No objective benefit from steroids inhaled via a spacer in infants recovering from bronchiolitis.

A double-blind randomized placebo-controlled trial was conducted to investigate the efficacy of 3 months' inhaled steroids delivered via a spacer device with face mask attachment to infants recovering from bronchiolitis. Forty-eight previously healthy infants recovering from their first documented episode of acute bronchiolitis were randomized to receive 150 microg fluticasone propionate (FP) b.i.d. or placebo delivered via the Babyhaler spacer. Longitudinal assessments were performed on seven occasions over 1 yr based on symptom diaries and health records, clinical examinations, overnight cou…

Bronchiolitis obliterans in children with Stevens-Johnson syndrome: follow-up with high resolution CT.

About one third of children with Stevens-Johnson syndrome have pulmonary involvement. As a consequence of airway epithelial injury, bronchiolitis obliterans can occur in these patients. Two cases of Stevens-Johnson syndrome-associated bronchiolitis obliterans in children were diagnosed and followed by high resolution CT without open lung biopsy. Serial changes of high resolution CT features of bronchiolitis obliterans are discussed and the literature is reviewed.

T cell receptor biases and clonal proliferations among lung transplant recipients with obliterative bronchiolitis.

Obliterative bronchiolitis (OB) is the most serious late complication of lung transplantation, but the pathogenesis of this disorder has not been elucidated. We sought evidence that OB is mediated by a cellular immunologic response by characterizing T cell antigen receptor beta-chain variable gene (TCRBV) repertoires in lung allograft recipients. Expression levels of 27 TCRBV among recipients were determined by multiprobe RNase protection assay after PCR amplification. In comparison to recipients with no evidence of rejection (n = 9), the PBL TCRBV repertoires of OB subjects (n = 16) exhibited…

Development of bronchiolitis obliterans syndrome in recipients of heart-lung transplantation--early risk factors.

Given the internationally recognized definition of bronchiolitis obliterans syndrome (BOS) and longer follow up of heart-lung transplant recipients, it is possible to establish some of the major risk factors for development and progression of BOS. Between April 1984 and 31 December 1993, 157 patients underwent heart-lung transplantation; 126 survived at least six months after operation and so were at risk of developing BOS. The following early risk factors were assessed for development of BOS grade 1 (21-35% decline in FEV1) and progression from grade 1 to grade 2 (36-50% decline in FEV1): age…

Persistence of respiratory syncytial virus genome and protein after acute bronchiolitis in guinea pigs.

Children with acute respiratory syncytial virus (RSV) bronchiolitis often develop sequelae of recurrent wheezing and asthma. To determine whether RSV persists within the lung after resolution of acute bronchiolitis, we examined the lungs of guinea pigs 60 days after intranasal inoculation with either human RSV (n = 10) or uninfected cell culture supernatant (n = 11). Evidence of viral persistence within the lung was determined by viral culture to test for replicating virus, immunohistochemistry to test for viral protein, and the reverse transcriptase-polymerase chain reaction (RT-PCR) to test …

CT findings in bronchiolitis obliterans organizing pneumonia (BOOP) with radiographic, clinical, and histologic correlation.

The CT features of 12 patients with bronchiolitis obliterans organizing pneumonia (BOOP) were reviewed and correlated with clinical history, histologic specimens, and chest radiography. From our series, a spectrum of CT findings of parenchymal lung involvement in BOOP emerged. Focal nodular or mass-like opacities were found in 42% (5 of 12). Areas of consolidation resembling pneumonia were seen in 33% (4 of 12). Peripheral subpleural reticular opacities were identified in 25% (3 of 12). Patchy ground glass infiltrates were seen in 8% (1 of 12). One patient demonstrated a mixed pattern consisti…

Latent sensitisation to respiratory syncytial virus during acute bronchiolitis and lung function after recovery.

