Combating opportunistic infections: coccidioidomycosis.

Coccidioidomycosis is a growing problem in the southwestern US. Although most cases of symptomatic coccidioidomycosis occur in healthy individuals, individuals with depressed cellular immunity are at particular risk. These include individuals with HIV infection, those who have undergone allogeneic transplantation and others on immunosuppressive medications. Most instances of primary pulmonary coccidioidomycosis do not require antifungal therapy. However, almost all cases of disseminated coccidioidomycosis will require such therapy. The triazole antifungals, fluconazole and itraconazole, are ef

Coccidioidomycosis of the knee diagnosed by fine-needle aspiration: a case report.

A case of septic arthritis from coccidioidomycosis in a 62-year-old man is described. The diagnosis was made by fine-needle aspiration and supported by positive cultures and exoantigen testing. Coccidioidomycosis can infect bones and joints, especially the knee. This case is presented to increase awareness of involvement of the knee by this organism, and to demonstrate the feasibility of diagnosis by fine-needle aspiration. The cytologic findings of coccidioidomycosis of the knee are described and discussed.

The incidence and implications of coccidioidouria.

To estimate the frequency and significance of isolating Coccidioides immitis from urine specimens, we reviewed the records of 75 patients with various forms of coccidioidomycosis of whom 29 had urine cultures for fungi. The 7 patients (24%) who had coccidioidouria appeared similar as a group to those whose cultured urine did not yield C. immitis. Over half of the patients had no other evidence of extrathoracic spread, and in 2 patients with coccidioidouria, illness resolved without treatment. Usually only a few colonies of the fungus were isolated from 200-ml specimens. Thus, our use of a conc…

Experimental irradiated arthrospore vaccine against coccidioidomycosis in mice.

An experimental irradiated ((60)Co) arthrospore vaccine against coccidioidomycosis protected approximately 75% of mice from death after an intraperitoneal challenge sufficient to kill approximately 90% of the nonimmunized control mice. Although the majority of the immunized mice became infected with Coccidioides immitis, the histologic lesions were substantially less severe than those in the nonimmunized controls, particularly in the pulmonary region. Although arthrospores irradiated with 1, 2, or 3 million roentgens lost their ability to multiply in various laboratory media (probably through …

The Return of Delayed-Type Hypersensitivity Skin Testing for Coccidioidomycosis.

A skin test that detects dermal hypersensitivity in persons with past infection with Coccidioides species is again available for clinical use. Nearly all of the clinical studies with similar materials were published prior to the 1990s, and as a result, many practicing physicians will be unfamiliar with how skin testing for coccidioidomycosis might be useful in patient management or as a research tool. We review clinical and epidemiological studies with past skin test antigens, the composition of past and current skin test preparations with particular attention to differences in the preservativ…

Disseminated coccidioidomycosis associated with extreme eosinophilia.

Primary coccidioidomycosis is frequently accompanied by eosinophilia in the range of 5%-10% of the peripheral white blood cell count. Dissemination of Coccidioides immitis to organs such as skin, bone, joints, and CNS usually is associated with risk factors such as sex (male), race (non-Caucasian), pregnancy, and immunosuppression. We report a case of coccidioidomycosis in an otherwise healthy African-American man with 72% eosinophilia who had dissemination to the skin, and we review cases in the literature of disseminated disease associated with eosinophilia. Marked eosinophilia may be an imp…

A rare case of 'coccidioidoma' of the breast.

Coccidioidomycosis has been reported in almost every tissue of the human body. The gastrointestinal tract and breast have been uniquely spared from this fungal infection. Despite the exceedingly large number of screening mammographies that are performed every year, to our knowledge subcutaneous breast tissue involvement as the sole presentation or of secondary spread of coccidioidomycosis has not been reported. We describe a patient who showed an unusual manifestation of a coccidioidal breast nodule simulating a neoplasm. The patient had been transiently immunosuppressed by prednisone therapy

Early Experiences with Intrathecal Administration of Amphotericin B Liposomal Formulation at a Neurosurgical Center.

