Large Language Model and Pediatric Neurosurgery - A Neurosurgeon's Perspective on the Artificial Nervous System.

Large language models (LLMs) are rapidly transforming healthcare, yet their implications for pediatric neurosurgery remain underexplored. This narrative review interprets LLM evolution through a neuroscientific lens familiar to pediatric neurosurgeons. We trace the parallel development of LLMs and the human brain : architecture evolving from sequential processing to attention mechanisms mirrors prefrontal cortex maturation; training stages parallel synaptic exuberance, adolescent pruning, and socialization; reasoning capabilities emerge through chain-of-thought prompting and reinforcement lear

Cognitive and brain function enhancement in Gen X group after personalized, AI supervised EEG-neurofeedback training.

Objective.Interventions supporting medical care and enhancing quality of life in neurodegenerative or age-related cognitive decline are strongly needed. Electroencephalographic (EEG) neurofeedback can enable users to modulate their brain activity through real-time feedback. However, evidence for its clinical effectiveness remains inconclusive, partly due to limited personalization and insufficient task relevance of existing protocols. We tested whether personalized EEG neurofeedback supervised by deep neural networks (DNNs) can enhance cognitive performance in older adults (Gen X).Approach.Fif

Integration of 2D and 3D Imaging Descriptors with Large Language Models for Assessing Pediatric Foreign-Body Aspiration Risk.

Background/Objectives: Foreign-body aspiration (FBA) is a common and largely preventable pediatric emergency, yet current safety standards and risk assessments rely predominantly on object size and on anecdotal descriptions and bronchoscopy findings. We propose a clinically oriented proof-of-concept workflow that combines high-resolution three-dimensional (3D) scanning and calibrated two-dimensional (2D) imaging of retrieved objects with radiomic shape descriptors and large language model (LLM) reasoning to support aspiration risk assessment and guide prevention. Methods: Objects were obtained

Heterogeneous tau deposition patterns in the preclinical stage link to domain-specific cognitive deficits.

INTRODUCTION: The spatial heterogeneity of tau deposition is closely linked to clinical variants of Alzheimer's disease (AD). Detecting these patterns in the preclinical stage is challenging, but second-generation tau tracers provide a unique opportunity to do so. METHODS: We used independent component analysis (ICA) and tau positron emission tomography (PET) imaging with the 18F-MK6240 tracer in 590 cognitively healthy adults (mean age 66.58 ± 5.13 years, 340 females) to identify tau patterns in the preclinical stage. RESULTS: Using all individuals, seven distinct patterns emerged, with med

Generating Complex Explanations for Artificial Intelligence Models: An Application to Clinical Data on Severe Mental Illness.

We present an explainable artificial intelligence methodology for predicting mortality in patients. We combine clinical data from an electronic patient healthcare record system with factors relevant for severe mental illness and then apply machine learning. The machine learning model is used to predict mortality in patients with severe mental illness. Our methodology uses class-contrastive reasoning. We show how machine learning scientists can use class-contrastive reasoning to generate complex explanations that explain machine model predictions and data. An example of a complex class-contrast

Neuropsychological disorders induced by cerebellar damage.

INTRODUCTION: Motor coordination disorders caused by cerebellar dysfunction are well known. However, the less known cerebellar neuropsychological disorders also merit attention, since they occur more frequently than one might imagine. CASE REPORT: We describe a 66-year-old patient with severe cerebellar damage caused by hemorrhagic stroke and associated with cognitive impairments (including impaired executive function, reasoning and judgment). A review of the literature on these neuropsychological disorders revealed a set of clinical, anatomical and functional imaging arguments that prompted

Neurocognitive outcomes in survivors of ALL: Risk patterns and individual profiles in a single-protocol cohort.

OBJECTIVE: Increasing survival probabilities among children and young adults with acute lymphoblastic leukemia (ALL) have led to a growing population at risk for long-term neurocognitive sequelae. This study investigated cognitive functioning among individuals treated for ALL under the Nordic Society of Paediatric Haematology and Oncology ALL2008 protocol in Eastern Denmark, including performance across multiple domains and associations with age at diagnosis, sex, time since end of treatment, hematopoietic stem cell transplantation (HSCT), and neurotoxic events during treatment. METHOD: Eight

Multi-Granularity Topological Reasoning for Anatomically Consistent Vasculature Parsing.

Quantitative analysis of retinal vascular morphology is vital for clinical decision-making and the investigation of systemic diseases. Central to this process is the accurate segmentation of retinal arteries and veins (A/V) from the background, a task challenged by substantial variations in vessel calibers and the presence of low-contrast or ambiguous structures in fundus images, especially in ultra-wide field imaging where peripheral distortions and large-scale anatomical variability are pronounced. These factors often lead to fragmented semantic representations and topological inconsistencie

Association between maternal glycohemoglobin in pregnancy and adult offspring cognition: results from the Transgenerational Effects of Adult Morbidity (TEAM) Study.

