Apnea and factitious illness (Munchausen syndrome) by proxy.

OBJECTIVE: Munchausen syndrome by proxy (MSP) is recognized in the differential diagnosis of apparent life-threatening events, but the early signs and the full spectrum of this presentation are not well recognized. We aim to describe MSP presenting with apnea to illustrate this spectrum and the evolution in our management over a period of 10 years. PATIENTS AND RESULTS: Eleven children in five families seen in one institution and assessed by one team are described in detail. The children had apnea and/or pallor, but with a wider age range than usually seen with apparent life-threatening event…

[Obstructive apnea and periodic respiration in premature infants].

BACKGROUND: Disturbances of cardiorespiratory function are common clinical problems in preterm infants. Polygraphic recordings during sleep were performed in order to determine frequency and severity of idiopathic apnea of prematurity. METHOD: In 137 preterm infants at the corrected age of < +/- 0 weeks, 0-12 weeks, 13-24 weeks and > or = 25 weeks respectively central and obstructive apnea and periodic breathing were recorded by polysomnography including nasal airflow and thoracic and abdominal breathing movements and compared with 50 normal full-term infants. RESULTS: Apnea frequency was in…

Postoperative risk following uvulopalatopharyngoplasty for obstructive sleep apnea.

A study was conducted to assess oxygenation and respiratory changes on the first and second postoperative nights after uvulopalatopharyngoplasty (UPPP) for obstructive sleep apnea (OSA). Twelve patients were postoperatively evaluated with 8-hour nocturnal polysomnography on four occasions: (1) PREOP--night before UPPP, (2) POPN1--first postoperative night, (3) POPN2--second postoperative night, and (4) 3MOS--3-month follow-up study. Results demonstrate that apnea index (AI) and respiratory disturbance index (RDI) were significantly improved at 3MOS from PREOP levels: AI (p less than 0.01) and …

Uvulopalatopharyngoplasty for obstructive sleep apnea: a community's experience.

Uvulopalatopharyngoplasty (UPPP) has become an accepted method for treating obstructive sleep apnea (OSA), with a reported success rate as high as 77%, depending upon inclusionary and outcome criteria. The authors reviewed the records of 90 patients with moderately severe OSA (apnea plus hypopnea index [AHI] greater than 20) who underwent UPPP at either a private community or an academic hospital. Forty percent of patients experienced more than a 50% reduction in their AHI with UPPP. Only 22 (24%) of the patients had a postoperative AHI less than 50% of the preoperative AHI and less than 20, i…

Cardiovascular changes associated with obstructive sleep apnea syndrome.

Five men free of lung or cardiovascular diseases and with severe obstructive sleep apnea participated in a study on the impact of sleep states on cardiovascular variables during sleep apneas. A total of 128 obstructive apneas [72 from stage 2 non-rapid-eye-movement (NREM) sleep and 56 from rapid-eye-movement (REM) sleep] were analyzed. Each apnea was comprised of an obstructive period (OP) followed by a hyperventilation period, which was normally associated with an arousal. Heart rate (HR), stroke volume (SV), cardiac output (CO) (determined with an electrical impedance system), radial artery …

Multichannel polysomnographic recording for evaluation of infant apnea.

The preceding discussion has consisted of a review of the technical and clinical aspects of pediatric multichannel recordings, which have become a widely used procedure in the clinical evaluation of infants with various apnea syndromes. It has been shown that multichannel recordings are superior to two-channel pneumocardiograms. Multichannel recordings should therefore be considered in all infants with unexplained episodes of apnea, bradycardia or cyanosis, in order to clarify the type of apnea and to rule out underlying conditions such as gastroesophageal reflux or seizures. The role of multi…

[Late postoperative apnea in a premature newborn infant].

