This paper describes a universal simulation system which enables simulation models of cardiac rhythm disturbances and of artificial pacemaker actions to be constructed. Three types of heart element are introduced according to the way in which excitation impulses are propagated or produced. Cycle rate dependence of repolarization periods and of impulse transmission speeds is incorporated. Random and predetermined deviations from natural behavior are also allowed for in the model. Selectivity of alinidine for SA nodal automaticity vs. other cardiac activities was investigated using isolated, blood-perfused SA node, AV node and papillary muscle preparations of the dog. A 69-year-old man presented with recurrent palpitations since childhood. Electrophysiology studies performed on two separate occasions revealed the combination of sick sinus syndrome and AV node re-entrant tachycardia. The case is reported because it illustrates marked temporal variability in the electrophysiological properties of the dual AV node pathways, and also deleterious effects of verapamil on sinoatrial node function.

We describe an intracardiac epithelial cyst in association with esophageal atresia. The case is unusual in that the cyst was symptomatic and ultimately fatal. In addition, there was no other cardiac anomaly, although a range of extrathoracic malformations was present. There are three types of intracardiac epithelial cysts: congenital polycystic tumor of the atrioventricular node, a cyst as part of a teratoma, and, as in this case, a gross cyst. We describe a modified method for examination of the atrioventricular and sinoatrial nodes that displays the major elements of the cardiac conducting system, yet is not time-consuming for the technician. We examined 31 cases in three categories: cases with previous cardiac history, sudden deaths from no known cause, and cases where death was sudden and accidental. All cases showed histologic abnormalities in the cardiac tissue. We contend that our method would permit such examination to be routinely used in the pathology laboratory.

Electrophysiologic and antiarrhythmic effects of the new benzofurane derivative carocainide given intravenously have been studied in normal patients (Group I) and in patients with paroxysmal supraventricular tachycardias (Group II).
