A case of cavernous sinus dural arteriovenous fistula successfully treated using a chronic total occlusion wire.

BACKGROUND: In cavernous sinus dural arteriovenous fistulas (CS-dAVFs), posterior venous drainage may become occluded first, followed by anterior drainage, occasionally leading to spontaneous resolution. Immediately before this stage, however, a phenomenon known as paradoxical worsening may occur, in which symptoms rapidly deteriorate despite apparent angiographic improvement because of progressive venous outflow obstruction. In such cases, the shunt pouch (SP) becomes nearly isolated, and the lesion must be reached by penetrating the occluded cavernous sinus (CS) along a conventional access r

The cuneus vein: initial anatomical description and preservation technique for posterior interhemispheric approaches.

BACKGROUND AND OBJECTIVE: The posterior interhemispheric approach provides optimal access to lesions located in a number of areas. A major advantage of this approach is that the parieto-occipital vein is typically the only venous structure encountered, which minimizes venous interference during exposure. In a surgical series of 12 patients, we identified a previously undescribed venous variant extending from the cuneus to the falx cerebri. Here, we characterize this venous structure and propose a microsurgical strategy to preserve it. METHODS: A total of 303 posterior interhemispheric approac

Efficacy of Ventriculoperitoneal Shunting Using Keen's Point Versus Frazier's Point in Patients With Hydrocephalus: A Non-Randomized Controlled Trial From Pakistan.

Background The ventriculoperitoneal (VP) shunt is crucial for draining excess cerebrospinal fluid, preventing complications like elevated intracranial pressure. Common points for the procedure, such as Keen's and Frazier's points, are established, with emerging interest in Choudhary's point. Despite advancements, the challenge remains, as studies reveal cases with incorrectly placed catheter tips, emphasizing the ongoing need for improved ventriculostomy techniques. Objective To assess and compare the efficacy of VP shunting utilizing Keen's point versus Frazier's point in individuals diagnose

Strategies and tactics to perform safe pancreaticoduodenectomy for 94-year-old patient: report of a case.

BACKGROUND: Despite improvement of postoperative management, pancreatoduodenectomy still has a high rate of major complications. Therefore, careful assessment is critically important when we consider high risk surgery for extremely elderly patients. CASE PRESENTATION: A 94-year-old man, who suffered dark urine, epigastric pain, and loss of appetite, was diagnosed as bile duct cancer and underwent endoscopic retrograde biliary drainage. He has past history of hypertension and paroxysmal atrial fibrillation. Computed tomography (CT) showed a nodule in the lower bile duct, which was slowly enhan

Diagnosis and Treatment of a Dural Arteriovenous Fistula Involving the Superior Petrosal Vein.

OBJECTIVE: A dural arteriovenous fistula involving the superior petrosal vein (SPV DAVF) is an extremely rare condition. Therefore, its clinical presentation, imaging characteristics, treatment methods, and risk factors remain unclear. In this study, we discuss and analyze the aforementioned features of an SPV DAVF. METHODS: We retrospectively reviewed 30 patients with SPV DAVFs, with a 1-year follow-up rate of 96.67% (29 of 30). The neurological function of the patients was assessed using the modified Aminoff-Logue scale and the modified Rankin Scale score. The risk factors before and after

Autonomic dysreflexia in a tetraplegic patient due to a blocked urethral catheter: spinal cord injury patients with lesions above T-6 require prompt treatment of an obstructed urinary catheter to prevent life-threatening complications of autonomic dysreflexia.

BACKGROUND: The Manchester Triage System is commonly used as the triage system in emergency departments of the UK. As per the Manchester Triage System, patients presenting with retention of urine to the accident and emergency department are categorized to yellow, which denotes that the ideal maximum time to first contact with a treating clinician will be 60 min. Cervical spinal cord injury patients, in whom urinary catheter is blocked, may develop suddenly headache, sweating, high blood pressure, cardiac dysrhythmia, convulsions, intracranial bleed, and acute neurogenic pulmonary oedema as a r

Ovine hourly fetal urine production: relation to fetal electrocortical activity.

OBJECTIVE: Ultrasound studies of hourly urine production rate in human fetuses have suggested that a fall in urine production occurs in state 2F (fetal quiet sleep) secondary to a state-dependent decrease in renal blood flow. We sought to ascertain the relationship between fetal hourly urine production rate and behavioral state in the near-term ovine fetus, a model in which urine production and fetal brain activity can be directly measured. METHODS: Six ewes with singleton pregnancies were prepared with vascular and amniotic fluid catheters. Fetuses were prepared with hindlimb vascular cathet

A simple relationship between radiological arteriovenous malformation hemodynamics and clinical presentation: a prospective, blinded analysis of 31 cases.

