Recent research has thrown considerable doubt on the claim that King George III suffered from variegate porphyria, but indicates that he suffered recurrent attacks of mania as part of his bipolar disorder. George III's last episode of ill health occurred during the final decade of his life (1810-20). This has been diagnosed as chronic mania with an element of dementia. During this period the king was blind and possibly deaf, which may have contributed to his psychiatric condition. His blindness was due to bilateral cataracts; serious consideration was given at the time to surgery, but this was…

BACKGROUND: The frequency of jugular venous reflux (JVR) is higher in patients with transient monocular blindness (TMB). We hypothesize that JVR influences ocular venous outflow, and resulting disturbances in cerebral and ocular venous circulation might be a cause of TMB. To substantiate this hypothesis, we aimed to demonstrate that: (1) TMB patients have vasculature changes in their retinal venules, and (2) JVR could influence ocular venous outflow, as revealed by dilated retinal venules.
METHODS: This study has 2 parts. The case-control study included 31 TMB patients and 31 age/gender-match…

BACKGROUND: Eclampsia is rare in molar pregnancy. Sudden cortical blindness in eclampsia is an uncommon but very dramatic experience for the patient. Because of its rarity, blindness associated with eclampsia may pose a significant problem for the obstetrician.
CASE REPORT: We describe cortical blindness and posterior reversible encephalopathy syndrome (PRES) complicating molar pregnancy-related eclampsia. The clinical presentation in our patient was consistent with PRES associated with eclampsia together with magnetic resonance imaging (MRI) findings.
CONCLUSION: Preeclampsia and eclampsia …

PURPOSE: To assess the prognosis of different types of glaucoma in relation to unilateral and bilateral blindness at a University Hospital.
METHODS: Charts of glaucomatous patients which presented complete data of clinical history, visual acuity, visual field, fundus examination and diagnosis were retrospectively analyzed. The patients were classified as: not blind, legally blind (best corrected visual acuity <20/200 and/or visual field <20 masculine), or totally blind (no light perception) in one or both eyes. Patients with blindness due to congenital glaucoma and other no glaucomatous cause…

PURPOSE: This hospital-based study was carried out to assess the common causes of blindness in Ibadan, the largest city in the southwestern part of Nigeria, and to strengthen the primary eye care approach to control and reduction of cases of blindness in the community.
METHODS: Following Hospital Ethical Board approval and informed consent of participants, a prospective study was performed in which 1,544 consecutive patients with bilateral or unilateral blindness seen between January and December 2003 at the Ring Road State Hospital, Ibadan, Nigeria, were evaluated for causes of blindness. Vi…

PURPOSE: To describe the clinical features and genetic analysis of a 3-year-old boy diagnosed with familial fleck retina with night blindness.
METHODS: The proband and his parents and grandparents were included. History, visual acuity and fundus examinations were evaluated. Bright-flash (rod-plus-cone) electroretinograms (ERGs) were recorded after 30 mins and 180 mins of dark adaptation. Mutation screening of the RDH5 gene encoding 11-cis retinol dehydrogenase was performed.
RESULTS: The parents noticed the proband's night blindness when he was 2 years old. Best corrected visual acuity was 1…

BACKGROUND: Blindness and low vision are thought to be common in southern Sudan. However, the magnitude and geographical distribution are largely unknown. We aimed to estimate the prevalence of blindness and low vision, identify the main causes of blindness and low vision, and estimate targets for blindness prevention programs in Mankien payam (district), southern Sudan.
METHODS AND FINDINGS: A cross-sectional survey of the population aged 5 y and above was conducted in May 2005 using a two-stage cluster random sampling with probability proportional to size. The Snellen E chart was used to te…

INTRODUCTION: We report a case of a Guillain-Barre syndrome (GBS) with subarachnoid hemorrhage and regressive occipital white matter lesions.
OBSERVATION: A 62-year-old woman developed ascendant progressive paresthesia and weakness of arms and legs, 48 hours after enteritis infection. Neurological examination showed tetraparesia with loss of deep tendon reflexes and alteration of proprioception tests. Nerve conduction studies revealed polyradiculoneuritis. Then she presented an acute blindness and hypertension. Brain magnetic resonance imaging showed bilateral occipital lesions and subarachno…

This report documents congenital stationary night blindness (CSNB) in two non-Appaloosa horse breeds (Thoroughbred and Paso Fino). History of vision impairment since birth, normal ocular structures on ophthalmic examination, and electroretinographic findings were consistent with CSNB. In one horse (Thoroughbred), a 9-year follow-up was carried out. In the Paso Fino, severe vision impairment from birth to approximately 1 year of age in both dim and bright light situations led to humane euthanasia and histopathologic confirmation of the disorder.

Onchocerciasis (river blindness) is a serious public health problem with important socioeconomic consequences. The presence of onchocercal skin lesions is unsightly and has a psychosocial effect on the affected. This cross-sectional study was undertaken to assess the perception and social implication of onchocerciasis in the village of Apana in Nigeria. Apana is a rural community, hyper-endemic for onchocerciasis and currently receiving ivermectin treatment. The village is one of the ten communities of Uzairue district of Etsako West Local Government Area (LGA), Edo State, Nigeria. There are m…

Extended set of visual evoked potentials (VEPs) using pattern-reversal (PREPs), linear motion-onset and radial (expansion) motion-onset stimuli (M-VEPs) (detailed specification at http://www.lfhk.cuni.cz/elf) was used to verify congenital stationary night-blindness (CSNB) characteristics in 7 patients (compared to 7 age matched controls) in photopic conditions (luminance of 17 cd/m2). No differences were found in any of the M-VEPs, whilst PREPs displayed prolonged latencies in 3 of 7 CSNB patients. Additionally, the PREPs and M-VEPs were tested in 3 normal and 3 CSNB subjects (the only availab…

AIMS: To determine the overall reported incidence and causes of registrable blindness and visual impairment in the West of Scotland and any trends that have occurred in the previous 16 years since data from the same area were published.
METHODS: Data for analysis were obtained from BP1 registration forms returned to the Resource Centre for the Blind serving the Strathclyde region in the West of Scotland between 1 April 1996 and 31 March 1997.
RESULTS: A total of 1595 visually handicapped people were registered during the study year. Of these, 99 forms (6.2%) were excluded from further analys…
