The present article critically analyzes the current approach to treatment outcome research and the uncritical use of treatment packages. Comparing various combinations of treatments with groups of clients randomly assigned to these conditions has led to the neglect of individual differences between persons falling into broad diagnostic categories. The inconclusive results of such studies say little about the efficacy of a particular treatment for an individual client and are also not very useful in helping clinicians to decide which specific treatment to use for a given client. A phobia is an excessive fear or dread of a situation or object which is out of keeping with the danger it may actually present. Phobias can be classified into 'simple phobias', 'social phobias' and agoraphobia. A number of psychological theories have been put forward to account for both the onset and maintenance of a phobia. The case of a social phobia is presented together with a cognitive behavioural intervention package.

The relationship between personality disorders and panic disorder in adults is analyzed in this study. The patients included in this work presented anxiety disorder with agoraphobia and were compared with a group of normal controls. The percentage of patients with personality disorder was 46% significantly higher than those encountered in the control group (16%). The existence of these disorders do not seem to modify the severity of panic disorder.

Of 20 patients attending a clinic for maintenance therapy of schizophrenia, seven had regular panic attacks, and these were often associated with agoraphobia and social phobia. Similar fears and avoidance in other cases were associated with paranoid ideas and negative symptoms. The relationship of panic to psychotic symptoms varied greatly. In two patients neuroleptics were associated with an increase in panic attacks.

The reliability of psychiatric diagnosis using the Schedule of Affective Disorders and Schizophrenia-Lifetime Version in personal and telephone interviews with 39 subjects was assessed using a 12- to 19-month test-retest design. Interrater reliability was high (kappa, .69 to .84) for the diagnosis of panic disorder, agoraphobia with panic attacks, probable panic disorder, major depression, and alcohol abuse. We conclude that it is possible to reliably make these lifetime diagnoses in a family study using the telephone interview.

Two experiments incorporating methodological refinements were conducted to replicate previous findings that agoraphobics selectively recall phobic material compared with normal subjects. In addition, patients with anxiety states but no phobic symptomatology were studied to assess whether selective recall is specific to agoraphobia or a feature of anxious patients generally. The results of both studies failed to replicate previous findings and fail to offer support for recent theorizing regarding the presence of cognitive differences between agoraphobics and normals.

The association between agoraphobia and alcohol dependence has been frequently observed in Western literature, but reports from India are lacking. Two cases demonstrating such an association are described. In both of them, the agoraphobic symptoms followed alcohol dependence. The interaction between these two conditions may set up a vicious cycle, and failure to recognize this co-morbidity may result in treatment failure.

The relationship between isolated sleep paralysis and panic disorder in adults is analyzed in this study. The patients included in this work presented anxiety disorder with agoraphobia and were compared with a group of normal controls. The percentage of patients with sleep paralysis was 40% significantly higher than those encountered in the control group (20%). The existence of this syndrome do not seem to modify the clinical manifestation or severity of panic disorder.

Not all people who become housebound suffer from agoraphobia. Other emotional illnesses can underlie this behaviour. Early diagnosis is important. Some conditions can be treated by family physicians without special training, and some are appropriately treated only in special settings. This paper describes psychiatric disorders that may present as a "housebound syndrome" and outlines the principles of treatment for each.

Using a standardized behavioural programme, nine in-patients with severe agoraphobia were treated by staff who had minimal training in the theory and practice of behaviour therapy. With one exception patients showed marked reduction in subjective fear and avoidance, both following treatment and at follow-up. The indications for such treatment are considered and compared with those for alternative behavioural approaches that have recently been advocated.