OBJECT: The authors sought to establish prospectively whether there is a simple relationship between radiological features of brain arteriovenous malformation (AVM) hemodynamics and a patient's clinical presentation. METHODS: Thirty-one consecutive patients with AVMs underwent cerebral angiography at 3.8 frames/second during each standardized injection of contrast material. Contrast dilution curves were derived from the image sequences by using regions of interest (ROIs) traced on arteries feeding and veins draining the AVM nidus. Angiographic parameters were then analyzed in a blinded fashio

Intracranial venous hypertension and the effects of venous outflow obstruction in a rat model of arteriovenous fistula.

A model of rat arteriovenous fistula (AVF) was created using a proximal common carotid artery to distal external jugular vein anastomosis. Anatomical dissections revealed that the external jugular vein is the primary vessel draining intracranial venous blood. Physiological measurements were made with the AVF open and closed, and during venous outflow occlusion of the contralateral external jugular vein. Opening the AVF increased torcular pressure from 6.5 +/- 0.6 to 13.5 +/- 1.1 mm Hg and decreased mean arterial pressure from 82.7 +/- 1.8 to 62.8 +/- 1.8 mm Hg (both P less than .05), decreasin

Journal of Global Health's Guidelines for Reporting Analyses of Big Data Repositories Open to the Public (GRABDROP): preventing 'paper mills', duplicate publications, misuse of statistical inference, and inappropriate use of artificial intelligence.

In recent years, global accessibility to large 'big data' repositories that enable 'open research' - such as the UK Biobank, National Health and Nutrition Examination Survey (NHANES), and Global Burden of Disease (GBD) datasets - has created unprecedented opportunities for researchers worldwide to conduct secondary data analyses. This development is particularly beneficial for early-career researchers in low- and middle-income countries (LMICs), as it lets them access large and otherwise costly datasets without the need for local infrastructure, potentially curbing brain drain. However, throug…

Use of the intrathoracic pressure regulator to lower intracranial pressure in patients with altered intracranial elastance: a pilot study.

OBJECT: The intrathoracic pressure regulator (ITPR) is a novel noninvasive device designed to increase circulation and blood pressure. By applying negative pressure during the expiratory phase of ventilation it decreases intrathoracic pressure and enhances venous return, which increases cardiac output. It is possible that the ITPR may both decrease intracranial pressure (ICP) and increase cerebral perfusion pressure (CPP) in brain-injured patients by decreasing cerebral venous blood volume and increasing cardiac output. The authors conducted an open-label, "first-in-humans" study of the ITPR i

Treatment of a persistent iatrogenic cerebrospinal fluid-pleural fistula with a cadaveric dural-pleural graft.

BACKGROUND CONTEXT: A cerebrospinal fluid (CSF)-pleural fistula is a unique condition with which all spine surgeons need to be familiar, particularly those who use anterolateral approaches to the thoracic region. When direct suturing of the dural defect is not possible, techniques for indirect repair must be considered. PURPOSE: To report a novel technique for repair of a thoracic CSF-pleural fistula with a cadaveric dural-pleural graft after failure of initial treatment with Duragen (Integra, Plainsboro, NJ) and fibrin glue. Comparison of micro-CT image enhancement after use of different vascular casting agents.

BACKGROUND: Microvascular visualization is crucial in understanding the mechanisms of several pathologies. For instance, visualization of the tumor microenvironment is important in understanding angiogenesis and role in cancer progression. Visualization would provide insights to cancer diagnosis, predicting metastatic growth, and evaluating therapeutic protocols. Similarly, understanding the microvascular network could be beneficial for study of degenerative diseases and tissue repair. The use of microscale computed tomography (micro-CT) and vascular casting agents provides high-resolution ima

Clinical potential of automated convolutional neural network-based hematoma volumetry after aneurysmal subarachnoid hemorrhage.

OBJECTIVES: Cerebrospinal fluid hemoglobin has been positioned as a potential biomarker and drug target for aneurysmal subarachnoid hemorrhage-related secondary brain injury (SAH-SBI). The maximum amount of hemoglobin, which may be released into the cerebrospinal fluid, is defined by the initial subarachnoid hematoma volume (ISHV). In patients without external ventricular or lumbar drain, there remains an unmet clinical need to predict the risk for SAH-SBI. The aim of this study was to explore automated segmentation of ISHV as a potential surrogate for cerebrospinal fluid hemoglobin to predict

Conical drill bit for optimized external ventricular drain placement: a proof-of-concept study.

OBJECTIVE: Despite external ventricular drain (EVD) procedures being commonplace in neurosurgical practice, suboptimal placement rates remain high, and complications are not uncommon. The angle of the EVD catheter insertion and the accuracy of the drill hole placement are major factors determining successful EVD placement that are dependent on the drill bit morphology. The standard cylindrical 2-fluted twist drill bit creates a relatively deep and narrow drill hole that requires precise positioning, has limited visibility of the drill hole bottom and restricted catheter angular adjustment rang
