Modiolar Proximity of Slim Modiolar Electrodes and Cochlear Duct Length: Correlation for Potential Basis of Customized Cochlear Implantation With Perimodiolar Electrodes.

OBJECTIVES: Recent studies have shown that cochlear duct length (CDL) varies among individuals and could significantly influence the final position of the electrode and its trajectory in the cochlea. Given this, we hypothesized that the degree of modiolar proximity of novel slim modiolar electrodes, such as CI532 and CI632, can also be affected by CDL. To test this hypothesis, we retrospectively evaluated individual CDL to determine if there is any significant correlation of CDL with degree of modiolar proximity. METHODS: Fifty-one ears from 38 subjects implanted with slim modiolar electrodes

Affective nonverbal vocalizations and implications for cochlear implants: A scoping review.

BACKGROUND: Nonverbal vocalizations (NVVs) like laughs, cries and screams are powerful carriers of affective meaning and trigger rapid attentional responses. Evidence linking specific acoustic cues of adult NVVs to affective perception remains fragmented across heterogeneous studies in typically-hearing listeners, and largely absent in cochlear‑implant users. OBJECTIVE: To map empirical evidence linking acoustic cues of adult NVVs to emotion and salience perception in typically‑hearing listeners, and to determine whether comparable evidence exists for cochlear implant users. METHODS: Followi

Cochlear Implant Candidacy Support Tool Using Conjunctive Consolidation.

IMPORTANCE: Despite the proven benefits of a cochlear implant, utilization rates remain low. Current screening tools have improved awareness but rely on binary classification (candidate vs noncandidate), limiting individualized counseling and shared decision-making. OBJECTIVE: To develop a risk stratification system for cochlear implant candidacy based on routine audiometric data, enabling individualized estimates of cochlear implant candidacy likelihood, supporting improved shared decision-making. DESIGN, SETTING, AND PARTICIPANTS: This retrospective cohort study including adults with heari

Long-Term Use of Cochlear Implants in the Older Adult Population.

OBJECTIVE: Hearing loss affects 60-70% of elderly people, with ∼10% of these potentially eligible for cochlear implantation (CI). While outcomes in older adults have been positive, concerns remain regarding long-term auditory performance due to higher incidence of comorbidities, reduced cortical plasticity, and cognitive decline. This study evaluates 10-year speech perception outcomes and potential mediating factors in adults ≥60 years old at implantation. METHODS: A retrospective chart review identified 98 patients implanted between 2006 and 2014 who were ≥60 years old at surgery with ≥10 ye…

Consonant perception and error patterns in children and adults with cochlear implants.

This study investigated the effects of hearing history on consonant perception scores and confusion patterns in early implanted youth (i.e., child-implanted) and post-lingually implanted adults (i.e., adult-implanted), as well as in participants with normal hearing (NH) who listened to spectrally degraded (vocoded) stimuli. Vocoded consonant perception improved with chronological age in children with NH. For the cochlear implant (CI) users, more auditory experience and shorter durations of deafness were associated with better consonant perception scores, especially in child-implanted listeners

Is the Presence of eCAP Intraoperatively a Predictor of Cochlear Implant Outcomes in Adults?

INTRODUCTION: The electrically-evoked compound action potential (eCAP) is commonly used to assess the physiological integrity of the auditory nerve's first neuron in cochlear implant (CI) users. Electrical stimulation of this neuron is essential for transmitting auditory signals to the auditory cortex, enabling the recognition of acoustic cues. OBJECTIVE: To determine whether the presence of intraoperative eCAP influences CI outcomes in adult individuals with pre- or postlingual deafness. METHODS: The present retrospective cross-sectional study included adults with hearing loss of various et

Towards a Personalized Vestibular Assessment in Older Patients with Cochlear Implant.

Background: Age-related vestibular decline frequently accompanies presbycusis, and older adults undergoing cochlear implantation (CI) may be particularly vulnerable to postoperative dizziness due to a reduced compensatory capacity and a higher burden of comorbidities. Although CI is an effective treatment for severe-to-profound sensorineural hearing loss in the elderly, its impact on vestibular function remains a critical concern. This study aimed to compare pre and postoperative vestibular performance in older patients (≥65 years) versus younger adults undergoing CI in order to identify the r…

The neural characteristics influencing literacy outcome in children with cochlear implants.

Early hearing intervention in children with congenital hearing loss is critical for improving auditory development, speech recognition and both expressive and receptive language, which translates into better educational outcomes and quality of life. In children receiving hearing aids or cochlear implants, both adaptive and potentially maladaptive neural reorganization can mitigate higher-level functions that impact reading. The focus of the present study was to dissect the neural underpinnings of the reading networks in children with cochlear implants and assess how these networks mediate the

Binaural fusion in single-sided deaf cochlear implant users.

OBJECTIVE: Single-sided deaf (SSD) cochlear implant (CI) patients often have difficulty with spatial perception, which may be partly due to limited fusion of acoustic and electric hearing. In the present study, we investigated binaural fusion in SSD CI patients. METHODS: Eight adult SSD CI patients participated in the study. Binaural fusion was measured using speech and non-speech stimuli. Listeners indicated whether they heard 1 or 2 auditory images, the diffuseness of the sound, and lateralization of sounds in a virtual space. RESULTS: Binaural fusion and similarity ratings were highly var

Exploring the McGurk Effect in Cochlear-Implant Users: A Systematic Review.

Speech perception is a neurocognitive process that involves both auditory and visual modalities to interpret the meaning of spoken utterances. The cohesive integration of visual and auditory information (AV integration) improves speech perception. AV integration can also occur even when incongruent auditory and visual information is presented, known as the McGurk effect. The McGurk phenomenon signifies the importance of visual articulatory cues (such as place of articulation) and auditory information for speech perception. The McGurk effect can be decreased or absent in the deaf population eve

A Concept for MRI-Based Cholesteatoma Detection in Cochlear Implant Recipients.

INTRODUCTION: Cochlear implantation is the treatment of choice for severe hearing loss and deafness. Cholesteatomas can cause this deafness. A frequently used procedure in the course of surgical rehabilitation is a subtotal petrosectomy combined with a cochlear implant. The clinical follow-up of residual cholesteatomas is related to the blind sac closure difficult. Cholesteatoma MRI sequence-related CI magnet artefacts make follow-up challenging. Recent developments in combining cochlear implants and necessary MRI examinations enable the assessment of the internal auditory canal and cochlea. T…

Threshold-distance functions predict speech recognition with cochlear implants.

Cochlear implantation shows unexplained outcome variability. Among the key factors affecting outcomes measured by speech understanding is the functional state of the auditory nerve and the amount of its degeneration due to hearing loss. In the present study we actively varied the distance of the stimulating electrodes from the modiolus (the spiral ganglion), quantified it using fluoroscopy and cone-beam computed tomography and related it to electrically-evoked compound action potentials (eCAPs) in human subjects. Stimulation was monopolar. The distance from modiolus could explain up to 91% of …
