Risk factors for proximal junctional kyphosis after pediatric spinal deformity surgery with halo gravity traction.

PURPOSE: To determine risk factors for proximal junctional kyphosis (PJK) in pediatric patients with scoliosis undergoing halo gravity traction (HGT) prior to posterior spinal fusion (PSF). METHODS: Data from consecutive patients who underwent PSF after HGT with 2-year follow-up were retrospectively collected from a single center. Patients were divided into two groups: PJK vs. no PJK. RESULTS: Twenty-five patients (age 13.6 ± 3.1 years) underwent HGT for a mean of 42 ± 37 days. Eight patients (32%) developed radiographic PJK and 1 (4%) developed proximal junctional failure. PJK patients had

Vertebral locking lesion following cervical spine fracture in ankylosing spondylitis.

Spine fractures in patients with ankylosing spondylitis frequently extend to all 3 columns, which can lead to displacement and deformity with severe instability. Cervical spine fractures occasionally cause severe kyphotic deformities, such as chin-on-chest deformities. In such cases, the patients typically exhibit a chronic progression of hyperkyphosis after the traumatic event. This article describes a unique case of ankylosing spondylitis associated with an acute chin-on-chest deformity following a spine fracture due to a vertebral locking lesion.A 60-year-old man fell while walking and sust

Burst Fracture of C5 with Traumatic Anterior Spondyloptosis of C6 and Posterior Spondylolisthesis of C4 Vertebra: A Case Report.

Burst fracture of C5 with traumatic anterior spondyloptosis of C6 and posterior spondylolisthesis of C4 vertebra is an exceedingly rare high energy injury. Treatment includes decompression, reduction, stabilization, and fusion via anterior or posterior or combined anterior-posterior approach with or without prior traction. We report this rare subaxial cervical spine injury associated with quadriplegia managed with combined anterior and posterior instrumented fusion. A multidisciplinary approach with preoperative assessment and planning is crucial in managing cervical spine injury. Immediate po

[Acute cerebral circulatory disorders of an ischemic nature in young persons with an accessory cervical rib].

Two young patients with ischemic strokes in the vertebrobasilar basin were observed. The results of the observation verified by brachiocephalic aortography and operation showed that the development of the ischemic lesion because of a circulation disorder in the vertebrobasilar basin in young people could be due to the presence of an additional cervical rib. The leading role in the pathogenesis of those disorders belongs to arterial spasms developing because of irritation of the vegetative structures of the subclavicular artery. The authors consider it reasonable to differentiate between the ad

[Anterior plating of the cervical spine with the hollow screw-plate system of titanium].

A hollow screw-plate system made of titanium for osteosynthesis of the cervical spine is presented. This system is designed to achieve a stable fixation for fusion of the vertebra. The posterior corticalis of the vertebral body need not and indeed cannot be perforated by the screws. Loosening of the screws is not longer possible because of growth of bone into the lumen of the screw and direct attachment of bone to the surface of the screw.

Strategies to prevent positioning-related complications associated with the lateral suboccipital approach.

The lateral positioning used for the lateral suboccipital surgical approach is associated with various pathophysiologic complications. Strategies to avoid complications including an excessive load on the cervical vertebra and countermeasures against pressure ulcer development are needed. We retrospectively investigated positioning-related complications in 71 patients with cerebellopontine angle lesions undergoing surgery in our department between January 2003 and December 2010 using the lateral suboccipital approach. One patient postoperatively developed rhabdomyolysis, and another presented w

Developmental morphology and ossification patterns of the C1 vertebra.

BACKGROUND: The first cervical vertebra (C1) is a unique ring-shaped structure and is largely cartilaginous at birth. Our objectives were to retrospectively analyze the age of closure of the synchondroses and determine age-dependent morphological characteristics of the C1 vertebra using computed tomographic (CT) scans. METHODS: Helical CT scans were made in fifty-four children as part of routine imaging of patients presenting to the emergency room with head injury, to exclude cervical spine trauma. Axial and sagittal reconstruction images of the C1 ring were analyzed for widths of the anterio

Sublaminar stabilization (stapling) of the caudal cervical instability in Yorkshire terrier-A case report.

This case report describes the long-term outcome of surgical treatment of rare spinal instability in small-breed dogs. A 2-year and 7-month-old male Yorkshire terrier, weighing 1.5 kg, was presented with tetraparesis and proprioceptive deficits, neck pain, and intact deep pain sensation for a clinical examination. The conservative treatment had been unsuccessful for 20 months. A diagnosis of C6-C7 instability with a slight vertebral subluxation was made, which led to the decision about sublaminar stabilization. The Kirschner wire was placed on both sides of the spinous processes from C5 to T1

Fetal development and growth of the human neck axial musculature.

Neck epaxial muscles, which are differentiated for suspending the head, occupy a large space posterior to the cervical lordosis. Limited information exists regarding developmental process that determines the muscle fiber direction and bony attachment of neck epaxial muscles. We examined histological sections of 28 human fetuses aged approximately 7-18 weeks (crown-rump length, 20-150 mm). In place of the underdeveloped lordosis, the transverse process of cervical vertebrae was shifted anteriorly at the cervicothoracic junction. The semispinalis and longissimus were distinguished by the directi…

Visualization of the Cervicothoracic Junction With EOS Imaging Is Superior to Conventional Lateral Cervical Radiographs.

