Infection with Chlamydia trachomatis lymphogranuloma venereum serovar L1 in homosexual men with proctitis: molecular analysis of an unusual case cluster.

Among 767 rectal isolates of Chlamydia trachomatis obtained from men over the period 1981-1991, 7 were found to be a rare lymphogranuloma venereum serovar, L1. These isolates were from rectal specimens taken from five male patients in 1982-1983. Six of the seven isolates were available for DNA sequencing studies. All six of these isolates shared the same DNA sequence in the major outer membrane protein (MOMP) gene variable domains and had different MOMP sequences than did the prototype L1, L2, and L3 strains, suggesting a point source outbreak. All five patients infected with serovar L-1 were

Impact of Long-term Stability on Mental Health in Adults with Atopic Dermatitis: A Cohort Study with Unsupervised Cluster Analysis.

Atopic dermatitis (AD) is a heterogeneous disease characterized by unpredictable flares, potentially leading to impaired mental health. Adult patients with a dermatologist-verified AD diagnosis received after their 18th birthday were grouped according to patient-reported flare activity between 2018 and 2023. The k-means clustering algorithm was used in combination with dynamic time warping and barycenter averaging. Clusters were analysed according to current levels of anxiety and depression and severity across the 4 time points. The clustering method divided the 262 patients into 4 groups. Clu

Comfort-Focused Assessment and Patient-Reported Experience After Regional versus General Anaesthesia: Post Hoc Analysis of a Cluster-Randomised Trial.

INTRODUCTION: Postoperative pain management typically relies on numerical (NRS) or analogue rating scales. However, exclusive focus on nociception may overlook broader aspects of patient experience, especially under regional anaesthesia (RA). This post hoc analysis of the COMFORT trial (NCT05234216) examined whether replacing the NRS with a comfort-focused scale affects patient-reported experience, opioid use, and related outcomes. METHODS: We analysed data from a cluster-randomised trial in 29 French centres. Adults undergoing elective non-cardiac surgery managed in the post-anaesthesia care

Looking at the big picture: demonstrating benefits of Bayesian latent cluster spatio-temporal analysis for understanding maternal prescription opioids misuse.

BACKGROUND: As the opioid crisis rages, maternal prescription opioid misuse (MPOM) is particularly concerning because it influences both the mother and child’s wellbeing. However, little was known about the trend of MPOM and its county-level risk factors. Potentially varying relationships between MPOM and local environmental factors across time and geospatial context constitute analytic challenges. OBJECTIVES: (1) Employ Bayesian latent cluster modeling to detect spatial clusters in each of which the temporal association between MPOM rates and local structural risk factors is varying and uniqu

Profiling crash-associated factors and injury risk patterns among lost-in-thought (daydreaming) drivers: a combined cluster-sequence analysis approach.

Cognitive distraction, particularly in the form of being lost in thought or daydreaming, is a significant yet underexamined contributor to traffic crashes. This study investigates crash patterns and risk profiles associated with drivers who experienced lost-in-thought distraction at the time of a crash in Washington State. Using cluster correspondence analysis (CCA), distinct associations between crash characteristics and driver attributes were uncovered. In addition, process mining was employed to identify typical sequences of crash events. Three meaningful clusters emerged. Cluster 1 involve

Comprehensive analysis of RNA methylation-related genes to identify molecular cluster for predicting prognosis and immune profiles in bladder cancer.

m6A, m5C and m7G are common types of RNA methylation modifications that are widely involved in key mechanisms regulating malignancy. However, the role of RNA methylation-related genes in the immune microenvironment of bladder cancer (BLCA) remains elusive. In this study, we established RNA methylation molecular subtypes by analyzing the TCGA and GEO datasets. Risk model and nomogram were constructed by LASSO and multivariate Cox regression analysis and validated by external datasets. Genetic variations, functional enrichment analysis and immune cell infiltration were analyzed. The expression l

InSeq analysis of defined Legionella pneumophila libraries identifies a transporter-encoding gene cluster important for intracellular replication in mammalian hosts.

Legionella pneumophila is an intracellular bacterial pathogen that replicates inside human alveolar macrophages to cause a severe pneumonia known as Legionnaires' disease. L. pneumophila requires the Dot/Icm Type IV secretion system to deliver hundreds of bacterial proteins to the host cytosol that manipulate cellular processes to establish a protected compartment for bacterial replication known as the Legionella-containing vacuole. To better understand mechanisms apart from the Dot/Icm system that support survival and replication in this vacuole, we used transposon insertion sequencing in com

Association between polymorphisms of immune response genes and early childhood caries - systematic review, gene-based, gene cluster, and meta-analysis.

BACKGROUND: Early childhood caries is a significant public health concern affecting about 600 million children globally. The etiology of early childhood caries can be explained as an interplay between genetic and environmental factors. Single nucleotide polymorphisms are the most common variations in the human genome. Genetic variations of immune response genes can modify the defense response of the host, and alter the susceptibility to bacterial colonization of the oral cavity and early childhood caries. The aim of this systematic review is to identify genetic variants of immune response gene
