A SHAP-Based Interpretable Model for MDRO-Infection Risk in Chronically Multimorbid Middle-Aged and Elderly ICU Adults.

AIMS/BACKGROUND: Multidrug-resistant organism (MDRO) infection poses a significant clinical challenge, resulting in poor outcomes for middle-aged and older intensive care unit (ICU) patients with chronic multimorbidity. While conventional prediction models can identify high-risk patients, they often lack transparency in clinical decision-making. To bridge this gap, we aimed to develop a prediction model that incorporates Shapley Additive Explanations (SHAP)-based explainability analysis. This approach aims to achieve accurate risk prediction while quantitatively assessing the contribution of e

What Are the Predictors for and Psychosocial Correlates of Chronic Headache After Moderate to Severe Traumatic Brain Injury?

OBJECTIVE: Although headache (HA) is a common sequela of traumatic brain injury (TBI), early predictors of chronic HA after moderate to severe TBI are not well established, and the relationship chronic HA has with psychosocial functioning is understudied. Thus, we sought to (1) determine demographic and injury predictors of chronic HA 1 or more years after moderate to severe TBI and (2) examine associations between chronic HA and psychosocial outcomes. SETTING: Community. PARTICIPANTS: Participants in the TBI Model System (TBIMS) with moderate to severe TBI who consented for additional chron

Strengthening Capacity in Primary Palliative Care: A Scoping Review from Latin America and the Caribbean.

INTRODUCTION: Primary palliative care (PPC) is widely recognized as essential for equitable palliative care access, yet evidence on its integration across Latin America and the Caribbean (LAC) remains limited. This scoping review synthesized evidence on strategies, opportunities, and challenges shaping PPC integration throughout the region. METHODS: Guided by the PRISMA Extension for Scoping Reviews and Joanna Briggs Institute methodology, peer-reviewed and gray literature published from 2014 to February 2025 were searched across eight databases and relevant sources. Using population-concept-

Formation and Growth of a Bronchiectasis and Pulmonary Nontuberculous Mycobacteria Multidisciplinary Program Using a Patient-Centered and Integrated Care Model Improves Outcomes.

BACKGROUND: Bronchiectasis and pulmonary nontuberculous mycobacteria (NTM) infections are chronic diseases that can cause debilitating respiratory symptoms, exacerbations, and even lead to respiratory failure. Clinical care has historically been provided by individual specialists, but multidisciplinary care teams have recently emerged to improve evaluation of underlying etiology and interventions that disrupt the trajectory of disease progression. RESEARCH QUESTION: Does a multidisciplinary care program reduce bronchiectasis exacerbations or improve sputum conversion rates in pulmonary NTM in

Impacts of health insurance on tympanostomy tube outcomes in the pediatric population.

OBJECTIVES: Tympanostomy tube (TT) placement is a common surgical procedure for treating pediatric patients with chronic otitis media with effusion (COME) with or without recurrent acute otitis media (rAOM). Prior work suggests children from low-income families face significant disparities in access to care for rAOM or COME. The impact of these health disparities in the care of children with rAOM or COME has yet to be investigated in a state with an expanded public health insurance model. We seek to examine differences in care for patients with rAOM and COME based on insurance status and socio

Social work practice with childbirth-injured women in Nigeria.

Nigerian social workers in health care settings have been actively involved in the treatment of women in northern Nigeria who suffer from childbirth injury during obstructed labor. The patient population studied suffers from vesicovaginal fistula (VVF), a debilitating and chronic condition whereby a woman leaks urine uncontrollably and experiences grave social consequences. Crisis intervention, social rehabilitation, fighting stigma, and discharge planning similar to the American professional model were functions of the social workers in addressing the needs of this patient population. Implica

Ten Questions on Using Lung Ultrasonography to Diagnose and Manage Pneumonia in Hospital-at-Home Model: Part II-Confounders and Mimickers.

The hospital-at-home (HaH) model offers hospital-level care within patients' homes and has proven effective for managing conditions such as pneumonia. The point-of-care ultrasonography (PoCUS) is a key diagnostic tool in this model, especially when traditional imaging modalities are unavailable. This review explores how PoCUS can be optimized to manage pneumonia in HaH settings, focusing on its diagnostic accuracy in patients with comorbidities, differentiation from mimickers, and role in assessing disease severity. Pulmonary comorbidities, such as heart failure and interstitial lung disease (

Barriers to Viral Hepatitis Elimination and New Strategies to Overcome Them in Low- and Middle-Income Countries.

Chronic infections with hepatitis B virus (HBV), hepatitis C virus (HCV), and hepatitis D virus (HDV) are major causes of cirrhosis and hepatocellular carcinoma (HCC) worldwide. The greatest burden is borne by low- and middle-income countries (LMICs). Despite the availability of highly effective preventive tools, simplified diagnostics, and affordable antiviral therapies, progress toward the World Health Organization's (WHO) goal of eliminating viral hepatitis as a public health threat by 2030 is insufficient in most LMICs. Gaps persist throughout the entire care continuum, from vaccination an

Uplifting the Social Determinants of Health of Adults Exiting Carceral Settings May Aid Health Care Access.

