BACKGROUND AND PURPOSE: Sinus venosus defect is a rare cardiac abnormality, provoking an interatrial shunting outside the interatrial septum. Echographic diagnosis is difficult and may require examination by a specialized cardiologist.
SUMMARY OF CASE: We report the case of a young woman who presented with repeated episodes of hemiparesis.
CONCLUSIONS: Surgical correction of sinus venosus defect led to disappearance of neurological symptoms.

BACKGROUND: Cardiogenic shock (CS) is a complex heterogeneous syndrome with various causes and phenotypes, making it difficult for clinicians to diagnose. At our institution, we found several cases of missed and late recognition of CS.
PROJECT RATIONALE: Our institutional CS work group interviewed clinicians to identify barriers to CS recognition. Thirty-minute interviews with a series of 6 scripted questions were conducted.
PROJECT SUMMARY: Fifteen interviews were conducted among the following clinicians: cardiology fellow (n = 1), emergency medicine (EM) resident (n = 3), heart failure nur

The transtelephonic transmission of electrocardiograms (ECGs) was introduced in Slovenia in 1996 and has been used regularly since September 1997 by health institutions and patients with cardiovascular diseases. Portable 12-channel ECG equipment weighing less than 200 g is generally used. The diagnostic centre is located in the intensive-care unit at the University Medical Centre Ljubljana. In the first 12 months of regular operation we received 463 calls, 213 of which were diagnostic. The most common reasons for calling were: ischaemic chest pain, atypical chest pain, palpitations and dyspnoe

During the past decade, developments in the field of DNA diagnostics have resulted in the confirmation of the genetic nature of several cardiac diseases. In a cardiogenetics outpatient clinic, a cardiologist and a clinical geneticist together evaluate persons with a (possible) hereditary cardiac disease. It is of utmost importance that patients with hereditary cardiac diseases be recognised and subsequently referred for genetic counselling as several preventive and therapeutic options are available.

A 75-year-old female patient with a history of arterial hypertension presented with oppressive chest pain radiating to the left upper limb, accompanied by dyspnea on mild exertion. Electrocardiogram showed no significant changes, but positive cardiac biomarkers prompted admission to intermediate care and coronary angiography. Findings included a small-caliber vessel originating from the pulmonary artery trunk and extending toward the right atrioventricular groove, an arteriovenous fistula between the left anterior descending artery and the right coronary artery draining into the pulmonary arte

Symptomatic non-obstructive coronary artery disease (NOCAD) is an increasingly recognised entity that is associated with poor cardiovascular outcomes. Nearly half of those undergoing coronary angiography for appropriate indications, such as typical angina, or a positive stress test have no obstructive lesion. There are no guideline recommendations as to how to care properly for these patients. Physiologic assessment of the coronary arteries beyond two-dimensional angiography is not standardised, yet it can provide valuable information in patients presenting with typical angina in the setting o

OBJECTIVES: Patients on antiplatelet therapy have a gastrointestinal bleeding risk. It is increased by risk factors. The frequency of those risk factors, the prevalence of upper digestive symptoms and their management in patients on antiplatelet agents is unknown.
PATIENTS AND METHODS: We performed an observational multi-centred prospective survey among 560 French cardiologists with private practice. Each cardiologist completed a questionnaire for the first four patients treated with antiplatelet agents in primary or secondary prevention.
RESULTS: Among the 2182 patients included, (age = 67 …

In the course of studies covering 40 years, 1913 to 1953, leading to the development of cardiopulmonary bypass, many ingenious blind instrumental or digital intrusions were made into the heart chambers to treat lesions therein. Limits were defined for the arrest of circulation through part or all of the heart to permit a brief glimpse of the lesion and effect at least a partial correction. The often remarkably good results of operations performed under less than ideal operating conditions for lesions such as isolated pulmonary stenosis encouraged the interventional cardiologist and radiologist

There is a broad spectrum of mitral valve diseases ranging from young patients with rheumatic mitral valve stenosis up to older patients with secondary mitral valve regurgitation and numerous comorbidities. A profound understanding of the etiology, anatomical characteristics of mitral valve diseases and current treatment options is necessary to be able to prepare a patient-centered treatment approach. The interdisciplinary collaboration of referring physicians, interventional cardiologists, cardiac surgeons, heart failure and imaging specialists as well as anesthesiologists is a cornerstone of…

Heart transplantation is a pivotal treatment of end-stage heart failure, and recent advancements have extended median posttransplant life expectancy. However, despite the progress in surgical techniques and medical treatment, heart transplant patients still face complications such as rejection, infections, and drug toxicity. CT is a reliable tool for detecting most of these complications, whereas MR imaging is particularly adept at identifying pericardial pathologies and signs of rejection. Awareness of these nuances by radiologists, cardiologists, and surgeons is desired to optimize care, red…

Patients with chronic total occluded coronary arteries (CTO) have worse prognosis than patients who do not have CTO. Medical therapy before embarking on invasive treatment is recommended. Invasive management with percutaneous coronary intervention or coronary artery bypass grafting is reserved for patients who receive medical treatment and have resistant angina and reversible ischaemia. Treatment should always be decided in collaboration with the patient, cardiologists, and cardio-thoracic surgeons. Follow-up treatment in the primary sector should include cardiac rehabilitation and reconsidera…

Important differences still exist in the management of patients who receive TAVR in Europe and in the USA. Provisional participation of cardiovascular surgeons and imaging cardiologists, use of TEE, general anesthesia with endotracheal intubation, and length of stay are some of the major differences which may define the future of TAVR in the USA. TAVR procedure will most likely be performed without CT surgery presence and with limited echocardiographic imaging guidance in USA in the near future as it is already being done in Europe.

Coronary artery disease is a leading cause of morbidity and mortality in the western world. Plaque rupture in an atherosclerotic lesion is the most commonly encountered underlying pathophysiology. Spontaneous coronary dissection can have similar presentation, but we as a community of cardiologists may not be aware of uncommon aetiologies, such as vasculitis presenting as ST elevation myocardial infarction (STEMI). Here we present a case report of a lady, who presented with STEMI on three occasions within five days, due to underlying granulomotosis with polyangiitis (GPA).

In 1992, the Brugada brothers, cardiologists from Spain, described 8 patients who survived sudden cardiac death and had a unique ECG pattern. Electrocardiographically characterized by a distinct ST-segment elevation in the right precordial leads. The syndrome is associated with a high risk for sudden cardiac death in young and otherwise healthy adults. Since then, thousands of cases were published. The syndrome is still under-diagnosed which has a serious implication on patient's prognosis. We described one patient who was misdiagnosed at the ER.
