Rotigotine transdermal patch in the treatment of Parkinson's disease and restless legs syndrome.

Dopamine agonists are effective in delaying levodopa-induced dyskinesia in early Parkinson's disease (PD) and reducing motor fluctuations in advanced PD. Rotigotine, a novel dopamine receptor agonist, improves motor function in both early and advanced PD using a transdermal route of administration. A smaller, but convincing body of data, supports its ability to ameliorate the symptoms of restless legs syndrome as well. The side-effect profile mimics other dopamine agonists, with the addition of application-site reactions, most of which are mild-to-moderate. Advantages over existing dopamine ag

Novel cutaneous uses for botulinum toxin type A.

Botulinum toxin type A is a neurotoxin produced by the bacterium Clostridium botulinum which causes a flaccid muscle paralysis. It has been used extensively in the field of dermatology for the treatment of dynamic rhytides and in the treatment of hyperhidrosis. Botulinum toxin has an excellent safety profile and few side effects when used for these purposes. Recently, botulinum toxin has also been used experimentally in a number of other dermatologic conditions with good results. These conditions include: persistent facial flushing, gustatory sweating and epiphora, anal fissures, familial beni

Treatment of cutaneous leishmaniasis among travellers.

Leishmaniasis is endemic in 88 countries on five continents. There are 1-1.5 million cases of cutaneous leishmaniasis reported yearly worldwide. There has been a sharp increase in recorded cases over the last 10 years. Based on geographical distribution, cutaneous leishmaniasis is divided into Old World and New World leishmaniasis. In the past, species could be inferred from geographical setting or determined by performing culture and isoenzyme analysis. The recently developed and now widely available PCR technology allows a rapid diagnosis with determination of most species, and thus enables

Percutaneous ablation of hepatocellular carcinoma: state-of-the-art.

Percutaneous ablation is considered the best treatment option for patients with early-stage hepatocellular carcinoma (HCC) who are not candidates for surgical resection or liver transplantation. Several methods have been developed, including intratumoral injection of ethanol or acetic acid, and thermal ablation with radiofrequency, laser, microwaves, or cryosurgery. Percutaneous ethanol injection (PEI) has been the most widely used technique. Several series have provided indirect evidence that PEI improves the natural history of HCC. Patients with Child-Pugh class A cirrhosis and either a sing

Exogenous lipoid pneumonia caused by facial application of petrolatum.

Exogenous lipoid pneumonia is a well-described entity in the literature. To our knowledge, this is the first reported case that occurred secondary to external application of petrolatum to the face for erythrodermic psoriasis.

A case of post-transplant hyperparathyroidism treated with ethanol injection.

A 15-year-old boy with chronic renal failure secondary to Alport's syndrome underwent living-related renal transplantation from his 48-year-old father. His primary immunosuppressive regimen was composed of tacrolimus, mizolibine, and methylprednisolone. The postoperative course was satisfactory with one episode of mild acute rejection, treated successfully with methylprednisolone pulse therapy. Two months later, hypercalcemia (11.8-13.2 mg/dl) and hypophosphatemia (2.5-3.0 mg/dl) were noted without any bone symptoms. The serum intact-parathyroid hormone (PTH) and serum alkaline phosphatase lev

Topical glyceryl trinitrate: a possible treatment for cutaneous leishmaniasis.

The treatment of cutaneous leishmaniasis (CL) is difficult in both the old and new worlds. However, nitric oxide (NO) is involved in host cell mediated immune responses against intracellular parasites such as Leishmania major, and both in vitro and in vivo immunological studies indicate that Leishmania parasite killing by macrophages is mediated by this substance. Glyceryl trinitrate (GTN) is an exogenous NO donor; we have successfully treated a young man with cutaneous leishmaniasis with topical GTN. We believe this to be the first reported use of GTN in the treatment of human CL.

Successful treatment of pyoderma gangrenosum with topical 5-aminosalicylic acid.

Pyoderma gangrenosum in a twenty-nine-year-old woman with associated long-standing Crohn's disease was treated with topical 5-aminosalicylic acid cream. Significant improvement in the ulceration occurred despite progressive deterioration of the inflammatory bowel disease. The possible mode of action is reviewed. The efficacy, together with the lack of side effects, warrants further study into the therapeutic role of topical 5-aminosalicylic acid.

Bullous pemphigoid treated by topical corticosteroids.

Three patients with bullous pemphigoid have been successfully treated with topical corticosteroids (clobetasol propionate) alone. Another patient required adjuvant systemic corticosteroid therapy. In view of its minimal side effects, we confirm a previous report that topical corticosteroid therapy could represent a valuable alternative in the treatment of limited bullous pemphigoid, although for its extensive forms, such therapy may be incomplete.

Role of topical immunotherapy in the treatment of alopecia areata. Quality analysis of articles published between January 1977 and January 1988 about three treatments. Reading Group.

We conducted a survey of clinical trials to assess the scientific evidences presented for the practical use of dinitrochlorobenzene, squaric acid dibutylester, and diphencyprone in the treatment of alopecia areata. Twenty-six papers published between January 1977 and January 1988, in English, French, and Italian were selected. We used a standardized protocol of evaluation, which focused principally on the reporting of methods. Twelve papers were uncontrolled studies. Of the controlled studies, 11 had a self-controlled design, two studies involved the use of parallel concurrent controls, and se

Transdermal scopolamine attenuates methacholine-induced bronchospasm.

Anticholinergic agents have been widely used in the management of asthma. The use of scopolamine has been limited by significant side effects. Transdermal delivery of scopolamine (TS) has, however, been used successfully for the prevention of motion sickness. The purpose of this study was to determine if TS would decrease methacholine-induced bronchospasm in a group of subjects with mild asthma. Bronchoprovocational challenges with inhaled methacholine were performed on three separate occasions in 10 male subjects who each had a past history of asthma. After a baseline challenge, each subject
