[Combined atlantoaxial fractures].

PURPOSE OF THE STUDY: Combined fractures of the atlas and epistropheus account for 3 % of all acute injuries to the cervical spine. In relation to all C1 and C2 injuries this is 43 % and 16 %, respectively. The aim of this study is to evaluate a group of patients with combined C1-C2 fractures and to suggest an effective therapeutic procedure. MATERIAL: In the years 1996 to 2003, a total of 16 patients with trauma to the atlantoaxial complex were treated at the Orthopedic Department of the Third Faculty of Medicine, Charles University, Prague (1996-2001) and the Department of Spinal Surgery of

Anterior atlas fracture following suboccipital decompression for Chiari I malformation. Report of two cases.

Chiari I malformation, a congenital disorder involving downward displacement of the cerebellar tonsils through the foramen magnum, is often treated surgically by performing suboccipital craniectomy and C-1 laminectomy. The authors report two cases in which fracture of the anterior atlantal arch occurred during the postoperative period following Chiari I decompression and C-1 laminectomy causing significant neck pain. The findings indicate that interruption of the integrity of the posterior arch of C-1, iatrogenically or otherwise, confers increased risk of anterior arch fracture. A C-1 fractur

Providers and women's perspectives on opportunities, challenges and recommendations to improve cervical cancer screening in women living with HIV at Mbarara Regional Referral Hospital: a qualitative study.

BACKGROUND: Cervical cancer screening uptake remains low despite being a critical prevention method for adult women living with HIV(WLHIV). These women experience greater incidence and persistence of high-risk human papillomavirus (HPV) and severe outcomes, including cervical cancer comorbidity and death. OBJECTIVE: We explored the opportunities, challenges, and recommendations of clinical care providers and WLHIV to improve cervical cancer screening uptake among WLHIV in Southwestern Uganda. METHODS: In a cross-sectional qualitative study from January to June 2021 at Mbarara Regional Referr

[Two patients with pseudogout manifested by severe neck pain].

We reported 2 patients with pseudogout manifested by severe posterior neck pain. Patient 1 was a 78-year-old woman. She had experienced attacks of posterior neck pain several times for 4 years. On July 3, 2001, she developed severe pain in the posterior neck and left acromioclavicular joint, and was admitted to our hospital. On examination, her body temperature was 38.1 degrees C, ESR 140 mm/hr and CRP 14.7 mg/dl. Linear calcifications in meniscus of the right knee and left acromioclavicular joint were observed in roentgenograms. The CT of the cervical spines revealed multiple nodular calcific

Troubleshooting a Rare Anatomic Variation with Intraoperative Navigation in a Patient with Bilateral C2 Pars Fractures.

C2 pars fractures occur most commonly after traumatic hyperextension injuries. Although a significant number of cases may heal with conservative measures, some require surgical intervention. Anatomical variations of the V3 segment of the vertebral artery are not uncommon and may present an obstacle to safe instrumentation. Intraoperative CT-guided navigation is a useful tool in these cases, but the limitations of accuracy in the upper cervical spine especially in the context of unstable injuries must be understood to avoid complication. In this case we present a rare anatomic variation of the

Traumatic occipitocervical disruption: a new technique for stabilisation. Case report and literature review.

Traumatic atlanto-occipital dislocation in adults is usually fatal and survival without neurological deficit is rare. The surgical management of those who do survive is difficult and controversial. Most authorities recommend posterior occipitoaxial fusion, but this compromises cervical rotation. We describe a case in which a patient with a traumatic atlanto-occipital disruption but no neurological deficit was treated by atlanto-occipital fusion using a new technique consisting of cancellous bone autografting supported by an occipital plate linked by rods to lateral mass screws in the atlas. Th

Atlanto-Occipital Fusion With Complete Absent Atlas Posterior Arch and Complete Ossified Caroticoclinoid and Anterior Interclinoid Ligaments.

The skull base exhibits numerous morphologic variants, some of which are uncommon and hold clinical significance. The imaging report presents a rare instance of multiple skull base variants identified incidentally through a computed tomography (CT) scan of a 53-year-old female patient. The findings revealed a bilateral partial atlanto-occipital fusion (AOF), an absent C1 posterior arch, and a complete bilateral ossification of the caroticoclinoid and anterior interclinoid ligaments in the form of bars (CCBs and AIBs). This distinctive combination of variants has not been emphasized in existing

Superior biomechanical stability of pedicle screws compared to lateral mass screws: recommendations for bicortical positioning and enhancing bone contact in geriatric C1 vertebrae.

BACKGROUND: In atlantoaxial instabilities, posterior C1/C2 fusion using lateral mass screws (LMS) or pedicle screws (PS) in a mono- or bicortical position in the atlas is a typical treatment. The bone microstructure and positioning of the screw trajectories appear to be of significant relevance for stability. PURPOSE: The aim of this study was a comparative analysis of the mechanical durability of screw fixation concerning microstructural characteristics of the trajectories of LMS and PS in mono- and bicortical position. METHODS: Human C1 from geriatric body donors (n = 28; 50% female, age 8

Evaluation of Dural Parameters at C1 Level in Patients with Chiari 1 Malformation Following Foramen Magnum and C1 Posterior Arch Removal: Introduction of a Novel Concept to Decompress without Affecting Stability.

BACKGROUND: Chiari 1 malformation has crowding at craniovertebral junction (CVJ), treated by Foramen magnum decompression (FMD) but is associated with high failure rates, which is explained by recently introduced concept of central instability. So, we propose a new concept of relieving this crowding without affecting stability. OBJECTIVE: To derive a threshold for coring out of internal surface of C1 posterior arch instead of complete laminectomy accompanying FMD. METHODS AND MATERIAL: We prospectively included nine patients with a mean age of 25.33 ± 7.97 years, diagnosed with ACM-1 without

Prevalence of ponticuli posticus among patients referred for dental examinations by cone-beam CT.

BACKGROUND CONTEXT: Ponticulus posticus (PP) is the bony bridge that can completely or partially embrace the vertebral artery and the suboccipital nerve root at the atlas posterior arch. The PP can be a possible cause of vertigo, vertebrobasilar insufficiency, neck pain, shoulder pain, and cervicogenic headache. Moreover, the vertebral artery injury may happen during atlas lateral mass screw insertion in the presence of PP. PURPOSE: The purpose of this study was to determine the prevalence of PP in a population of patients undergoing dental cone-beam computed tomography (CBCT) and the associa