Maternal diabetes, a common pregnancy complication, has long-term implications for both mother and offspring. While the developmental origins of metabolic health from prenatal diabetes exposure are well known, cognitive consequences in offspring are still being explored. The timing of hyperglycemia during pregnancy that most affects cognitive development and whether these effects persist into adulthood remains unclear. This study aimed to determine the association between trimester-specific hyperglycemia exposure and adult cognition in the offspring of women with pregestational diabetes. The T

Parental perspectives on the quality of life of children with Down syndrome.

Down Syndrome (DS) is the most common chromosome abnormality and the most frequent cause of developmental delay/intellectual disabilities in children. Although the investigation of the quality of life (QoL) is crucial in children with DS, relatively poor attention has been paid to this topic. The current study aimed to evaluate parent-reported QoL in a group of children with DS and identify children's individual and clinical features associated with different levels of QoL. We included in the study 73 children with DS (5-12 years) and investigated the parent-reported levels of QoL by means of

Refractory Iron-deficiency anemia and intermittent melena in phenotypic type 2 Glanzmann thrombasthenia: a diagnostic challenge.

Glanzmann thrombasthenia (GT) is a rare platelet disorder caused by quantitative or qualitative defects of glycoprotein (GP) IIb/IIIa, leading to impaired platelet aggregation[1]. Classic hereditary GT, when molecularly confirmed, typically manifests with lifelong mucocutaneous bleeding, whereas gastrointestinal bleeding is uncommon and usually presents as overt hemorrhage. Here we report a case of phenotypic type 2 GT characterized predominantly by iron-deficiency anemia and intermittent melena. GT was considered a potential contributing factor based on systematic diagnostic reasoning. This c

We present a time-efficient, clinically integrated lung ultrasound protocol tailored for prehospital use by providers not yet routinely using ultrasound. Specifically designed to support the recognition of pneumothorax and interstitial syndrome, the protocol emphasizes operational feasibility, spectrum-based diagnostic reasoning, and strict time limitations to avoid delays in definitive care. It includes standardized scanning sequences, documentation, and a governance framework for training, certification, and quality assurance. Developed and implemented in an Austrian ambulance service, it ma

Enhancing the Role of Digital Health Navigators With Lived Experience: AI-Assisted Conavigation for Safety, Support, and Engagement.

As artificial intelligence (AI) expands across mental health services, questions arise about whether it can replace digital health navigators (DHNs), including those with lived experience. The DHN role generally resists automation because lived experience deepens specific dimensions that AI cannot replicate. Drawing on emerging evidence of AI's limitations in contextual reasoning, cultural sensitivity, and clinical safety, the authors identify where DHNs with lived experience are least substitutable: real-world knowledge of services, cultural grounding in the communities they serve, and relati

Mandibular condyle osteochondroma. Review of the literature and report of a misdiagnosed case.

Temporomandibular disorders can usually be diagnosed on the basis of a thorough history and a comprehensive examination of the patient. Additional diagnostic tests, such as imaging of the temporomandibular joint (TMJ) area, are mandatory and must be flawless in case of atypical findings. The aim of this report is to illustrate pitfalls in clinical reasoning and in imaging procedures in the diagnosis of temporomandibular pain and dysfunction. A case report of a patient with osteocartilaginous exostosis of the mandibular condyle, which was erroneously diagnosed and treated as an internal derange…

The approach emphasises that patterns are prompts for further clinical reasoning rather than diagnostic equations, and that escalation does not require diagnostic certainty. Beyond the Numbers provides a cognitive scaffold for moving from recognition of an abnormal FBC to proportionate clinical action. It is intended to compleme …

Detecting Stigmatizing Language in Clinical Notes with Large Language Models for Addiction Care.

RATIONALE: Recent studies have found that stigmatizing terms can incline physicians to pursue punitive approaches to patient care. The intensive care unit (ICU) contains large volumes of progress notes that may contain stigmatizing language, which could perpetuate negative biases against patients and affect healthcare delivery. Patients with substance use disorders (alcohol, opioid, and non-opioid drugs) are particularly vulnerable to stigma. This study aimed to examine the performance of Large Language Models (LLMs) in the identification of stigmatizing language from ICU progress notes of pat

Integrating multimodal clinical data with a large model for prostate cancer diagnosis.

Accurate prostate cancer (PCa) diagnosis remains difficult because of tumor heterogeneity and the challenge of integrating multimodal clinical information. We developed Prost-LM, a multimodal large language model that jointly embeds MRI-derived features, numerical PSA values, and free-text clinical reports into a unified semantic space to enable deep cross-modal reasoning. Trained and validated on a large multi-center cohort of 3940 patients, Prost-LM achieved strong diagnostic performance, with an internal validation AUC of 0.954 for distinguishing PCa from benign conditions, outperforming MR
