedly aggressive behaviour: Challenging the "watch and wait" approach.

The incidence of duodenal neuroendocrine neoplasms has risen over the past decades as a result of the wide availability of endoscopy and associated expertise. Although it is considered that tumour size greater than 10 mm, higher tumour grade and/or location in relation to the ampulla of Vater represent the main risk factors for local or distant metastases, we describe two cases of well differentiated grade 1 and grade 2 neuroendocrine tumours, which measured < 10 mm at the time of diagnosis but had an aggressive course during follow-up. Furthermore, we also summarise the available therapeutic
