The postoperative treatment of cataract surgery is an important element for success of the surgery. The postoperative therapy primarily aims at the prevention of intraocular bacterial infection by administration of antibiotics. Anti-inflammatory drugs can suppress the operation-related inflammation. In addition to fundamental risks of topical eye drop application, this review article discusses the current therapeutic strategies for the prevention of postoperative bacterial infection and suppression of surgically induced inflammation.

Topical administration of Isoproterenol reduces the intraocular pressure in glaucomatous eyes, but produces systemic side-effects (tachycardia arterial hypotension) that make its clinical use unadvisable. Such side-effects are significant when the concentration of Isoproterenol is 4 and 5%. Pre-treatment of the contralateral eye with a solution of Propranolol 1%, 30 and 60 min. before Isoproterenol instillation, markedly reduces systemic side-effects, without diminishing the ocular hypotensive effectiveness.

Beta-blockers are a well-known prophylactic treatment for migraine; however, treatment by the ocular route has not been widely considered. This case illustrates the resolution of a visual field defect associated with migraine and improvement of symptoms possibly due to administration of a topical beta-blocker. We screened patients with episcleritis or scleritis to detect syphilis by using either fluorescent treponemal antibody absorption testing or microhemagglutination assay for antibodies to Treponema pallidum. Four patients, two with nodular episcleritis and two with scleritis, had ocular involvement as the initial manifestation of late syphilis. Topical or oral corticosteroid therapy provided minimal initial clinical benefit, but ocular inflammation resolved within one to three weeks after administration of systemic penicillin.

In each of the experimental conditions studied, fluorometholone penetrated into the cornea and aqueous humor following topical administration of a standard drop. The amount of drug measured in each location was less than that previously documented for dexamethasone and prednisolone preparations. In contrast to these more conventional steroids, the ocular penetration of fluorometholone appeared to be unaffected by the presence or absence of the corneal epithelium or of intraocular inflammation.
