Intra-abdominal pressure, vertebral column length, and spread of spinal anesthesia in parturients undergoing cesarean section: An observational study.

BACKGROUND: In parturients with increased physiologically intra-abdominal pressure (IAP) and a short stature, a greater cephalad spread of spinal anesthesia is often observed after a fixed amount of plain bupivacaine is administered. Therefore, we designed this prospective study to test whether IAP and vertebral column length (VCL) were predictors of spinal spread in parturients undergoing a cesarean section. METHODS: A total of 113 parturients, all undergoing elective cesarean sections with single-shot spinal anesthesia, were enrolled. The L3-L4 interspace was entered, and 2 mL of 0.5% plain

The hemodynamic and pain impact of peripheral nerve block versus spinal anesthesia in diabetic patients undergoing diabetic foot surgery.

PURPOSE: Comparison of hemodynamic profiles and pain scores in diabetic patients undergoing diabetic foot surgery receiving peripheral nerve block (PNB) or spinal anesthesia [subarachnoid block (SAB)]. METHODS: This was a prospective, randomised controlled trial. We recruited diabetic patients aged > 18 years, American Society of Anesthesiologists class II-III, who were scheduled for unilateral diabetic foot surgery below the knee. All patients were assessed for autonomic dysfunction using the Survey of Autonomic Symptoms score. Participants were randomly assigned to receive either PNB or SAB

Norepinephrine Intermittent Intravenous Boluses to Prevent Hypotension During Spinal Anesthesia for Cesarean Delivery: A Sequential Allocation Dose-Finding Study.

BACKGROUND: The use of phenylephrine as the first-line agent for prevention and treatment of maternal hypotension during cesarean delivery (CD) may reduce cardiac output, posing a theoretical risk to mother and fetus. Norepinephrine has been suggested as a potential alternative, because its β-adrenergic effects might result in greater heart rate and cardiac output than phenylephrine. The use of norepinephrine to prevent and treat hypotension during CD is new, and its use as a bolus has not been fully determined in this context. The purpose of this study was to determine the effective norepinep

Paraplegia caused by giant intradural herniation of a lumbar disk after combined spinal-epidural anesthesia in total hip arthroplasty.

Total paraplegia after epidural or spinal anesthesia is extremely rare. We herein report a case of total paraplegia caused by a giant intradural herniation of a lumbar disk at the L3-L4 level after total hip arthroplasty for coxarthrosis. The patient had no preoperative neurologic abnormalities. Intraoperative anesthetic management involved combined spinal-epidural anesthesia at the L3-L4 level with continuous intravenous propofol administration. Postoperatively, the patient complained of numbness and total paraplegia of the lower extremities. Magnetic resonance imaging showed a giant herniati

[A Case of Postoperative Paraplegia Caused by Idiopathic Spinal Cord Infarction following Hepatectomy under Both General and Epidural Anesthesia].

A 73-year-old woman (height : 155 cm, weight : 55 kg) was scheduled to undergo a laparotomic hepatectomy and radiofrequency ablation for hepatocellular carcinoma. Her medical history did not include any relevant conditions such as cardiovascular or neurological disorders. A thoracic epidural catheter was introduced at T8-9 before the induction of anesthesia with intravenous propofol. General anesthesia was maintained with the inhalation of oxygen, air, and desflurane, and the continuous infusion of remifentanil. Several intraoperative episodes of mild hypotension occurred, each of which was su

Intraoperative neurological event during cesarean section under spinal anesthesia with fentanyl and bupivacaine: Case report and review of literature.

Neurological events similar to transient ischemic attack in a peripartum woman are uncommon. Cerebral complications of preeclampsia, thrombo-embolic phenomena, or high spinal can mimic such situations. Spinal anesthesia with local anesthetic and opioid is an established anesthetic technique for cesarean section. Although intrathecal opioids are safe for both the mother and fetus; some unusual complications such as dysphagia alone or associated with facial numbness, aphasia, have been reported. We report a case of transient aphonia and tingling sensation over the face without any dysphagia afte

Prospective study of hypotension after spinal anesthesia for cesarean section at Siriraj Hospital: incidence and risk factors, Part 2.