To determine whether latent sensitivity to respiratory syncytial virus antigen(s) occurs after infection, 27 infants with acute bronchiolitis were studied and compared with 15 hospital controls. Blood was collected for whole blood challenge, and histamine release was measured by a high performance liquid chromatography technique with fluorometric detection. There was a significantly greater histamine release to respiratory syncytial virus antigen(s) in those with bronchiolitis than in controls, expressed either in amount (median 154 nmol/l compared with 104 nmol/l) or percentage release (media…

Prioritising Recommendations for Conversion to Living Mode in the Australasian Bronchiolitis Guideline: A Modified Delphi Study.

BACKGROUND: The Australasian Bronchiolitis Guideline (ABG) provides evidence-based recommendations for managing one of the most frequent reasons for infant hospitalisation. As traditional guidelines age, they suffer from lack of relevance when new evidence is available but not incorporated. To address this limitation a guideline, or parts thereof, can be converted into a "living" mode, where new evidence is regularly incorporated into recommendations, enabling faster translation to practice. This study aimed to prioritise ABG recommendations for conversion to a living mode. METHOD: Members of…

Global trends and advances in pediatric bronchiolitis treatment in the last two decades: a bibliometric analysis.

BACKGROUND: Bronchiolitis, a leading respiratory disease in infants, causes substantial global morbidity. This is the first study to employ bibliometric analysis to explore global trends and progress in pediatric bronchiolitis treatment, addressing a previously unexamined gap. METHODS: A comprehensive literature search was conducted in the Web of Science Core Collection on January 31, 2025, encompassing publications between 2004 and 2024. The bibliometric analysis utilized VOSviewer, CiteSpace, and the R package "bibliometrix" for data visualization, network analysis, and the identification o…

Optimizing the Management of Pediatric Bronchiolitis: A Multi-cycle Clinical Audit at a Regional Hospital in Oman.

Background Bronchiolitis is a major cause of lower respiratory tract infection and hospital admission among infants. Although international guidelines consistently recommend supportive care as the primary management approach, significant variation in clinical practice persists, often resulting in avoidable investigations, treatments, and healthcare expenditure, contributing to unnecessary costs and potential patient harm from avoidable interventions. Objective The objective of this study is to evaluate the effectiveness of a multi-cycle clinical audit in improving adherence to evidence-based b…

Treatment Options for Bronchiolitis Obliterans Secondary to Hematopoietic Cell Transplantation in Children: A Systematic Review.

Bronchiolitis obliterans (BO) is a chronic obstructive lung disease affecting bronchioles and smaller airways, often secondary to infection, hematopoietic cell transplantation (HCT), or lung transplantation. Despite its associated morbidity and mortality post-HCT, no standard treatment exists. This systematic review aimed to compare treatment outcomes for post-HCT BO in children. Eligible studies were retrieved from PubMed, Scopus, and Web of Science using terms "bronchiolitis obliterans" and "hematopoietic stem cell transplantation," including clinical trials, cohorts, cross-sectional studies…

Pirfenidone for the treatment of bronchiolitis obliterans syndrome related to chronic graft-versus-host disease.

Bronchiolitis obliterans syndrome (BOS) is a severe form of chronic graft-versus-host disease (cGVHD) following allogeneic hematopoietic cell transplant (HCT), with a 5-year survival rate of 40%. Currently, there is no curative therapy for BOS. Preclinical data suggest that pirfenidone, an antifibrotic drug, may benefit small airway fibrosis in HCT-associated BOS. A single-arm, open-label, 56-week phase 1 trial with 56-month extension evaluated pirfenidone's tolerability, safety, and efficacy in patients with BOS. Efficacy was measured using pulmonary function tests (PFTs), quantitative comput…

An Autopsy Case of Bronchiolitis Obliterans Associated With Oral Lichen Planus and Non-Hodgkin Lymphoma.

We report the autopsy case of a 66-year-old woman with indolent B-cell non-Hodgkin lymphoma (NHL) who underwent repeated chemotherapy for 18 years. Two and a half years ago, she developed oral lichen planus and began corticosteroid treatment. Lichen planus recurred repeatedly with corticosteroid reduction. Obstructive ventilation impairment and respiratory failure had progressed two years ago. She was admitted to the hospital due to cytomegalovirus infection and died of respiratory failure. An autopsy revealed bronchiolitis obliterans, oral lichen planus, and recurrence of pre-existing NHL. Th…
