CASE: A 34-year-old patient was involved in a motorcycle accident, resulting in an anterior hip dislocation associated with an open book pelvic fracture and compression to the femoral artery. He required an emergent closed reduction of the hip dislocation and open reduction and internal fixation of the pelvic fracture.
CONCLUSION: This case underscores the importance of maintaining a high level of suspicion for anterior hip dislocation, particularly in patients involved in motorcycle accidents, where a flexed, abducted, and externally rotated hip is common. Timely diagnosis allows for prompt

CASE: We encountered a case of entrapment of the ulnar nerve and artery associated with displaced distal radius and ulna fracture. After the closed reduction, a patient noted a severe sensory deficit and had claw hand. Intraoperatively, the proximal edge of the fractured radius was positioned in the space between the ulnar artery and the nerve, and the nerve was entrapped. The entrapment was released surgically with a good 1-year outcome.
CONCLUSION: In cases of distal radius fracture, there is a possibility of entrapment of the ulnar nerve. When neurological symptoms worsen after the reducti

Subtalar dislocation is the simultaneous dislocation of the distal articulations of the talus at both the talocalcaneal and talonavicular joints. It can occur in any direction and always produce significant deformity. Most common is the medial dislocation. Less common presentations are lateral, anterior and posterior dislocations. These dislocations are associated with osteochondral fractures. Closed reduction and immobilisation remains the mainstay of treatment. Proper radiographs and CT scan confirm the post-reduction alignment stability of subtalar joints and intraarticular fracture fragmen

A lateral Monteggia fracture in a child is reported. Between 1941 and 1977, only 16 such cases were reported in the literature. Of these, 2 cases needed open reduction of the fractured ulna. The present case needed osteotomy of a nearly healed fracture of the ulna and reconstruction of the orbicular ligament to stabilise the reduction of the radial head. No similar case has previously been described in a child. Prompt diagnosis and early reduction should yield good results by closed methods of treatment, but open reduction is necessary if the diagnosis is missed and treatment is delayed. Even

OBJECTIVE: Traditionally, rhinoseptoplasty for nasal bone fracture is only considered after an unsatisfactory outcome from initial closed reduction. However, better surgical outcomes may be achieved if rhinoseptoplasty is performed at the same time as the nasal bone fracture reduction. This study investigated the surgical outcomes of patients who underwent rhinoseptoplasty concomitantly with nasal bone fracture reduction according to their computed tomography image-based nasal bone fracture classifications.
STUDY DESIGN: Case series with chart review.
SETTING: Academic tertiary care medical

BACKGROUND: Zygomatic arch fractures can occur either as isolated fractures or as a part of multiple facial fractures. There are several techniques available for reduction of zygomatic arch fractures, which include closed reduction and open reduction with fixation using metallic splints, closed reduction via Gillies or a gingivobuccal approach is preferred for minimal invasiveness, short operative time, and low complication rate. The aim of the present study is to present a simple, easy, and reliable method for closed reduction of isolated zygomatic arch fractures.

The rate of nonunion is estimated to be 1.1% to 10% following closed treatment of proximal humerus fracture and 5.5% following closed treatment of humeral shaft fracture. Surgical management should be considered for fractures that demonstrate no evidence of progressive healing on consecutive radiographs taken at least 6 to 8 weeks apart during the course of closed treatment. In the case of proximal humerus nonunion, recent series have demonstrated union in >90% of patients treated with reconstruction using locking plates and autogenous bone graft. Shoulder arthroplasty is reserved as a salvage

Traumatic ankle fractures and dislocations that fail closed reduction present a challenging set of circumstances that can potentially lead to unnecessary complications and require surgical intervention. Interposition of adjacent tendons occurs rarely and can obstruct the anatomic realignment. Because of the potential for neurovascular compromise and possible skin tension necrosis, an irreducible fracture dislocation must be addressed with open reduction and internal fixation. The present case details an unusual, low-energy, external rotation ankle fracture and dislocation that was incapable of

We reviewed the use of a low radiation portable fluoroscopy unit in 100 patients. The most common indication was closed reduction of distal radial fractures. Fracture and joint stability were assessed on the real-time monitor and stored on videotape. Static images were stored on thermographic paper. Fluoroscopically guided joint injections and localization of implants, foreign bodies and bone tumours were performed. Fluoroscopy is a useful adjunct to arthroscopic assisted fracture reduction and other arthroscopic procedures such as distal ulnar resection. These new generation units produce sup

Results of a study show a 20% incidence of nonunion of fractures of atrophic edentulous mandibles. Accurate bony reduction is essential in elderly paatients with thin mandibles. Closed reduction should be tried first as open reduction tends to limit vascular supply to the fracture. Maintenance of periosteal coverage seems advisable if open reduction is necessary. Bone grafting should be considered unless contraindicated because of the patient's age or physical condition. Wire mesh is preferable to bone plates for stability.

Injuries to the Lisfranc complex range from purely ligamentous disruptions to fracture-dislocations of the tarsometatarsal joint. Treatment options include closed/open reduction with percutaneous pinning, open reduction and internal fixation (ORIF), and primary arthrodesis. We present a ligament reinforcement technique utilizing a flexible fixation device for the treatment of ligamentous Lisfranc injuries. Level of Evidence: Level V, expert opinion.

Fracture of the nasal skeleton is a relatively common injury. Satisfactory treatment requires appropriate skills, diagnosis, and management. Some injuries require immediate attention; others are better treated in delayed fashion. Closed reduction provides satisfactory treatment in the majority of nasal fractures; however, open reduction often is a more appropriate choice. Injuries to the nasal septum should be recognized and given proper treatment.

BACKGROUND: Although many pediatric Monteggia fractures can be treated nonoperatively, the presence of any residual radiocapitellar subluxation following ulnar reduction mandates a more aggressive approach to restore and maintain ulnar length. In younger children, restoration and maintenance of ulna length may be achieved through intramedullary fixation of the ulnar shaft.
DESCRIPTION: A Steinmann pin or flexible intramedullary nail is introduced percutaneously through the olecranon apophysis and advanced within the medullary canal to the ulnar fracture site. If necessary, the ulnar length an

OBJECTIVES: To investigate whether a conventional fracture hematoma block (FHB) or an ultrasound-guided peripheral nerve block has more superior analgesic effect during nonoperative management of distal radius fractures in an emergency department setting. Two peripheral nerve block types were investigated, one at the level of the elbow, or cubital nerve block (CNB), and another an axillary nerve block (ANB).
DESIGN: Two prospective randomized controlled studies were performed to compare the difference in pain intensity during closed reduction of a distal radius fracture between FHB-, CNB-, an

Ulnar nerve palsy is a rare complication of closed midshaft forearm fractures; only 8 cases have been reported. This article describes a case of ulnar nerve palsy associated with a midshaft forearm fracture. A 12-year-old girl sustained a right midshaft forearm fracture. Whether she had a peripheral nerve injury was unknown due to strong pain. She underwent emergency manual reduction and intramedullary pinning. However, ulnar nerve palsy was remarkable postoperatively and gradually worsened. Therefore, neurolysis was performed 9 weeks later. The nerve had adhered to surrounding scar tissue. Si

OBJECTIVE: Closed skeletal traction (CST) to reduce unilateral locked facets in the subaxial cervical spine can expedite spinal realignment prior to definitive surgery but is not always successful. What predicts successful closed reduction is not completely understood. In addition, whether open anterior or posterior surgery is superior for achieving successful reduction after failed closed skeletal traction has not been investigated. The authors sought to assess predictors of successful closed reduction with skeletal traction and to compare the efficacy of anterior versus posterior surgery aft
