BACKGROUND: The ideal intraocular lens in cases of aphakia without capsular support is debated. Choices include anterior chamber lenses, iris- or scleral-sutured lenses, and iris-claw lenses. We describe 2 cases of Brown-McLean syndrome with corneal edema successfully treated by anterior stromal puncture (ASP). Ophthalmologic examinations showed peripheral edema and aphakia in both patients. The patients had ASP to control the severe foreign-body sensation. The procedure was effective over a long-term follow-up, suggesting that peripheral ASP may be an effective therapeutic option for patients with symptomatic Brown-McLean syndrome.

Twenty-six of 75 eyes undergoing pars plana vitrectomy developed significant corneal complications requiring treatment. Slow or nonhealing epithelial abrasions, recurrent corneal erosion, and microcystoid and striate keratitis were among the most frequent problems. Significant predisposing factors included diabetes, surgical trauma such as epithelial debridement and prolonged operative time, aphakia, and postoperative glaucoma or hyphema. Corneal complications may be minimized with careful preoperative and operative precautions.

Intracorneal haemorrhage is a rare complication of modern contact lens wear. While a limited number of reports have described intracorneal haemorrhages (typically stromal) associated with the extended wear of soft contact lenses with limited oxygen permeability for the correction of aphakia, this case report documents the management and resolution of a significant intraepithelial corneal haemorrhage associated with long-term rigid contact lens wear for keratoconus and high myopia.

Ocular malformations associated with agnathia in a 34-week gestational age infant were studied histopathologically and included asymmetric microphthalmos. Aphakia and retinal dysplasia were noted in the most severely affected globe; the fellow eye was characterized by microcornea, anterior segment dysgenesis, uveal colobomas, and retinal dysplasia. Aplasia of the optic nerve was seen bilaterally. The combination of agnathia and ocular malformation in the absence of holoprosencephaly is challenging to explain embryologically.

A photographic demonstration of the focal nature of the retinal illumination from the operating microscope is presented. It illustrates that under normal anterior segment operating procedures, the side-mounted and coaxial illuminators are imaged as small, intense spots on the retina. The irradiance (in watts per square centimeter) of these spots is reduced by their defocusing in aphakia. Cautionary attention is drawn to the danger of allowing these retinal hot spots to be imaged on or near the macula during surgery.
