Speedy orthodontics: a case report.

Management of severe rotation poses a great challenge for the orthodontist, especially when cortical anchorage occurs. The conventional methods require excessive treatment time, rely on patient compliance, and can cause root resorption. A groundbreaking new procedure developed by Wilcko et al, which is actually a modification of the conventional corticotomy procedure, amalgamates the orthodontic mechanics, alveolar decortications, and augmentation procedure to make treatment time three to four times faster than conventional orthodontic techniques. This procedure uses the dynamics of bone physi

Modern therapy for severe alveolar ridge atrophy.

This clinical report illustrates a modern method for oral rehabilitation of severe edentulous atrophic ridge. Shallow vestibule and small denture-bearing area of the maxilla provide insufficient retention, and only the symphyseal region of the mandible has obvious alveolar ridge. The surgery of deepening vestibule of the maxilla ridge was done by using tentative miniscrew combined with old denture and soft liner. The osseointegrated dental implants were installed over symphyseal region of the mandible to act as retentive devices for prostheses. In addition, functional reline with rebase materi

Rehabilitation of maxillary anterior esthetics by alveolar distraction osteogenesis with immediate implant placement: a case report.

Esthetic reconstruction of severe soft and hard tissue deficiencies is the utmost challenge in implant dentistry. To prevent postoperative bone resorption and to control the amount of hard and soft tissue volume, distraction osteogenesis followed by immediate implant placement has been proven to be a promising combined technique. The aim of this report is to introduce a treatment strategy to maintain anterior esthetics and to control hard and soft tissue volume in a case, treated with the combined technique of vertical alveolar distraction and immediate implant placement. An osteotomy was perf

Vertical and horizontal bone augmentation with the intraoral autogenous J-graft.

This article describes a technique to predictably reconstruct both horizontal and vertical bony defects with one graft, the autogenous J-graft. Bone harvested from the ramus is contoured in three dimensions resulting in both a horizontal and a vertical component. When there is inadequate soft tissue due to the increase in bone volume at time of augmentation, the bone graft is combined with a pedicle palatal connective tissue graft. This provides both a source of blood supply and soft tissue to the grafted site. There are several advantages to the J-graft technique; a single procedure can recon

Histologic and ultrastructural analysis of regenerated bone in maxillary sinus augmentation using a porcine bone-derived biomaterial.

BACKGROUND: The purpose of the present study was the histologic and ultrastructural evaluation of a biomaterial composed of cortical pig bone in the form of granules. METHODS: After maxillary sinus augmentation using this biomaterial, 10 specimens were retrieved after 5 months in 10 patients using this biomaterial. The specimens were processed to be observed under light microscopy (LM) and transmission electron microscopy (TEM). Histomorphometric measurements were presented by means +/- standard deviations. RESULTS: LM showed that most of the particles were surrounded by newly formed bone. I

[Alveolar ridge atrophy. The free vascularized fibular flap as alternative treatment in severe alveolar atrophy].

Implant-supported overdentures are the goal of treatment of the edentulous atrophic jaw. To gain adequate height there is in some instances need for additional bone augmentation. Ridge augmentation using a free vascularised fibular flap is one of the newer techniques. Prefabrication of fibular flaps, as described in this article, is a two-stage procedure to overcome the problems of osseointegration and missing attached gingiva. Accurate pre-operative 3-D-model planning, implant placement and vestibuloplasty in the first stage allow for occlusion driven reconstruction and immediate function in

A 5-year randomized controlled clinical trial comparing 4-mm ultrashort to longer implants placed in regenerated bone in the posterior atrophic jaw.