BACKGROUND: Intrathecal administration of amphotericin B represents an important adjunctive therapy for management of severe fungal meningitis. Intrathecal preparations have traditionally used amphotericin B deoxycholate. Liposomal amphotericin B is an alternative formulation with good clinical outcomes as systemic therapy, but scant data exist investigating intrathecal use. OBJECTIVE: The aim of this exploratory study was to evaluate outcomes following intrathecal administration of liposomal amphotericin B for treatment of severe fungal meningitis. METHODS: A national shortage of amphoteric

Regional Increases in Incidence of Coccidioidomycosis (Valley Fever) - Arizona, 2005-2022.

Incidence of coccidioidomycosis (Valley fever), a fungal infection caused by inhalation of Coccidioides species spores, has increased substantially across the southwestern United States in association with increasing aridity, warming temperatures, and precipitation volatility. Arizona and California report >95% of U.S. coccidioidomycosis cases, and incidence in Arizona has increased statewide. Patterns within Arizona's distinct climatological regions have not been characterized, especially in regions outside the known zone of persistently high levels of disease occurrence (hyperendemicity) in …

DECTIN-1 stalk length determines binding ability, immune response, and survival in a mouse model of coccidioidomycosis.

Coccidioidomycosis is a leading cause of pneumonia in the endemic area of the Sonora desert and the central valley of California. The causative agent are the fungi Coccidioides immitis and C. posadasii. While most infections ultimately resolve without treatment, some infections spread to the periphery and cause serious and even fatal illness. Much work has shown that a T cell response with production of interleukin-17A and interferon ɣ is critical. Less work has focused on innate responses that ultimately set the stage for the adaptive response. Genetic analysis in both mice and man have shown…

Multisystemic Imaging Features of Coccidioidomycosis.

Coccidioidomycosis, a disease induced by the dimorphic fungi Coccidioides immitis and Coccidioides posadasii, is endemic to the southwestern United States and occurs with a myriad of clinical and imaging manifestations, which range from limited pulmonary disease to disseminated multiorgan infections. Humans are infected when arthroconidia in the soil are disturbed and dispersed into the air. Once inhaled, arthroconidia form spherules, which trigger a host cell-mediated immune response. If this response is adequate, it results in granuloma formation, halting the infection. However, an insuffici…

Musculoskeletal Coccidioidomycosis in the Setting of Adalimumab: A Case Report.

Musculoskeletal coccidioidomycosis is a rare disseminated fungal infection caused by either Coccidioides immitis or Coccidioides posadasii endemic to the southwestern United States and northwestern Mexico, as well as Guatemala, Brazil, and other locations in Central and South America. Symptomatic primary infection of coccidioidomycosis can present as pneumonia with influenza-like symptoms, but the majority of cases remain asymptomatic. When dissemination occurs, the most common extrapulmonary sites include the skin, lymph nodes, musculoskeletal system, and meninges. We present a case of a 53-y…

Donor-derived endemic mycoses after solid organ transplantation: A review of reported cases.

BACKGROUND: Donor-derived endemic mycoses are infrequently reported. We summarized the clinical characteristics and outcomes of these infections to provide guidance to transplant clinicians. METHODS: Multiple databases were reviewed from inception through May 31, 2023 using endemic fungi as key words (e.g., Coccidioides, histoplasma, blastomyces, talaromyces, paracoccidioides). Only donor-derived infections (DDI) were included. RESULTS: Twenty-four cases of DDI were identified from 18 published reports; these included 16 coccidioidomycosis, seven histoplasmosis, and one talaromycosis. No cas…

Disseminated coccidioidomycosis in a patient with juvenile idiopathic arthritis receiving infliximab.

BACKGROUND: Coccidioides immitis is a dimorphic fungus endemic to the arid climates of the Southwest United States, Mexico and parts of Central and South America. Human infection occurs through inhalation of spores with less than half of exposures progressing to a symptomatic state that primarily consists of pulmonary manifestations. Disseminated coccidioidomycosis is exceedingly rare, occurring in fewer than 1 % of symptomatic infections. Through hematogenous spread, the fungus can infect most organ systems and may be fatal without systemic antifungal treatment. Individuals with impaired cell…
