Postpregnancy genital tract and wound infections.

Despite the widespread application of standard aseptic techniques during vaginal birth, cesarean birth, and/or termination of pregnancy, postpregnancy infections remain a significant source of maternal morbidity and mortality. Obstetric infection accounts for more than 12% of maternal deaths. Infection occurs most frequently in women who have cesarean births, and following spontaneous or elective termination of pregnancy. Infection is estimated to be the second highest cause of under-reported maternal death in the United States. This article identifies measures to aid in primary prevention and

Nutritional support for patients with infected wounds.

Many factors affect wound healing, including nutritional status and recent nutritional intake. Patients with infected wounds have increased requirements for nutrients and often have a reduced food intake. It is therefore important that nutritional status is carefully considered, and all patients should be screened for malnutrition using a nutrition screening tool. Nutrition plans should aim to provide sufficient energy while maintaining good glycaemic control, adequate protein and hydration, and a minimum of 100% reference nutrient intake for micronutrients. Measures required to achieve this w

Current issues in burn wound infections.

As we have emphasized, the diagnosis of burn wound infections in the high-risk burned child can be difficult and depends on a very high degree of suspicion and daily clinical evaluation of the burn wound site by consistent observers. Appropriate precautions include meticulous hand-washing and the use of gloves when handling the wound site and prophylactic application of a topical antibacterial agent such as SSD cream. Wound therapy should include routine vigorous surgical débridement. Surveillance wound cultures should be done weekly to determine the emergency of colonization and aid in the se

Bite wound infections of the lower extremity.

Animal bite wounds of the lower extremity are considered high-risk wounds for infection, especially in the medically compromised individual. Management should be aggressive. Copious irrigation and debridement of the wound clearly proves to be of greater value in the prevention of infection than the indiscriminate use of so-called "prophylactic" antibiotic agents. Frankly infected wounds must also be managed aggressively, incorporating the use of antibiotics to eradicate the infection. Empiric use of an agent containing clavulanic acid or sulbactam is suggested until results are obtained from t

Noncandidal, fungal infections of the burn wound.

Candidal (yeast) and noncandidal (filamentous fungal) wound infections have become an increasingly important cause of burn-associated morbidity and mortality. However, these two diseases differ markedly in their epidemiology, onset, appearance, diagnosis, and treatment. In the last four years, we have had five cases of noncandidal sepsis (four mucoraceae and one aspergillus) that are illustrative of these differences. Early recognition and diagnosis are essential. Radical excision when the infection is confined to superficial invasion of only one anatomic area is key to decreasing mortality fr

[Anaerobic clostridial infection of surgical wounds].

Two cases of anaerobic clostridial infection in operative wounds of the anterior abdominal wall in women of 65 and 86 are presented. The infection developed by the second or third day following surgical intervention for small intestine ileus. Death occurred with septic shock manifestations 10-13 hours after the onset of the first clinical symptoms of infectious complications which were contributed to by late hospitalization of the patients, inadequate antibacterial treatment as well as age-dependent decrease of immunologic responsiveness. An increasing incidence of anaerobic operative wounds i

The prevention of wound infections in burn patients.

To investigate the necessity of "protective isolation" and/or gastrointestinal tract decontamination in the prevention of infections in burn patients treated under ward conditions, a bacteriological inventory of burn sites, oropharynx and faeces was made 3 times a week. The origin of bacteria isolated from the lesions was determined by typing. In addition, the effect of local protection by topical application of silver-sulphadiazine or nitrofuralum was investigated. Of potentially pathogenic bacteria known to cause infections in burn, St. aureus was found to be almost exclusively exogenous in

Early wound excision within three days decreases risks of wound infection and death in burned patients.

INTRODUCTION: In lieu of limited studies on the timing of burn wound eschar excision for burns, a more comprehensive analysis is indicated to determine the effects of early wound excision following burns. This study aims to address the outcomes of early wound excision in burn patients. METHODS: Data collection were from TriNetX research database. Three groups of burn patients were stratified by the number of days in which they received burn wound excision within 14 days of injury. Five outcomes were observed: death, wound infection, sepsis, myocardial contractile dysfunction, and blood transf

Wound Disruption after Lower Extremity Bypass Surgery is a Predictor of Subsequent Development of Wound Infection.

