The arts of war and medicine: a study in symbiosis.

Practitioners of warfare and medicine have both considered their respective disciplines part art and part science. Each has seen dramatic advances over the past several centuries, and the histories of progress in warfare and medical practice are inextricably intertwined. For example, the congregation of large armies inspired the development of epidemiologic analysis and preventive medicine, which forestalled the disastrous loss of life that typified all wars through the U.S. Civil War. Warfare also helped to clarify the critical distinction between medicine and health in several ways. Battles

Heterogeneity and associated factors of palliative care core competency among oncology nurses: a latent profile analysis.

BACKGROUND: Global cancer incidence is expected to rise by 60% by 2040, intensifying the burden experienced by patients and their families. Palliative care improves quality of life in advanced disease and depends on oncology nurses' competencies in ethical decision-making, symptom management, and psychological resilience. Despite moderate overall proficiency, psychosocial shortcomings and heterogeneity in competency remain insufficiently characterized. This study utilized latent profile analysis (LPA), guided by shared decision-making and symptom management theories, to identify competency sub

Sustainable improvement of interprofessional care for better resident outcomes: protocol for the INTERSCALE hybrid type III effectiveness cluster-randomized trial comparing individualized and collaborative delivery of an evidence-based care model for long-term care.

BACKGROUND: Over recent decades, multifaceted nurse-led care models have been developed to reduce unplanned hospital transfers from long-term care facilities (LTCFs). In Switzerland, the INTERCARE model has demonstrated effectiveness, with core components including deployment of nurses in expanded roles (INTERCARE nurses), evidence-based communication tools, and advance care planning. However, resource-intensive implementation strategies such as 1:1 support meetings for model implementers pose challenges for scale-up, underscoring the need for more scalable implementation support. The INTERSCA

Education: The prevention of acute rheumatic fever and rheumatic heart disease in Malawi.

BACKGROUND: With an estimated prevalence of 183,200 cases, rheumatic heart disease (RHD) is a major public health problem in Malawi. However, patients in Malawi with advanced RHD are left with substantial and life-threatening disability because there are no surgical options available in our country at present. In order to tackle this epidemic, it is critical to provide appropriate education and attempt to diagnose the disease earlier. In this study, we aimed to pilot a RHD education program that could be subsequently adopted country-wide. METHODS: We designed and piloted a RHD educational pro

The Sky's the Limit: Expanding Nursing's Contribution to Acute Stroke Science.

Stroke is the number one cause of preventable disability in adults in the United States. Significant advances have occurred in medications and technology supporting rapid stroke diagnosis and treatment during the past 30 years, along with blurring of the lines of what traditionally constituted nursing or medical research. Ischemic stroke is a disease of vascular insufficiency that mirrors myocardial infarction more than any other neurologic diagnosis. My primary program of research is focused on exploration of methods to improve intracranial blood flow in patients with hyperacute ischemic stro

Beyond the Technology: Nursing Critically Ill Children on Extracorporeal Organ Support-A Hermeneutic Review of the Literature.

BACKGROUND: Using advanced forms of medical technology such as extracorporeal organ support to take over organ function in the face of critical illness is a manifestation of the technological expertise to support and prolong life. However, clinician focus on the technical aspects of extracorporeal organ support both in research and clinical practice has the tendency to relegate to the background and disregard multiple, non-technical components such as the meanings, interactions, and experiences of nurses, families and other healthcare professionals. AIM: To understand how experiences and soci

The lived experience of Dupuytren's disease of the hand.

AIMS: To describe patients' experiences of living with Dupuytren's disease. BACKGROUND: Dupuytren's disease is a chronic, progressive deformity of the hand which limits active extension of the fingers due to advancing and irreversible flexion deformity. It is estimated that two million people are affected by the condition in the UK. Nurses may frequently encounter patients with this condition in a wide range of settings. However, the disease is neglected in the nursing literature and little is known about patients' experience of living with the condition. DESIGN: A phenomenological approach,

Clinicians' Perceptions About Institutional Factors in Moral Distress Related to Potentially Nonbeneficial Treatments.

IMPORTANCE: Clinicians commonly experience moral distress related to potentially nonbeneficial life-sustaining treatments (LST). Hospitals' institutional culture (eg, shared beliefs, values, and practices), structures (eg, policies, practices, resource allocation), and societal-level factors (eg, national culture, local and national policies, medical hierarchies) may contribute to moral distress related to potentially nonbeneficial LST. OBJECTIVE: To investigate clinicians' perspectives on how hospitals' institutional culture and structures might exacerbate, prevent, or mitigate the influence

Barriers and facilitators to integrated mental health and HIV care: administrator and provider perspectives.

Since the 1980s when the HIV epidemic began, HIV and substance use have been intricately linked. A promising area for improving access to care and treatment for people living and aging with HIV and substance use is through integrated care (i.e., "the systematic coordination of general and behavioral healthcare"). There is a recognized need to increase collaboration and integration of mental health, substance use, and primary care providers, including infectious disease specialties. We conducted in-depth interviews with 15 administrators (e.g., clinic managers and care coordinators) and 21 prov