This article investigates health care access (i.e., health insurance coverage, usual source of care, doctor visit) among adults exiting carceral settings. Between 2022 and 2024, adults aged 18 and over (N = 202) enrolled in two health studies in San Diego, California. Surveys collected demographic and health care access data. Logistic regression models assessed factors independently associated with health care access. Overall, 27% of participants were uninsured, 66% had Medicaid insurance, and 21% lacked a usual place of care. In logistic regression models, adults released from custody within

From behavioral signals to therapy management: a real-world pilot implementation of human-in-the-loop AI workflow in proactive patient care.

BACKGROUND: Spinal cord stimulation (SCS) is an established therapy for chronic pain, but post-implant care depends on timely identification of evolving patient needs. Remote monitoring may help detect therapy-use changes that scheduled follow-up and patient-initiated contact can miss. We evaluated Proactive Intelligence, a workflow-integrated, human-in-the-loop AI recommender using passively collected SCS device-interaction data to support care without added patients' data-entry burden. METHODS: We conducted a prospective, observational real-world pilot among research-consented patients impl

A remote first nation community-informed virtual care approach to chronic back pain management: a mixed methods study.

Chronic back pain (CBP) is a widespread public health issue. There is a lack of community-based care options for Indigenous Peoples with CBP. A virtual care clinic co-designed with community was implemented in the Cree Community of Pelican Narrows using remote presence (RPR) technology. Methods We used a community-based participatory action framework and pre-post design to evaluate this intervention. Pain, quality of life and experience outcomes were measured. An assessment was provided by a local nurse practitioner and a physical therapist joining over RPR. The physical therapist provided jp

Medication management of depression: the impact of comorbid chronic medical conditions.

OBJECTIVE: This paper addresses the following question: Does quality of care for depression differ between depressed persons with and without chronic medical conditions (CMCs)? METHODS: We used a population-based mental health survey to identify respondents aged 18 to 64 with the diagnosis of major depression in the past year (N = 278). In our model, the dependent variable was guideline-level medication management of depression. Determinants for guideline-level care were modeled using multivariate logistic regression. RESULTS: Depressed persons with CMCs were significantly more likely to rec

Improving end-of-life care in the cardiac intensive care unit: Navigating the challenge of timing palliative care intervention.

BACKGROUND: Integrating end-of-life (EoL) care in cardiac intensive care units (CICUs) is particularly complex because it requires a shift from a purely curative approach to one that emphasizes symptom management, emotional and spiritual support and patient-centered care. Moreover, this transition is challenging due to the need to balance life-sustaining treatments with the goals of comfort and dignity. The concept of EoL care varies across countries and is influenced by cultural, ethical and legal factors. METHODS: This narrative review examines palliative and EoL care in critically ill card

Aging with Traumatic Brain Injury: Deleterious Effects of Injury Chronicity Are Most Pronounced in Later Life.

Understanding the effects of age on longitudinal traumatic brain injury (TBI) outcomes requires attention to both chronic and evolving TBI effects and age-related changes in health and function. The present study examines the independent and interactive effects of aging and chronicity on functional outcomes after TBI. We leveraged a well-defined cohort of individuals who sustained a moderate/severe TBI and received acute inpatient rehabilitation at specialized centers with high follow up rate as part of their involvement in the TBI Model Systems longitudinal study. We selected individuals at o

The ILD-GAP risk prediction model performs poorly in myositis-associated interstitial lung disease.

PURPOSE: Myositis-associated interstitial lung disease (MA-ILD) is associated with increased mortality, but no prognostic model exists in this population. The ILD-GAP index was developed to predict mortality risk across all subtypes of chronic ILD. The purpose of this study was to validate the ILD-GAP risk prediction model in patients with MA-ILD. PROCEDURES: We completed a retrospective cross-sectional study of patients enrolled in the Johns Hopkins Myositis Center database between 2006 and 2017. Cumulative mortality rates were estimated using the Kaplan-Meier test. Model calibration was det

Risk factors associated with hospital admission, clinical progression and 90-day mortality in adults with respiratory syncytial virus infection.

BACKGROUND: Respiratory syncytial virus has traditionally been associated with childhood illness, but it is increasingly recognised as a cause of severe disease in adults, particularly older people and those with chronic comorbidities. Despite its growing clinical relevance, evidence describing outcomes and predictors of disease severity in adults remains relatively limited. AIM: To identify factors associated with hospital admission, clinical severity and 90-day mortality in adults with respiratory syncytial virus infection. METHODS: A single-centre observational cohort study was conducted
