INTRODUCTION: Endometriosis is a common condition affecting 5 to 10% of women of childbearing age. The true incidence of endometriosis of the appendix is currently unknown. Since symptoms often overlap with those of acute appendicitis, endometriosis of the appendix presents a diagnostic challenge in the emergency department. This large retrospective study investigates the incidence and perioperative clinical, radiologic, and laboratory findings, as well as possible differences between patients with and without endometriosis.
METHODS: Data from consecutive patients who underwent appendectomy f…

We report a case of postoperative urinary leakage after bilateral laparoscopic totally extraperitoneal (TEP) herniorrhaphy. A man in his upper 80s with a healed cystostomy and appendectomy underwent bilateral TEP herniorrhaphy. Urinary leakage was noted by ultrasound examination 4 days after bilateral TEP. Cystography and computed tomography conclusively confirmed a 6-mm extraperitoneal fistula at the site of the previous cystostomy. The fistula involved the anterior bladder wall and was associated with an extended urinoma. The patient was treated by indwelling catheterization using a Foley ca…

Cecal appendix tumors are extremely rare (less than 0.5%), with carcinoid tumors being described as the most common type of appendix tumor. A case of a 32-year-old male patient is reported, with no significant pathological history, with acute abdomen data. Laparoscopic appendectomy was decided, presenting findings of an appendicular inflammatory process, resulting in a neuroendocrine tumor histopathology report. Despite the fact that appendix neoplasias are rare, appendicitis is a common presentation, hence the importance of conducting histopathology studies on every piece of cecal appendix ob…

Background Globally, appendicitis is the most frequent emergency surgical procedure. Laparoscopic Appendectomy (LA) is recommended as a standard surgical procedure to remove appendix. In Nepal, studies showed improved outcomes of Laparoscopic Appendectomy than Open Appendectomy (OA) in treating acute appendicitis. However, effectiveness of in Complicated Appendicitis (CA) has not yet studied in Nepal. Objective This study aims to assess the temporal trend of Laparoscopic Appendectomy in management of Complicated appendicitis and to compare outcomes with Open Appendectomy. Method The study is a…

BACKGROUND: Intraoperative cultures are commonly sent in complicated appendicitis. Culture-guided antibiotics used to prevent postoperative infectious complications are debated. In this study, we describe the microbial overlap between intraoperative and abscess cultures, and antibiotic resistance patterns.
METHOD: A local register of a children's hospital treating children 0-15 years old with appendicitis between 2006 and 2013 was used to find cases with intraoperative cultures, and cultures from drained or aspirated postoperative intraabdominal abscesses. Culture results, administered antibi

OBJECTIVES: To investigate the association between time to appendectomy and the risk of surgical site infections (SSIs) in children with appendicitis across multiple NSQIP-Pediatrics institutions.
BACKGROUND: Several recently published single institution retrospective studies have reported conflicting relationships between delaying appendectomy and the risk of increasing surgical site infections (SSI) in both children and adults. This study combines data from NSQIP-Pediatrics with institutional data to perform a multi-institutional analysis to examine the effects of delaying appendectomy on s…

BACKGROUND AND AIM: Appendectomy protects against the development of ulcerative colitis (UC). However, the relationship between appendectomy and the clinical course of UC is complex, and could be impacted by a number of variables. The aim of this study was to compare the clinical course of UC between appendectomized patients and nonappendectomized patients in Korea.
METHODS: Data on 2648 UC patients were retrieved from the Inflammatory Bowel Disease registry at Asan Medical Center. This retrospective cohort study compared the clinical course of UC in 68 patients who received an appendectomy b…

PURPOSE: Although surgical site infections (SSI) are a major postoperative complication of appendectomy, few studies have focused on the risk factors for SSI. In this study, we investigated the risk factors for SSI in patients who had undergone appendectomy.
METHODS: Three hundred patients who had undergone open appendectomy were enrolled. The patients were divided into two groups based on the presence or absence of SSI. A statistical analysis was performed to assess the clinical features associated with SSI after appendectomy.
RESULTS: A multivariate analysis using the results of univariate…

OBJECTIVE: Laparoscopic appendectomy is the primary treatment of appendicitis, but multimodal pain regimens are necessary to improve postoperative outcomes and optimize pain control. There are limited data comparing the analgesic requirements between perforated and non-perforated appendicitis and whether perforation requires more opioids due to its complications. The objective of this research is to compare the quantity of opioid administrations in perforated and non-perforated appendicitis following laparoscopic appendectomy to see if there is a difference in analgesic requirements.
METHODS:…

BACKGROUND: Laparoscopic appendectomy (LA) is the conventional treatment for chronic appendicitis. Endoscopic direct-vision appendicitis therapy (EDAT) has emerged as a minimally invasive, organ-preserving alternative.
OBJECTIVES: To compare the efficacy and safety of EDAT versus LA in chronic appendicitis.
DESIGN: Multicenter retrospective cohort study with propensity score matching (PSM).
METHODS: This multicenter retrospective cohort study was conducted in China between January 2019 and February 2024. PSM was performed to minimize selection bias. A total of 186 patients with chronic appe…

BACKGROUND: While laparoscopic appendectomy is the standard of care for acute appendicitis, the clinical utility of prophylactic abdominal drainage in perforated acute appendicitis remains controversial. Previous observational studies often lacked granular perioperative covariate adjustment and control for institutional clustering effects. We aimed to determine the association between prophylactic drainage and intra-abdominal abscess (IAA) formation using a unified mixed-effects framework.
METHODS: The Drainage of Laparoscopic Appendectomy for Perforated Appendicitis (DLAP) study was a retros…

BACKGROUND: Closure of the peritoneum has been a standard practice in appendectomy. However, it has been proposed that closure of the peritoneum following any abdominal surgery is more likely to give rise to adhesions and increased postoperative pain. This study was based on the hypothesis that peritoneal non-closure would result in decreased postoperative pain without causing any significant increase in complications like surgical site infection or wound dehiscence.
OBJECTIVE: The primary objective of this study was to compare the pain scores in patients undergoing open appendectomy and peri…

BACKGROUND: Schistosomiasis is an endemic parasitic disease still prevalent in some regions of China. Chronic infection may cause recurrent inflammation and intestinal mucosal remodeling, increasing the risk of lesions such as adenomas. Although intestinal lesions associated with schistosomiasis have been reported, cases involving both the colon and appendix with egg deposition are rare. With the maturation of natural orifice transluminal endoscopic surgery (NOTES), endoscopic transcecal appendectomy (ETA), a specific NOTES technique, has been increasingly applied in clinical practice. In this…

INTRODUCTION: Small bowel obstruction (SBO) is a common surgical emergency. Our report describes a case of a 61-year-old male who was found to have a PROLENE suture left in situ from a previous open appendectomy 22 years ago, over which a fibrous adhesive band had formed, resulting in a terminal ileal volvulus and subsequent SBO.
CASE: A 61-year-old male presented with a 3-day history of severe lower abdominal cramps, nausea and constipation. A previous open appendectomy, performed 22 years ago, was the only significant detail in his medical history. A CT can with oral contrast was performed …
