[Ultrasound imaging and localization of metal implants. A clinical study].

In a first experimental study we investigated the visualisation of metal implants in reference to dimension, shape, material (titanium, steel, biodegradable screws) and surface structures and an eventual change of the echo pattern in correlation to the surrounding structures, the vicinity to bone and the change of the sound angle. For this purpose ultrasound investigation was performed in artificial and cadaver bones and in cadaver limbs after implantation of screws, plates, K-wires and cerclage wires. We found that metal implants of a certain dimension can be localised by typical artefacts (r

Capitolunate arthrodesis with compression screws.

Scapholunate dissociation or scaphoid pseudarthrosis may lead to wrist osteoarthritis. When osteoarthritis concerns the midcarpal joint, proximal row carpectomy is not possible. Only 4-corner or capitolunate arthrodesis may be indicated. In this procedure, pseudarthrosis was frequently described in literature. However, in these series, fixation was performed with pins or staples. Type and position of the device are important to obtain carpal bones fusion. The efficiency of compression screws has been validated in scaphoid fracture or pseudarthrosis. Moreover, the axial position of the screws,

Unilateral biportal endoscopic technique combined with percutaneous transpedicular screw fixation for thoracolumbar burst fractures with neurological symptoms: technical note and preliminary report.

BACKGROUND: Previous studies on thoracolumbar fractures with neurological symptoms have focused on how to achieve satisfactory fracture reduction, adequate nerve decompression, and stable spinal alignment. With the development of the minimally invasive spine surgery technique, achieving satisfactory treatment results and reducing iatrogenic trauma at the same time has become a new goal of spinal surgery. This research used percutaneous transpedicular screw distraction to partially reduce the fractured vertebrae, followed by completing nerve decompression and reducing residual displacement bone

CT-scan Evaluation of Osteointegration and Osteolysis in Different Graft Types and Surgical Techniques for the Treatment of Shoulder Instability.

BACKGROUND: Bone graft is often needed in treating anterior shoulder instability in glenoid bone loss and graft integration is crucial in achieving good results. This study aimed to evaluate bone graft remodeling in different techniques for shoulder anterior-inferior instability. METHODS: Graft osteointegration and osteolysis were retrospectively evaluated with CT-scan imaging performed 6 to 12 months after surgery to compare the outcome of three procedures: Latarjet, bone block with allograft, and bone block with xenograft. Screw fixation and double endobuttons fixation were also compared.

Influence of screw to joint distance on articular subsidence in tibial-plateau fractures.

BACKGROUND: Tibial-plateau fractures with depressed osteochondral fragments have a high incidence of articular subsidence post reduction. Locking plates aim to prevent this via 'raft' screws below the subchondral bone. However, differences in plate design and patient anatomy result in variability of screw position in relation to the fragments they are designed to support. We evaluate the effect of screw placement and articular subsidence with this fracture pattern. METHODS: A retrospective cohort study of operatively treated tibial-plateau fractures with free osteochondral fragments was perfo

Static and dynamic guided bone regeneration using a shape-memory polyethylene terephthalate membrane: An experimental study in rabbit mandible.

BACKGROUND: Periosteal expansion (PEO) results in the formation of new bone in the space created between existing bone by expanding the periosteum. PEO has already been performed on rabbit parietal bone and effective new bone formation has been demonstrated. In this study, the utility of a polyethylene terephthalate (PET) membrane as an activator was evaluated in the more complex morphology of the mandible. METHODS: A PET membrane coated with hydroxyapatite (HA)/gelatine was placed in the rabbit mandibular bone at lower margin of mandibular molar region underneath periosteum, and screw-fixed.

The application of atlantoaxial screw and rod fixation in revision operations for postoperative re-dislocation in children.

INTRODUCTION: We evaluate the feasibility, safety, and efficacy of atlantoaxial screw and rod fixation for revision operations in the treatment of re-dislocation after atlantoaxial operations in children. METHODS: Eight consecutive children with atlantoaxial instability required a revision operation due to atlantoaxial re-dislocation caused by the failure of the initial posterior wire fixation. The children were 5-11 years of age with an average age of 8.5 years. The posterior atlantoaxial screw and rod fixation and fusion operation was then performed. Autograft bones harvested from rib (in 3

Percutaneous screw fixation for scaphoid nonunion in skeletally immature patients: a report of two cases.

Nonunion of the scaphoid waist in skeletally immature patients is rarely diagnosed. We report 2 cases of scaphoid nonunion in skeletally immature patients who underwent percutaneous screw fixation without bone graft. In stable nonunions with minimal sclerosis, percutaneous screw fixation without bone graft can be an alternative to the conventional open procedure in skeletally immature patients, with successful union and clinical outcome.

Complications of plate fixation of forearm fractures.

Sixty-four adult patients (87 diaphyseal forearm fractures) were treated by plating. Thirty-nine percent of the fractures were classified as single bone fractures (16% radius, 23% ulna); 43% were both radial and ulnar fractures, and 19% were Galeazzi or Monteggia fracture-dislocations. A major complication occurred in 18 (28%) patients. Nonunion occurred in six patients: three of 18 bones treated with four screws (17%), but only three of 69 bones fixed with five or more screws (4.3%), a nonunion rate four times higher for bones plated with four screws. Screws loosened in three fractures, all i

Stability of fixation of diacapitular fractures of the mandibular condylar process by ultrasound-aided resorbable pins (SonicWeld Rx® System) in pigs.

To assess the stability of osteosynthesis in diacapitular condylar fractures we compared fixation using ultrasound-aided resorbable pins with poly-(D,L)-lactide (SonicWeld® Rx, KLS Martin, Tuttlingen, Germany) with that of titanium screws in 20 pig mandibles, 10 in each group. Isolated diacapitular fractures were created using a surgical chisel. Ten fractures were each repositioned and fixed by two pins (17 and 11 mm long, 2.1mm in diameter), and 10 fractures were fixed by two titanium screws of equal length, 2.0mm in diameter. Shear tests were done immediately after treatment to measure the m

A Novel Titanium End-Disc-Assisted Allograft Preparation System for Anterior Cervical Corpectomy and Fusion: Development and Preliminary Biomechanical Evaluation.

Background: Allograft bone remains an attractive option for anterior cervical corpectomy and fusion (ACCF), but its use can be limited by variable graft geometry, insufficient endplate contact, and operator-dependent trimming. This study aimed to develop a titanium end-disc-assisted allograft construct with a standardized cutting and assembly system and to preliminarily evaluate its static mechanical integrity under axial compression, compression-shear, and torsional loading. Methods: The construct consisted of a controlled-length bone graft, two Ti6Al4V end-discs, and four countersunk fixatio
