The incidence of extrapulmonary tuberculosis (TB) has been rising due to the increasing number of immunosuppressed patients. Musculoskeletal system accounts for 25% of extrapulmonary TB. Most of the musculoskeletal TB involves the spine. The sternoclavicular (SC) joint is an unusual site for septic arthritis in healthy people. It is mostly described as affecting intravenous drug abusers. Septic arthritis of the sternoclavicular joint (SCJ) is an uncommon form of arthritis generally occurring in immuno-compromised patients with contiguous or distant foci of infection and central venous catheters. Septic SCJ arthritis in previously healthy subjects is a very rare occurrence requiring a high index of suspicion for the diagnostic assessment. We report here one patient without predisposing factors presenting Staphylococcus aureus SCJ infection.

Anaerobic bacteria are uncommon pathogens in septic arthritis. We report a case of pyarthrosis of the right hip caused by Fusobacterium nucleatum, following a transient synovitis in an otherwise healthy boy. There are only a few cases involving this species described in the literature. This report illustrates the difficulty of isolating this unusual organism and emphasises the usefulness of the Bactec blood culture bottles for the recovery of anaerobic bacteria.

Blastomycotic arthritis usually presents as acute monoarticular septic arthritis resembling a bacterial process. Only 2 cases in the literature have subsequently developed arthritis in a second joint. We describe the case of an elderly woman with polyarticular blastomycotic arthritis associated with lung, skin, and bone involvement. The organism was identified in the synovial fluid and skin biopsies. Our patient underwent open drainage and aspiration of the involved joints and received antimycotic therapy.

Salmonella paratyphi is an extremely rare cause of primary septic arthritis of the hip, especially in the U.K. The presentation is no different from other causative organisms. A case is described illustrating that uncommon organisms can cause septic arthritis and that for successful treatment correct organism identification is paramount.

The authors studied septic arthritis in patients of the Hospital das Clínicas da Faculdade de Medicina da Universidade de São Paulo. The pathogeny, the etiology and the diagnosis with clinical and laboratory findings are discussed. Our data are in accordance with those found in the literature. The main site was the knee joint and the etiologic agent Staphylococcus aureus. The authors point to the necessity of an early diagnosis and to the fact that the treatment of septic arthritis must be started as soon as possible.

Lyme borreliosis is a relatively new disease, so much remains to be learned about it. In this article, typical manifestations at each stage are reviewed. However, as the authors emphasize, diagnosis is still a challenge because a given patient may have from a few to all of the features discussed, stages often overlap, and characteristics come and go and may mimic other illnesses.

The development of gonococcal arthritis is reported in a man with HIV infection and CDC Stage IVC2 disease. The diagnosis of disseminated Neisseria gonorrhoeae was facilitated by microbiological examination of a joint aspirate.
