A total of 535 evaluable febrile episodes in neutropenic patients were randomly assigned to treatment with ticarcillin-clavulanate plus vancomycin (TV), ceftazidime plus vancomycin (CV), or all three antibiotics (TCV). Combinations of beta-lactams with aminoglycosides are widely used in the therapy of infections. There are a number of theoretic advantages to using such combinations. Indeed, combinations of beta-lactams and aminoglycosides usually result in synergism or indifference and very rarely in antagonism. The mechanisms of synergism, resistance to synergism, and antagonism have been studied extensively in enterococci; further studies are necessary to define such mechanisms in other organisms.

METHODS: Hospital pharmacists were asked to provide data on antimicrobial use and bacterial susceptibilities. Cefsulodin was the most active of the cephalosporins and exhibited 4-16 times more activity than carbenicillin or ticarcillin against 50 clinical isolates of Pseudomonas aeruginosa. Azlocillin and piperacillin showed good activity, while tobramycin was the most effective aminoglycoside. The activity of cefsulodin was unaltered by increases in inocula, but resistance was induced easily. When combined with gentamicin, no synergistic or antagonistic activity was observed against multiply resistant isolates.

The effects of combinations of beta-lactams with two beta-lactamase inhibitors, sulbactam and clavulanic acid, were determined in vitro against 22 clinical isolates of methicillin-resistant Staphylococcus aureus. A detailed kinetic study of the interaction between two ethylidene derivatives of tricyclic carbapenems, Lek 156 and Lek 157, and representative beta-lactamases and D-alanyl-D-alanine peptidases (DD-peptidases) is presented. Both compounds are very efficient inactivators of the Enterobacter cloacae 908R beta-lactamase, which is usually resistant to inhibition. Nineteen isolates of Klebsiella oxytoca were examined, representing 18 distinct strains. PURPOSE: This case-case-control study aims to identify clinical predictors for pneumonia due to Pseudomonas aeruginosa (PA) which is (1) susceptible to all routinely tested antipseudomonal beta-lactams (APBL-S) and (2) resistant to at least one antipseudomonal beta-lactam (APBL-R).