We report the case of a premature newborn child (36 weeks) who was operated on a teratoma of the sacrum when he was 12 days old and weighed 2,950 g. The patient presented a late postoperative apnea 17 hours after anesthesia. The anesthetic technique consisted of lumbar epidural blockade with 0.33% bupivacaine at a dose of 2.25 ml and superficial inhalation anesthesia with 0.5% isoflurane. Relaxing muscular agents used in this case were succinylcholine (3 mg) for orotracheal intubation and pancuronium bromide (0.3 mg) for maintaining the anesthetic level. The immediate postoperative phase was u…

Issues in selecting and using apnea documentation systems.

The primary purpose of apnea monitoring is to ensure adequate warning of certain life-threatening respiratory and cardiac events, often in infants monitored at home. However, parents often become frustrated by frequent alarms, and other may not use the monitor at all. Unfortunately, ECRI has received a number of incident reports in which infants have died while not connected to their monitors or where a failure to monitor was suspected. We believe that some of these deaths may have been prevented if monitoring had been used properly. The most important step that parents can take to achieve eff…

Surgical alternatives to uvulopalatopharyngoplasty in sleep apnea syndrome.

Uvulopalatopharyngoplasty (UPPP) is the surgery most often performed for sleep apnea syndrome (SAS). However, good results with UPPP, demonstrated by polysomnography, have been reported in only 50% of cases. Failure of UPPP may be caused by: 1) bad management of the SAS, which is better treated in some patients with nasal CPAP than with surgery; and 2) an airway obstruction located not only at the palatopharynx (PP) level. Other surgical procedures to enlarge other sites of obstruction are described. Retro-tongue-base-pharynx (RTBP) surgery is emphasized, including mandibular advancement, hyoi…

[Effects of uvulopalatopharyngoplasty in obstructive sleep apnea syndrome].

Thirty-one patients with obstructive sleep apnea syndrome underwent uvulopalatopharyngoplasty (UPPP). After three months, the average apnea/hypopnea index (AHI) decreased from 352.2 +/- 138.2 (mean +/- SD) to 182.2 +/- 140.4 episodes/hr and the duration of oxygen desaturation (less than 85%) decreased from 38.4 +/- 48.6 to 13.2 +/- 25.8 minutes. Although only 51.6% patients had a reduction of AHI greater than 50%, a questionnaire study revealed that most patients had subjective improvement of clinical symptoms and only mild complications were noted. We tried to find out the correlation between…

[Obstructive sleep apnea treated with uvulo-palato-pharyngoplasty].

Uvulo-palato-pharyngo-plasty (UPPP) was introduced as a method of treatment of obstructive sleep apnoea in 1980. The pathogenesis of obstructive sleep apnoea is described. The operative technique is also described and illustrative case histories are quoted. The effect obtained varies and, for this reason, the role of UPPP in the treatment of obstructive sleep apnoea cannot be finally established.

Cephalometric analysis of the obstructive sleep apnea syndrome.

The cephalometric measurements of 33 Finnish OSAS patients showed a fairly normal relationship between the base of the skull and the maxilla (SNA 82.3), whereas the mandible was slightly smaller than average (SNB 78.0). In 58% of the patients the mandible was significantly retrognathic and/or small in relation to the maxilla, even if dental occlusion was satisfactory. The sagittal lengths of the anterior cranial base and the maxilla were fairly normal. The hyoid bone was constantly situated more inferiorly and posteriorly than on average (MP-H 32.6). UPPP, genioplasty and advancement of the fl

Anemia and postoperative apnea in former preterm infants.

To examine the association between anemia and postoperative apnea in former preterm infants, 24 former preterm infants of less than 60 weeks postconceptual age undergoing inguinal hernia repair were studied. A hematocrit of at least 25% was required for study participation. General endotracheal inhalational anesthesia, supplemented with neuromuscular blockade and controlled ventilation, was used. No barbiturates or opioids were administered. Respiratory pattern and heart rate were recorded for at least 12 h postoperatively using an impedance pneumograph. Tracings were analyzed for evidence of …

[Role of sleep apnea in essential arterial hypertension].

