INTRODUCTION: Bronchopulmonary malformations (BPM) are lower respiratory tract anomalies that include congenital malformations of the pulmonary airways (CMPA), bronchogenic cysts (BC), bronchopulmonary sequestrations (BPS), and congenital lobar emphysema (CLE). Prenatal detection in low- and middle-income countries is less common than in high-income ones. This study aims to show the experience in the surgical approach to BPM in a Brazilian center, with emphasis on clinical evolution and surgical results, according to the time of diagnosis (prenatal versus postnatal).
METHODS: We retrospective…

OBJECTIVE: To review the experience with surgical intervention for congenital cystic lung disease (CCLD).
METHODS: From April 1988 to December 2001, 68 patients were diagnosed and treated surgically for CCLD. There were 27 males and 41 females ranging in age from 2 to 65 years. The majority of the cases were diagnosed by history, clinical manifestations and chest radiography or computerized tomography (CT). There were 25 (36.8 %) right-sided and 43 (63.2 %) left-sided cystic lung lesions. Four patients had massive bleeding and needed urgent or emergency operation. Pneumonectomy, lobectomy, an

Improvements in prenatal imaging techniques now permit in utero treatment of potentially serious lesions. We describe three cases in which decompression of significant space-occupying thoracic lesions resulted in an excellent clinical outcome. The pregnancies were all carried to term after second or early third trimester intervention and resulted in infants with minimal respiratory distress. Cytologic and chemical analysis of the thoracic fluid aspirated in utero provided diagnostic and physiologic information.

Cystic disease of the lung should be considered in the differential diagnosis of any patient presenting with respiratory symptoms. The most important aids available to the thoracic surgeon for the evaluation of cystic disease are history, physical examination, and chest radiograph. Confirmation of diagnosis often requires computed tomography, pulmonary and thoracic aortic angiography, and upper gastrointestinal barium series.

Congenital bronchopulmonary malformations are uncommon but potentially life-threatening anomalies of infants and children. Between 1970 and 1988, 45 patients from birth to 13 years of age (23 boys and 22 girls) underwent evaluation and treatment for bronchopulmonary malformations. Thirty-seven had solitary lesions: bronchogenic cyst (n = 13), cystic adenomatoid malformation (n = 9), congenital lobar emphysema (n = 6), pulmonary sequestration (n = 6), arteriovenous malformation (n = 2), and bronchial atresia (n = 1). Eight additional patients had two simultaneous abnormalities and three patient…

A Chinese woman presented with a spontaneous pneumothorax and a family history suggestive of the autosomal dominant transmission of pneumothorax. The patient also had skin fibrofolliculomas and folliculin gene deletion, compatible with Birt-Hogg-Dube (BHD) syndrome. The importance of BHD syndrome and other familial spontaneous pneumothoraces is discussed.

Magnetic resonance imaging (MRI) already has a number of indications in exploration of the mediastinum, the most frequent of which being exploration of vascular pathologies facilitated by visualization of vascular lumina without need for a contrast medium, MRI can provide detailed information concerning the size, relations and wall thickness of these vessels ans sometimes of aneurysmal thrombosis. It also makes it possible to study aortic dissections and venous diseases of the mediastinum. As regards mediastinal masses, MRI complements computerized tomography (CT): it is better than the latter

Two cases of diffuse dendriform pulmonary ossification (DPO) are presented, one of the secondary type and the other of the idiopathic type. Case 1 was an adult female patient who underwent thoracic surgery to remove a posterior mediastinal bronchogenic cyst, which was discovered on a computed tomography scan performed after an episode of pneumonia when traction bronchiectasis with interstitial lung disease/fibrosis was also suspected in the lungs. Histologic examination performed on the resected lung tissue revealed numerous scattered small osseous spicules on a background of intense interstit…

This study aimed to retrospectively summarize the clinical signs, diagnosis, and treatment of congenital bronchial atresia (CBA) in 12 patie … sions of the bronchus central to the mucocele, emphysematous changes of the peripheral lung fields, bronchogenic cyst, and anomalous branching of the bronchial tree and vascular structure were observed. CBA was detected in the right lobe in eight patients and the left lobe in the remaining four. No surgical intervention was performed in 5 CBA patients and the remai …

PURPOSE: The goal of this study was to identify the proportion of sequestrations that were atypical or associated with other entities, such as congenital cystic adenomatoid malformations, communicating bronchopulmonary foregut malformations, bronchogenic cyst, and scimitar syndrome.
METHODS: All charts of patients with pulmonary sequestration admitted at 2 children's hospitals from 1982 to July 1999 were reviewed retrospectively. The authors included all anomalies with a systemic arterial supply or without bronchial connection.
RESULTS: Only 22 of the 39 patients (56%) had a classic isolated…

Background: Due to their nonspecific symptoms, foreign bodies in the aerodigestive tract (FBA) are often misdiagnosed as respiratory conditions, especially when unwitnessed, posing significant risks for morbidity and mortality in the pediatric population. Case Presentation: We report a case of an otherwise healthy 4-month-old, predominantly breastfed infant with episodic "honking" breathing, choking with feeds, and a 2-month history of recurrent croup episodes requiring emergency services and an hospital admission. After referral to pediatric otolaryngology/respirology, a flexible laryngoscopy