BACKGROUND: The incidence of hypotension after spinal anesthesia is highest in cesarean section. The authors' first retrospective study identified three risk factors that included two non-modifiable (patient's height and low baseline systolic blood pressure) and one modifiable risk factor (sensory analgesia equal to or higher than TS) associated with hypotension. OBJECTIVE: To create a prospective record of the event in the patients who received successful spinal anesthesia for cesarean section. MATERIAL AND METHOD: A prospective data collection, together with questionnaires that were comple

Spinal anesthesia-induced hypotension: a risk comparison between patients with severe preeclampsia and healthy women undergoing preterm cesarean delivery.

We previously showed that, in comparison with term healthy parturients, patients with severe preeclampsia had a less frequent incidence of spinal hypotension, which was less severe and required less ephedrine. In the present study, we hypothesized that these findings were attributable to preeclampsia-associated factors rather than to a smaller uterine mass. The incidence and severity of hypotension were compared between severe preeclamptics (n = 65) and parturients with preterm pregnancies (n = 71), undergoing spinal anesthesia for cesarean delivery (0.5% bupivacaine, sufentanil, morphine). Construction and evaluation of a PONV prediction model in patients undergoing orthopedic surgery under spinal anesthesia: a prospective cohort study.

BACKGROUND: Postoperative nausea and vomiting (PONV) is a common complication following anesthesia. Most existing studies have focused on patients undergoing non-orthopedic surgeries with general anesthesia, neglecting the occurrence of PONV in patients receiving spinal anesthesia. This study aims to investigate the potential risk factors for PONV in patients undergoing orthopedic surgery with spinal anesthesia through a prospective cohort study and to construct and evaluate a clinical prediction model. METHODS: Relevant factors associated with PONV in patients undergoing elective orthopedic s

Case Report: Opioid-sparing general anesthesia with bilateral quadratus lumborum block for cesarean delivery in a parturient with spinal deformity.

For parturients with contraindications to neuraxial anesthesia, GA remains the primary alternative. However, conventional opioid-based GA may increase the risk of adverse neonatal outcomes. Therefore, exploring opioid-sparing GA combined with bilateral QLB is of considerable clinical value for reducing potential maternal and neonatal risks. In the present case, the patient was a 35-year-old full-term parturient with spinal deformity who declined neuraxial anesthesia, constituting a contraindication to such techniques.During cesarean delivery, we successfully implemented an opioid-sparing GA reg

Anesthetic Considerations for and Management of a Parturient With Jarcho-Levin Syndrome: A Case Report of Continuous Spinal Anesthesia for Preterm Cesarean Section.

Jarcho-Levin syndrome (JLS) is a rare congenital disorder marked by multiple vertebral abnormalities, a shortened spine, and rib fusion at the costovertebral junction. In addition to the typical physiological changes associated with pregnancy, patients with JLS face complications affecting their lungs, heart, spine, and mobility. Difficult airway management, pulmonary disease, and spinal deformities observed in these patients require a collaborative, multidisciplinary approach and meticulous perioperative planning. The presence of a challenging airway complicates the choice between general and

Prophylactic infusion of norepinephrine does not affect the rostral spread of spinal anesthesia in pregnancy: a prospective, randomized, double-blinded study.

Background: The infusion of phenylephrine to prevent spinal-induced hypotension (SIH) in cesarean delivery may decrease the rostral spread of a spinal local anesthetic. We hypothesized that infusion of norepinephrine may decrease the rostral spread of spinal anesthesia, similar to that caused by phenylephrine. The aim of this study was to compare the block height of spinal anesthesia in the presence or absence of norepinephrine infusion administered to prevent SIH during cesarean delivery. Methods: Eighty patients were enrolled and allocated into groups receiving a norepinephrine infusion (gro