Because sleep apnoea syndrome is often associated with arterial hypertension, it has been suggested that sleep apnoea might be responsible for hypertension. This hypothesis is mainly based on epidemiological studies showing a statistically significant association between snoring and arterial hypertension; this association remains true even after data correction to take into account the increased frequency of snoring with age and overweight. However, this statistical link is no evidence of a cause-effect relationship, and the mechanism through which sleep apnoea syndrome could produce arterial

[Apnea and oxygen desaturation following nitrous oxide inhalation].

Apnea and desaturation following nitrous oxide inhalation were studied in seven adult volunteers breathing spontaneously. Arterial oxygen saturation (SpO2), end-tidal CO2 concentration in the nasal cavity and respiratory patterns were measured in volunteers breathing air after N2O (50% or 67%) + O2. SpO2 was measured with Biox 3700 and end-tidal CO2 concentration was measured with Normocap, and respiratory patterns were recorded with RESPIGRAPH. After breathing N2O, two volunteers had frequent apnea (greater than 20 sec) accompanied by desaturation (SpO2 less than 90%). The lowest value of SpO…

[Personality markers in patients with sleep apnea syndrome].

In addition to the well-known medically oriented examination procedures and forms of treatment of sleep apnea, we are also interested in the psychological aspects of this pathological condition. Against the background of our experience of patients treated with nCPAP--here we have observed, among other things, an increase in general activity--we investigated the question as to whether typical personality traits are to be observed among persons suffering from sleep apnoea. As a psychodiagnostic procedure, the Freiburg personality inventory (FPI-R) was employed in the following persons: a) Person…

Obstructive sleep apnea syndrome in sickle cell disease.

We present a series of 21 children with obstructive sleep apnea syndrome and sickle cell disease (SCD). Thirteen patients underwent adenotonsillectomy with resolution of symptoms and improvement in alveolar hypoventilation. Adenotonsillectomy involves significantly increased risks in patients with SCD. We present our protocol for management of these difficult surgical patients, including vigorous hydration and transfusion therapy. Polysomnography has proven valuable both in diagnosis and in identifying severely affected patients. Increased end tidal alveolar carbon dioxide during sleep has pro…

[Transtracheal oxygen therapy in obstructive sleep apnea syndrome].

In a prospective trial we studied the effect of nocturnal low-flow oxygen administration in 10 consecutive patients with obstructive sleep apnea. In 5 patients the clinical symptoms disappeared completely under nasal oxygen administration and hypoxic episodes decreased from an average of 28 to 3 per hour. In the remaining 5 patients nasal oxygen alone was not sufficiently effective. After insertion of a transtracheal catheter for nocturnal oxygen delivery, however, in 4 of these 5 patients (one patient refused the procedure) clinical symptoms improved and hypoxic episodes were reduced to 3 per…

Sleep apnea syndrome after irradiation of the neck.

After irradiation of the neck for a squamous cell carcinoma of the tonsillar pillar and vocal cord, a 71-year-old man presented with a rapidly progressive sleep apnea syndrome. Previous reports describe the condition of patients with obstructive sleep apnea that developed after neck irradiation and secondary to supraglottic edema. Our patient had an obstructive component to his apnea similar to that described in previous cases, but, in addition, he had hypothyroidism. Myxedema is a well-described cause of both obstructive and central apnea. We believe both contributed to his condition. He was …

Automobile accidents involving patients with obstructive sleep apnea.

Although patients with obstructive sleep apnea often report falling asleep while driving, the frequency of auto accidents involving these patients has not been rigorously studied. Therefore, we compared the driving records of 29 patients with obstructive sleep apnea with those of 35 subjects without sleep apnea. The patients with sleep apnea had a sevenfold greater rate of automobile accidents than did the subjects without apnea (p less than 0.01). The percentage of persons with one or more accidents was also greater in the patients with apnea than in the control subjects without apnea (31% ve…