Источник пометил публикацию как отозванную

BACKGROUND: Short implants (up to 5-mm long) have shown good results when compared to longer implants placed in augmented bone. PURPOSE: To evaluate if 4-mm ultrashort implants could also be an alternative to bone augmentation in the severely atrophic posterior jaws. The primary aim of the study was to compare implant survival rates between study groups. MATERIALS AND METHODS: Eighty partially edentulous patients with posterior atrophic jaws (5-6 mm of bone above the mandibular canal and 4-5 mm below the maxillary sinus) were included: 40 patients in the maxilla and 40 in mandible. The patie

Histologic Evaluation of Wound Healing After Ridge Preservation With Cortical, Cancellous, and Combined Cortico-Cancellous Freeze-Dried Bone Allograft: A Randomized Controlled Clinical Trial.

BACKGROUND: Cortical and cancellous mineralized freeze-dried bone allografts (FDBA) are available for use in alveolar ridge preservation after tooth extraction. There are currently no data regarding use of a combination 50%/50% cortico-cancellous FDBA compared with a 100% cortical or 100% cancellous FDBA in ridge preservation. The primary objective of this study is to dimensionally and histologically evaluate healing after ridge preservation in non-molar sites using 50%/50% cortico-cancellous FDBA versus 100% cortical and 100% cancellous FDBA. METHODS: Sixty-six patients requiring extraction

[Ridge preservation with synthetic nanocrystalline hydroxyapatite reduces the severity of gingival invaginations-a prospective clinical study].

BACKGROUND AND OBJECTIVE: Gingival invaginations develop after tooth extraction and subsequent orthodontic space closure. Aetiological factors and long-term effects of gingival invaginations on oral health are nearly unknown. In addition, preventive or therapeutic strategies are rare. This prospective clinical study employing the split mouth technique was performed to investigate the effect of extraction socket augmentation with a synthetic nanocrystalline hydroxyapatite (NanoBone(®) Artoss, Rostock, Germany) on the incidence and degree of gingival invaginations. MATERIAL AND METHODS: A total

Autogenous bone grafting in a patient on long-term oral bisphosphonate therapy: case report.

A 66-year-old patient was referred to the Periodontal Clinic at Case Western Reserve University for implant placement in the mandibular left first molar area. The patient reported a history of oral bisphosphonate intake for the last 7 years for the treatment of osteoporosis. Autogenous bone block grafting was planned to augment the ridge before implant placement. The surgery was performed under local anesthesia, and the implant was successfully placed 8 months after ridge augmentation. Healing was uneventful postoperatively, and the buccolingual width of the ridge increased significantly, allo

Implant survival rates after maxillary sinus augmentation.

Implant therapy in the atrophic posterior maxilla becomes challenging in the presence of reduced maxillary bone height. Sinus augmentation can be performed for resolving this condition prior to implant placement. The aim of this article was therefore to evaluate implant survival rates in the grafted sinus taking into account the influence of the implant surface, graft material, and implant placement timing. A systematic review of the literature was performed. Articles retrieved from electronic databases were screened using specific inclusion criteria, and data extracted were divided according

Late secondary autogenous bone grafting in cleft patients comparing mandibular (ectomesenchymal) and iliac crest (mesenchymal) grafts.

A comparative study has been performed between two groups of 25 alveolar cleft patients. The cleft area was restored with mesenchymal (iliac crest) and ectomesenchymal bone (mandibular symphysis). The results are discussed; the mandibular symphysis is a reliable donor site; the ectomesenchymal graft is better incorporated, significantly less resorbed, and the orthodontic treatment postoperatively is not altered.

Oral rehabilitation of adult twins with severe lack of bone due to hypohidrotic ectodermal dysplasia--a 12-month follow-up.

PURPOSE: Patients with x-linked hypohidrotic ectodermal dysplasia (XLHED, OMIM 305100) often present with exceptional oral conditions, because alveolar ridges can remain severely hypotrophic due to oligo- or anodontia in the primary and/or permanent dentition. This can seriously disable a young person physically and psychologically, and lead to sociopsychological isolation. PATIENTS AND METHODS: The oral rehabilitation of fraternal male twins with XLHED, one with severe hypodontia and the other with anodontia is described. The treatment of both patients comprised augmentation of the jaws usin