BACKGROUND: Despite advances in endovascular surgery, lower extremity arterial bypass (LEB) remains the gold standard treatment for severe, symptomatic Peripheral Arterial Disease. With recent changes in health care, there has been an increasing emphasis on reducing the hospital length of stay (LOS). The purpose of this study was to identify the postoperative complications, which occur after discharge from hospital and to find risk factors for developing such complications. METHODS: The 2013 lower extremity revascularization-targeted American College of Surgeons-National Surgical Quality Impr

Risk factors for wound infection after burn plastic surgery.

This study aims to analyze the risk factors for wound infection in patients after burn plastic surgery and explore corresponding intervention measures to provide evidence for reducing infection rates in clinical practice. A retrospective study was conducted on 320 patients who underwent burn plastic surgery at SanTai County People's Hospital from January 2022 to December 2023. Patients were divided into an infection group (86 cases) and a noninfection group (234 cases) based on the occurrence of wound infection postoperatively. Data on age, gender, comorbidities (e.g., diabetes, hypertension),

Wound Chronicity, Impaired Immunity and Infection in Diabetic Patients.

BACKGROUND: Diabetic foot ulcers are a common diabetic complication leading to alarming figures of amputation, disability, and early mortality. The diabetic glucooxidative environment impairs the healing response, promoting the onset of a 'wound chronicity phenotype'. In 50% of ulcers, these non-healing wounds act as an open door for developing infections, a process facilitated by diabetic patients' dysimmunity. Infection can elicit biofilm formation that worsens wound prognosis. How this microorganism community is able to take advantage of underlying diabetic conditions and thrive both within

Measured Wound Outcome Feedback Improves Surgical Site Infection Rates.

Feedback supports learning. No matter how effective medical professionals think they are, evidence of measured patient wound outcomes helps inform their wound care practice, empowering them to improve patient outcomes. Prospective randomized clinical trials (RCTs) proved this principle in relation to healing chronic wounds. Measuring wound healing outcomes and providing caregivers with feedback about the 4-week healing progress increased the percentage of diabetic foot ulcers healed within 20 weeks and venous leg ulcers healed within 24 weeks. Longitudinal research suggests this principle hold

Wound Infection with an Unusual Pathogen after Liver Transplantation.

Mucormycosis is a rare and highly invasive fungal infection caused by Mucorales fungi of the class Zygomycetes. Cutaneous mucormycosis typically has a good survival rate when diagnosed early. In this report, we presented a patient with surgical site mucormycosis after liver transplant surgery. Our patient was a 50-year-old man with cirrhosis due to nonalcoholic steatohepatitis who received liver transplant from a deceased donor. On the 8th day of transplant, the patient had fever and purulent discharge from the surgical site. The wound became black and necrotic in the next day. A microbiologic

Silver oxysalts promote cutaneous wound healing independent of infection.

Chronic wounds often exist in a heightened state of inflammation whereby excessive inflammatory cells release high levels of proteases and reactive oxygen species (ROS). While low levels of ROS play a fundamental role in the regulation of normal wound healing, their levels need to be tightly regulated to prevent a hostile wound environment resulting from excessive levels of ROS. Infection amplifies the inflammatory response, augmenting levels of ROS which creates additional tissue damage that supports microbial growth. Antimicrobial dressings are used to combat infection; however, the effects

Pasteurella multocida wound infection transmitted by a pet dog.

INTRODUCTION: Human pasteurellosis is a severe human infection that accounts for 20-30 human deaths annually worldwide. Mucous secretions derived from pets comprise the primary source of infection, which are transmitted through animal scratches or bites. CASE REPORT: We describe a case of Pasteurella multocida wound infection in an immunocompetent adult with a decubitus ulcer of the lower extremity. The organism was also isolated from an oral swab of the patient's dog with which he reported close contact. The patient had a favorable outcome following aggressive surgical debridement, antimicro
