[Acute cholecystitis and cancer of the gallbladder in the aged].

We are reporting 35 cases of gallbladder cancer in elderly people. 9 of them (25.7%) were operated with the diagnosis of acute cholecystitis. We point out the high incidence of the association of acute cholecystitis and gallbladder cancer, being the cholelithiasis a predominant factor in the pathogenesis of cancer. Considering that we found gallbladder cancer in 6.1% of the cholecystectomies, we advise to perform the surgical treatment in all the patients suffering of cholelithiasis.

Acute cholecystitis in the elderly. A comparative study.

Acute cholecystitis is not an unusual sequela of chronic cholelithiasis, nor is its occurrence predictable. The elderly patient with acute cholecystitis faces an increased risk of death and illness. Prompt, remedial medical preparation followed by early surgical extirpation of the gallbladder (with common bile duct exploration where indicated) is the surgical treatment of choice. Cholecystostomy should be considered for those patients in whom a more extensive procedure is judged to be too hazardous. With Americans' intense interest in maintaining a vigorous, healthy, and youthful profile and t…

Accuracy of the hepatobiliary scan in acute cholecystitis.

The accuracy of the hepatobiliary scan in the diagnosis of acute cholecystitis was determined at a major United States Army Medical Center. Forty-two patients with complete medical records were selected for the study. There was 18 positive findings and 24 negative findings. The accuracy rate for the positive findings for acute cholecystitis was 97 percent, whereas the accuracy rate for the negative findings was 100 percent. The accuracy of the hepatobiliary scan was found to be similar to that reported in the literature.

[Echography and cholescintigraphy in the diagnosis of calculous cholecystitis].

The authors note high significance of results of echocholescintigraphy in diagnosis of acute and chronic calculous cholecystitis. Preoperative echographic diagnosis of destructive changes in the gall bladder wall was shown to be possible.

Acute cholecystitis as a complication in surgical patients.

Acute cholecystitis after operation or trauma is associated with reported mortalities of 10% to 50%. During a 16-year period at the Mayo Clinic, Rochester, Minn, 75 such patients were examined, eight of whom had traumatic injuries. The incidence of this complication was approximately one for every 10,000 surgical procedures. In contrast with acute cholecystitis that occurs de novo, elderly men who had other antecedent complications seemed to be at an increased risk. Also, acalculous cholecystitis with associated gangrene and perforation of the gallbladder was more commonly encountered. The dia…

[Selection of the time of operation in acute cholecystitis].

An experience with the treatment of 760 patients enabled the authors to recommend the early operative treatment only for irreversible (gangrenous and perforative) forms of cholecystitis. Multiple cutaneous electrothermometry, counting the electrocyte index of intoxication and urgent laparoscopy are considered to be helpful for the timely diagnosis of the disease.

Cholecystitis and cholelithiasis in a tropical African population.

Compared with some European and North American countries, inflammatory disease of the gallbladder is not common in tropical Africa. The presentation as seen in our institution is different from reported series because intestinal parasites seem to play a part in the disease, whereas obesity, diabetes, multiparity and socioeconomic class seem to play little or no part. There is also a much higher incidence of gall-bladder stones in our few cases than other reported series. The predominant gallbladder stone seen is the pigment stone. As in other series, the incidence in the females is higher than