STUDY DESIGN: Single-center retrospective review. OBJECTIVES: The cervicothoracic junction (CTJ) is typically difficult to visualize using traditional radiographs. Whole-body stereoradiography (EOS) allows for imaging of the entire axial skeleton in a weightbearing position without parallax error and with lower radiation doses. In this study we sought to compare the visibility of the vertebra of the CTJ on lateral EOS images to that of conventional cervical lateral radiographs. METHODS: Two fellowship-trained spine surgeons evaluated the images of 50 patients who had both lateral cervical ra

[Four cases of hangman's fracture].

Four cases of hangman's fracture were reported. The cases were disturbed between 67 and 85 years of age and all were males. The exact mechanisms of injuries of these cases were not precisely documented, but all struck the head by falling for some distance. Three of them showed cerebral concussion immediately after injuries. One of four patients had complete transverse cord lesion at C5 level and the other showed right Brown-Séquard syndrome at C4 level, while another two showed no neurological deficits except for neck pain. Radiological findings disclosed fracture of the pedicles or the lamina

Embryonic development of the head and neck: part 5, the brain and cranium.

The development of the brain is presented with respect to closure of the neural tube, the formation of the five vesicles of the brain and their derivatives, and the cranial flexures. The cranial nerves, meninges, choroid plexuses, and cerebral vasculature are also discussed. The origin and ossification of the bony elements of the head and neck, including the neurocranium, viscerocranium, and cervical vertebrae, are described, as well as major congenital defects of the brain and the cranium.

Syringomyelia associated with postmeningitic spinal arachnoiditis. Filling of the syrinx through a communication with the subarachnoid space.

A patient is described who became paraplegic because of postmeningitic spinal arachnoiditis. Twelve years later, upper limb signs appeared. Syringomyelia of an unusal communicating type was demonstrated by air myelography: Air, introduced by lumbar route, filled syrinx up to the level of the first cervical vertebra, through a communication with the subarachnoid space at the thoracic level.

[Management of complicated cervical and cervicothoracic deformities: pearls and pitfalls].

The etiology of cervical and cervicothoracic deformities is complex, often resulting in spinal cord compression and neurological dysfunction, seriously affecting patients' quality of life and life expectancy. This article focuses on the diagnosis, treatment, and clinical research of complex cervical and cervicothoracic deformities. In terms of surgical treatment, decisions are individualized based on the patient's condition, with techniques such as anterior-posterior combined approach, crossing the cervicothoracic junction for fusion and fixation, and three-column osteotomy being used when nec

Cervical and high thoracic ligamentum flavum frequently fails to fuse in the midline.

BACKGROUND: Cervical and high thoracic epidural anesthesia and analgesia have gained increasing importance in the treatment of painful conditions and as components of anesthetics for cardiac and breast surgery. In contrast to the hanging-drop technique, the loss-of-resistance technique is thought to rely on the penetration of the ligamentum flavum. However, the exact morphology of the ligamentum flavum at different vertebral levels remains controversial. Therefore, the aim of this study was to investigate the incidence and morphology of cervical and high thoracic ligamentum flavum mid-line gap

Anatomical description of a new ligamentous supporting structure of the canine cranial cervical spinal cord.

The cranial cervical vertebral column carries a unique range of mobility with the addition of dorsal and ventral flexion and rotation. The denticulate ligaments provides support and protection of the spinal cord, but little is known of the adaption of this apparatus at the cranial cervical portion of the spinal cord. We present in this publication a new pair of ligaments in dogs that supports the spinal cord inside the vertebral canal at the level of the C1-C2 spinal segments.

Extrication collars can result in abnormal separation between vertebrae in the presence of a dissociative injury.

BACKGROUND: Cervical collars are applied to millions of trauma victims with the intent of protecting against secondary spine injuries. Adverse clinical outcomes during the management of trauma patients led to the hypothesis that extrication collars may be harmful in some cases. The literature provides indirect support for this observation. The purpose of this study was to directly evaluate cervical biomechanics after application of a cervical collar in the presence of severe neck injury. METHODS: Cranial-caudal displacements in the upper cervical spine were measured in cadavers from images ta

An unusual homicidal stab wound of the cervical spinal cord: A single case examined by post-mortem computed tomography angiography (PMCTA).

We report an unusual case of homicidal stab wound of the cervical spinal cord, which illustrates the value of post-mortem computed tomography angiography (PMCTA) in cases of vascular injury. First, we noted a posterior and horizontal trajectory to the neck with complete section of the cervical spinal cord between the first and second cervical vertebrae. This lesion was accompanied by section of the right vertebral and right deep cervical arteries. We also noted an anterior cervical trajectory with an injury to the right internal jugular vein and an anterior right chest wound with a lung trajec…

Pseudogout or pseudolymphoma? Calcium pyrophosphate deposition disease of the cervical spine: a rare presentation and literature review.

Calcium pyrophosphate deposition (CPPD) disease is a crystal arthropathy primarily affecting peripheral joints, most commonly the wrist and the knees. However, CPPD in the cervical spine is a rare entity. This report describes a case of CPPD of the cervical spine which presents with symptoms of neck pain and brachalgia. A 62-year-old woman presented with left-sided upper limb and neck pain. MRI scanning revealed a low signal abnormality within the C6 and C7 vertebrae, and the possibility of lymphoma was raised. The patient was recalled for gadolinium-enhanced scans which showed perivertebral a
